Permit Number: NCG210409 Wake County Dear Mr. Williams: Dee Freeman Secretary On July 9, 2012, Thomas Ascenzo and Mack Wiggins of the Raleigh Regional Office (RRO) conducted a compliance inspection (CEI) of Quercus Forest Products Stormwater program, The assistance of yourself, was appreciated as it facilitated the inspection process. The Basin -Wide Information Management System (BIMS) inspection report is attached. The following observations were made: 1. The Quercus Forest Products Company is authorized to discharge stormwater from its facility located at 7634 Old Highway 1, New Hill, NC. 2. The Review of your Stormwater Pollution Prevention Plan(SP3) revealed the following: A. Employee training is being conducted and documented as observed with signed and dated forms located in the notebook. Last training was conducted on June 20, 2012. B. It was indicated that no spills took place in a three year period, Please document this fact and include a page inside the SP 3 manual. C. A responsible party list needs to include phone numbers and a list of companies that could be called for cleanup in case of a large spill from incoming trucks or equipment. This information should also be posted in a location for the employees. 3. Qualitative Monitoring Forms completed for January 2012 were observed inside SP3 manual. It was indicated that due to low rainfall it has been difficult to conduct monitoring semi annually. 4. Analytical Monitoring was noted for November 4, 2011. Lab bench sheets were checked and found to be within benchmarks. It was indicated that due to low rainfall. It was explained that it is difficult to collect enough sample volume due to low rainfall events. 5. All Outfalls were inspected and were dry with no flow observed. Cut vegetation near Outfall #2 to ease observation and sampling. It is suggested that you install signs identifying all outfalls locations. 6. Stormwater Pollution Prevention Plan (SP3) is reviewed and updated annually. The inspection revealed the SP3 has been updated according to permit. It'is'recomm tided that you add tabs that identify the respective section of the SP3 manual. This will enhance inspection efforts, locating items easier and help overall use of manual as a tool. 8. Site is well maintained and clean overall. Notification requirements are followed for both actual material releases and situations that represent a substantial threat of a release. Sections 8.1.1 and 8.1.2 outline the procedures to be followed for notifying facility personnel and outside agencies. Section 8.1.3 . describes procedures for releasing public information. Internal Notification In the event of a spill, the following procedures shall be used to notify facility personnel. 1) Notify Supervisor - The Supervisor will then investigate and assess the spill. If more than 25 gallons of oil are spilled or oil reaches a surface water body, the Supervisor will notify the Plant Manager who will in turn notify DENR. 2) Complete a Bonsal Chip Mill Spill Report Form, Appendix E.
