The following is in response to the Notice of Violation dated January 31, 2020.
➢ Incident#201901622, 2022 Carr-Purr drive Raleigh, NC
On October 5, 2019 The Towns on call personal were dispatched to possible sewer bubbling up
from the ground. Upon arriving the crew found that it was sewer coming from the ground every
time that pump station number 21 would come on and start pumping. The sewer water was
coming from around a new communication box that had just been installed by a contractor.
Locates were called in and the force main was dug up and found that it had been pulled apart by
the communication contractor and left with out notifying anyone of what they had done. The
contractor was not onsite when the on-call person arrived
Method of Abatement
The pump station was immediately turned off and the spill stopped. The pump station was able
to hold the sewer because it was Saturday and many of the businesses in the business park were
not open. Locates were called in and the force main was dug up and repaired. The pump station
was turned back on and no more leaks were found. The area was backfilled, and the impacted
area cleaned up
Clean up procedures
The area was washed down and the impacted area was limed seeded and strawed after the force
main was repaired. No impact was observed t the receiving stream.

The self-reported Sanitary Sewer Overflow (SSO) 5-Day Report submitted by Town of Fuquay-Varina indicates
violations of permit conditions stipulated in the subject permit and North Carolina G.S. 143-215.1. Violations include
failing to effectively manage, maintain, and operate the subject collection system so that there is no SSO to the
land or surface waters and making an outlet to waters of the State for purposes of G.S. 143-215.1(a)(1), for
which a permit is required.

a written response to this Notice of Violation. Your response is to be received by the regional office within 15
business days following receipt of this violation. Please include any additional documentation about this incident(s)
in the response. The submittal will be considered in determining whether the Division will assess a civil penalty for
the cited violations.
