The North Carolina Division of Water Resources conducted an inspection of the single family treatment
system located at 6906 Innesbrook Way on November 21, 2019. This inspection was conducted to verify
that the facility is operating in compliance with the conditions and limits specified in the subject NPDES
permit and Certificate of Coverage. A summary of the findings and comments noted during the inspection
were provided to you in the inspection letter dated December 6, 2019.
The Compliance Evaluation inspection was conducted by Division of Water Resources staff from the Raleigh
Regional Office. The following violation(s) were noted during the inspection:
Inspection Area Description of Violation
Effluent Sampling Failure to monitor the effluent.
In the inspection letter dated December 6, 2019, you were requested to collect a representative sample of
the effluent, have it analyzed by a certified commercial lab and submit the results to this office no later than
January 24, 2020. In addition, the letter requested you to provide a written response regarding the type of
chlorine tablets you use in the treatment system. As of the date of this letter, this office has not received
the requested written response nor the lab results. The Division of Water Resources may pursue
enforcement action for this and any additional violations of State law.

A Notice of Violabon/Intent to Issue Civil Penalty is being issued for the noted violation of North Carolina
General Statute (G.S.) 143-215.1 and the facility's NPDES permit. Pursuant to G.S. 143-215.6A, a civil
penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed against any person
who violates or fails to act in accordance with the terms, conditions, or requirements of any permit issued
pursuant to G.S. 143-215.1.
To prevent further action, please respond in writing to this office within 30 days upon your
receipt of this Notice of Violation regarding your plans or measures to be taken to address the
indicated violations and other identified issues, if applicable.
