On November 14, 2018 a site inspection was conducted for the Boise Cascade facility located at 1000
North Park Drive in Person County, North Carolina. A copy of the Compliance Inspection Report is
enclosed for your review. The site visit and file review revealed that the subject facility is covered by
NPDES Stormwater General Perm it-NCG210000, Certificate of Coverage-NCG210288. Permit coverage
authorizes the discharge of stormwater from the facility to receiving waters designated as Mitchell Creek,
a Class C, waterbody in the Roanoke River Basin.
As a result of the ins the facility was found to be in compliance with the conditions of the
NCG210000 pe mit. Please refer to the enclosed Compliance Inspection Report for additional comments
and observations made during the inspection.
Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil
penalty assessment of up to $25,000 per day for each violation. If you or your staff has any questions,

Inspection Summary:
The following updates to the SP3 plan are requested. Update the site map to include any new structures, stormwater or
otherwise.
Update the section of the SP3 dealing with onsite spills so there is a notation of no spills for years that have no spills.
Update the SP3 plan to include any improvements made to improve the site(stormwater)
Update the responsible parties in the plan
In addition, Clean up all spilt oil in the old an -compressor room and remove all containers with accumulated oil. Test the drain
in the old air compressor room to insure its been plugged. Sample the outfall pipe from the new products storage area and
submit results to Thad Valentine in the RRO fro review.
Boise Cascade has implemented numerous additional measures to improve water quality. Shelters have been built over roll
off containers and the drum area. Saw dust piles for spill cleanup have been placed in covered bins. All large shelter has
been built over the product sorting area and in additional misc. bins around the facility

On January 7, 2011, Autumn Romanski, of the Division of Water Quality (DWQ), Raleigh Regional Office
(RRO), inspected Mark Line Industries in (Person County) Roxboro, NC. Thank you for your time and
assistance to complete the inspection.
Georgia-Pacific Wooj Products owns and operates a laminated veneer lumber (I -beams andfloorjoists)
manufacturing/assembly facility in Roxboro, NC. Stormwater runoff from this activity is regulated by General
Permit NCG210000 and DWQ has therefore issued Georgia-Pacific Wood Products in Person County,
certificate of coverage, NCG210288 dated August 1, 2008 and expires July 31, 2013.
The facility was in compliance with the permit requirements. The permittee was very knowledgeable on the
permit requirements and has kept excellent records. The facility stores and uses unadulterated wood chips as a
fuel source. The facility stores logs treated in log tunnel (softening/water reuse process) for use in production of
final products.. The facility has a stormwater pond and three outfalls. DWQ approved representative outfall
status for 002 and 003 at outfall 001. The stormwater pond and outfalls were observed with no noted concerns
the day of the inspection. It was recommended that the facility change the location of their stormwater sampling
(as we reviewed the current sample location was inclusive of runoff sources from adjacent property owners) to a
location just before stormwater flow leaves the permittees' property at outfall 001. It was also recommended
that the facility continue to maintain stabilization BMPs (rip -rap, rock dam) and continue keeping storm ditches
clear of woody debris. It is our goal to insure the quality of the surface waters of this State.

Inspection Summary:
The facility keeps excellent records, the acting plant manager was very knowledgeable on the permit requirements, and
the facility was compliant with the requirements of the permit the day of the inspection.
It was brought to my attention by the Plant Manager that Mitchell Creek (stream index 27-123-4) is listed on the 303D List
for the Neuse River Basin, BUT the Mitchell Creek (stream index 22-58-12-6-3) subject in this permit is in the Roanoke
River Basin and IS NOT on the 303D List, therefore the conditions of the permit referring to 303D listed waters are not
currently applicable to this facility.

Comment: Twice per year monitoring is required for facilities storing exposed sawdust,
wood chips, bark, mulch, or like materials for more than a week. Monitoring Reports for
2009 and 2010 were reveiwed and the facility is compliant with this requirement. The
location for sampling was reviewed and it was determined that the sample point for
outfall 001 should be taken at the outfall 001 sw discharge before the flow leaves the
property boundary and enters a separate storm drain by the railroad. An intermittent
stream flows through the same area, but is not a drainage or stormwater outfall
associated with this permit, so the flow from it should not be included in the sample.
The March 2009 TSS result required a Tier I response that was completed and all other
samples have been within range.

Comment: This facility has three outfalls. The facility received representative outfall
status for Outfall 002 and 003, with sampling being conducted at outfall 001. A
stormwater pond on site overflows to Outfall 001. There was no flow the day of the
inspection, but leaf liter and some iron bacteria were present in the stagnant water.
There is a drainage area by the facility parking lot/roadside with a rock dam that
appears to store and filter stormwater before the stormwater sheet flows through the
woods. It is recommended that the facility continue to maintain this storage area/rip-rap
and rock dam located near an intermittent stream, as it prevents sediment loss and
bank rilling.

In response to your renewal application for continued coverage under stormwater General Permit NCG210000 the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater General Permit. This permit has been reissued pursuant to
the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the slate of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007.
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of stormwater General Stormwater Permit NCG210000
• A copy of the Technical Bulletin for the General Permit
• Five copies of the Discharge Monitoring Report (DMR) Form
• Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater
discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all
the changes in the reissued permit.
The more significant changes to the General Permit. include the following:
• Permit cover page, second paragraph — The General Permit now may cover activities that DWQ determines to be similar in
either the process, or in the exposed materials, to the Timber Products Industry.
• Part I Section A — A new provision that facilities draining to 303(d) listed waters, or in watersheds with an approved TMDL,
may not be eligible for continued coverage at the next renewal of the General Permit.
• Part I Section B second paragraph — A new clarification that the permit does not authorize discharges that DWQ determines
are wastewaters.
• Part II Section A —The required contents of the Stormwater Pollution Prevention Plan have been expanded and clarified.
• Part 11 Section B Table 1 — A new requirement to sample stormwater discharges twice per year for those facilities with
exposed piles of certain materials remaining on site longer than seven days.
• Part II Section B Tables 2 and 3 and following — For permittees with exposed piles, there are new provisions requiring the
permittee to execute Tier One and Tier Two response actions, based on the first benchmark exceedence (Tier One) and the
second consecutive benchmark exceedence (Tier Two). Tier Two requires that the permittee institute monthly monitoring
instead of twice -per -year monitoring, until three consecutive monitoring events show no benchmark exceedences.
• Part II Section B — For permittees with exposed piles, a new provision that four exceedences of any particular benchmark
will trigger increased DWQ involvement in the permittee's stormwater management and control actions. DWQ may direct
the permittee to apply for an individual permit, or may direct the implementation or installation of specific stormwater control
measures.
• Part II Section C — Clarification that under the qualitative monitoring provisions of the permit, the permittee is obligated to
respond to repeated observations of stormwater pollution. DWQ may impose additional stormwater management
requirements if the permittee is non -responsive, or if the responses are ineffective.
Your coverage under the General Permit is transferable only through the specific action of DWQ.

Your coverage under this general permit is not transferable except after notice to DWQ. The Division
may require modification or revocation and reissuance of the Certificate of Coverage. This permit does
not affect the legal requirements to obtain other permits which may be required by the Department of
Environment and Natural Resources, or relieve the permittee from responsibility for compliance with any
other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or decree.

This certificate of coverage shall become effective May 1, 2003.

on March 31, 2(H)3. The Division staff is currently in the process of rewriting this permit and is SChednlCd to have
the permit reissued by early spring of 2003. Once the perntit is reissued, your facility would he cligihle for
continued coverage under the reissued permit.
In order to assure your continued coverage under the general penult, you must apply to the Division of Wafer
Quality (DWQ) liar renewal ol'ynur permit coverage. To make this renewal process easier, we me informing you in
advance that your pennit coverage will be expiring. Enclosed you will find a Permit Coverage Renewal
Application Form. The application must be completed and rCltlr"ned by October 2, 2002 in order to assure
continued coverage under the general perntit. Due to staff and budget constraints, letters confirming our receipt of
the completed application will not he sent.
Failure to rCyucst renewal within the time period specified, may result in a civil assessment of at least $250.00.
