Meeting held to review Stormwater Pollution Prevention Plan, and
make any necessary changes to program.
Present were Jerri Cain, Steven Howard, William Wilkinson and
maintenance staff.
Changes were made to include updated phone numbers for supervisors
and a list of companies with phone numbers for clean up of possible
large spills.
It was also determined that next week, June 13, and June 14 meetings
will be held with all employees on training on who to properly respond
to spills. Documentation of these departmental meetings will become
part of Stormwater Pollution Prevention Plan.

Mr. Ascenzo,
With regard to the items of observation from your visit:
Item 2 We have the meeting with leaders of Stormwater Pollution Prevention and have set time for
meeting with all employees on
June 13 and 14 to make sure everyone here is properly trained on procedures for responding
to spills. Attached is a copy of
Documentation of that meeting and the notation in our plan of no spills in the last 3 years.
Also attached is a copy of our list
list of responsible parties and contact numbers in the event of a large spill.
Item 3 Qualitative Monitoring was done on 6-6-12 and samples were picked up by Geo-Science and
Technology for testing. We
upon the next qualifying
will do our Qualitative Monitoring a second time this year as our permit requires, dependent
rainfall.
Item 4 We will conduct analytical Monitoring at the next qualifying rainfall and record results.
Item 5 Outfalls have been cleared and marked as noted in attached pictures
Item 7 Reviewing and updating of plan done on June 5, 2012 and will be done annually going forward.
Item 8 Soil in front of Maintenance shop was cleaned up by Craig and Company and receipt is attached.
