multimedia compliance inspection of Arcola Lumber Company Inc. on December 7, 2015 for permits and

(1) DIRECTIONS TO FACILITY: From the PRO, take Capital Boulevard/US 1 North to US401 North. On the
north/east side of Louisburg, NC -highway 561exits US-401 to the right. Travel 20.3 miles, take a left to stay on
NC-561. Travel 2 miles further on NC-56 UNC-43 to the junction of NC-43.' Turn left, North on NC-43 and
continue 2.3 miles to Arcola Lumber on the left. The main office is close to the road by the mailbox. The "scaling
it
office" is 100 yards further down the road past the timber storage area. The plant contact, Mr. Harris, is located in
the scaling office (a small 14'x16' building 100 feet back from the road).
(In FACILITY DESCRIPTION: Arcola Lumber Company processes lumber primarily for crate and pallet
manufacturing, and operates under Air Permit No. 04184R09. The hours of operation are 7s30-5:00 M-Th, and 7:30
-11:30 Friday. Arcola operates two cyclones for control of particulate matter and fugitive dust as a result of their
wood cutting operations. **No special safety equipment required.**
(III) INSPECTION SUMMARY: On December 7, 2015, I, Maureen Conner along with Ray Williams (DWM)
and Cory Larsen (DWR) as part of a multimedia pilot inspection program, conducted an inspection at
Arcola Lumber. We meet with Gary Harris and his daughter to conduct our inspection. A copy of the current air
permit, and the cyclone inspection logbooks were found and reviewed in the Office. I then proceeded to inspect the
cyclones. Neither of the cyclones was operating, but appeared to be in good order with no patches on any of the..
ductwork. An alternate telephone at the Log Scale House is (252) 257-1139. Gary said that 2013 was the worst
year they have had only producing 8,000 units.

(V) SPECIFIC PERMIT CONDITIONS:
Al. Must comply with 2D .0202, .0512, .0521, 0535, and .0611. Appeared to be in compliance. See specific
conditions below for more detail.- '
A.2 2D .0202 Emission Inventory Requirement. The company is required to submit an emission inventory
within 90-days of their permit expiration. The facility is aware of this condition and will be required io submit a
CY2015 AQEI as part of their next renewal package.
A.3 2D .0512 Particulates from Wood Products Finishing Plants. Requires the permittee to have a wood dust
collection system so not to discharge particulate matter caused by the working, sanding, or fmishing of wood.
Appeared to be in compliance: The plant had two collection systems hooked to the two cyclones, respectively. The
facility is mostly''clean and the ducts appear to be well maintained. They did have a leak last month which they
repaired. ..
A.4 213.0521 Control of Visible Emission. VE from the permitted items at the facility shall not exceed 20%..
opacity. Appears to be in compliance. VE was 0% opacityfrom cyclone ES-02..
A-5 2D .0521 Control of Visible Emission. VE from the permitted items at the facility shall not exceed 40%
opacity. Appears to be incompliance. VE was 0% opacity from cyclone ES-01..

A-6 2D .0535 Excess Emission Reporting and Malfunctions. When particulate, visible, sulfur dioxide, volatile
organic compounds, odorous and /or visible emissions exceed environmental regulations for more than 4 hours, the
NCDAQ must be notified. Appeared to be in compliance. The facility stated that they had not operated in an upset
state.
A-7 2D .0540 Control of Fugitive Dust Emissions. Fugitive Dust shall not cause substantive complaints or
visible emissions beyond the property boundary. Appeared to be in compliance. No complaints have been
reported.
A-8 2D .0611 Cyclone Requirements. Establish inspection, maintenance and recordkeeping requirements for
the cyclones at the facility. Appeared to be in compliance. The company is keeping paper records.
(VI) COMPLIANCE HISTORY: The facility received a NOV on February 8, 2007 for failure to submit an
Emissions Inventory for a permit renewal.
(Vln EXEMPT EMISSION SOURCES: No new exempt sources were found.
(VIII) 112(r) APPLICABH.ITY: There is no indication that chemicals are stored in threshold volumes.
(XI) STACK TEST REVIEW: No record of testing found.
(IX) CONCLUSIONS/RECOMMENDATIONS: At the time of the inspection, this facility appeared to be in
compliance with all permitted requirements. It is recommended that the facility be re -inspected in two (2) years.

Inspection Summary:
A multi -media program compliance inspection was conducted by staff from the Division of Water Resources, Air Quality,
and Waste Management on December 7 2015 to evaluate compliance with NPDES stormwater and air quality permits -
issued to the facility. The stormwater inspection results for NCG210379 are presented below.
The stormwater pollution prevention plan (SPPP) was reviewed and contained most components but did not contain a
suitable site map with outfall locations and drainage areas, nor did it have a list of significant spills (site walkthrough
revealed fuel spills around fuel tank/pump). The SPPP needs to be reviewed and updated accordingly.
The facility has a, primary ouffall location from a small earthen detention basin that was installed to treat runoff from log
storage and spray areas. Logs are typically wetted only during summer months and the wastewater generated from this
activity is permitted under NCO088587'for discharge of combined stormwater and wastewater flows. A second outfall
location was observed between a storage building and stacked logs to the northwest of the pond. This outfall must also be
monitored. See file for map of outfall locations.
Qualitative monitoring of the pond SOO was performed up to 2013 but has not been documented since. Facility personnel
were instructed to resume this monitoring. Analytical monitoring has been performed at the pond outlet but is also required
under NPDES wastewater permit at this location. Stonnwater analytical monitoring for TSS, COD, and measurement of
total rainfall must beconductedsemi-annually at this SDO and the newly identified SDO going forward. Personnel stated
that no vehicle maintenance in excess of 55 gal/mo oil usage is conducted onsite therefore VM analytical monitoring is not
required:.. - - -
Other significant compliance issues included the following:..
- A diesel fuel tank and pump was observed with heavy staining of surroundingsoils with visible product on the surface
from either'a significant spill/release of petroleum and/or poor oversight and maintenance of fueling operations and/or lack of
an available spill kit onsite. This area requires cleanup of all contaminated soil and provision of a spill kit. Impacted soil .
must be properly disposed of.
- ! The detention pond has accumulatedsolids such that its available volume for settling has been significantly reduced.
Sediment must be excavated from the pond and properly stabilized onsite to return the pond to its design volume.
- Petroleum free product and sheen was observed on the detention pond. Facility personnel could not explain the source
but it's possibly related to releases at the fuel tank (above) or other sources that could not be determined at the time of
inspection. Absorbent booms should be deployed immediately to remove as much product as possible and an investigation
made to determine its source. Inspectors noted some sheen on headwaters of the unnamed tributary south of facility which
likely was a combination of naturally occurring sheen from'iron bacteria and smaller amounts of petroleum sheen from pond
discharge.

Dear Mr. Harris:
On March 8, 2011 Myrl Nisely of the Raleigh Regional Office carried out an inspection with the help of
Faye Woodruff and yourself. The facility was compliant. Attached is the inspection checklist.
Discussion points were these:
1. Documents for the permit were made available after searching through several folders.
Organizing them in the existing Stormwater Pollution Prevention Plan (SP3) binder by following
the paragraphs in Part 11 Section A of the permit will help anyone attempting to locate certain
pieces of the information in the future. A copy of the DWQ inspection checklist has been
emailed as the suggested sequence of topics. Please be aware that the SP3 is to be reviewed and
updated as needed annually.
2. Maintenance of the ditches and culverts that carry stormwater to the pond was needed to clear
away solids. This will be a frequent task. In the same way, removing solids from the first
several feet of the pond must be done periodically to allow water to freely flow into the pond and
to maintain room in the pond for settling.

In accordance with your application for a discharge permit received on February 19, 2007, we
are forwarding herewith the subject certificate of coverage (COC) to discharge under the subject state -
NPDES general permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual
permit application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate of coverage. This permit does not affect the legal requirements to obtain
other permits which may be required by the Division of Water Quality or permits required by the
Division of Land Resources, Coastal Area Management Act or any other federal or local governmental
permit that may be required.
With respect to the wet decking operation you mentioned in your NOI application, this permit
does not cover wet decking. You will need to apply for a separate waste water permit if you intend to
conduct wet decking operations. If you have any questions concerning this permit, please contact Bill

This Certificate of Coverage shall become effective March 30, 2007.

North Carolina General Statute 143-215.6 b (i) provides that:
Any person who knowingly makes any false statement, representation, or certification in any application, record, raped,
plan or other document filed or required to be maintained under Article 2'1 or regulations of the Environmental
Management Commission implementing that Article, or who falsifies, tampers with or knowingly renders inaccurate any
recording or monitoring device or method required to be operated or maintained under Article 21 or regulations of the
Environmental Management Commission implementing that Article, shall be guilty of a misdemeanor punishable by a
fine not to exceed $10,000, or by imprisonment not to exceed six months, or by both. (18 U.3.C. section 1001 provides
a punishment by a fine of not more than $10,000 or imprisonment not more than 5 years, or both, for a similar offense.)
