This Division of Water Quality received a Short C Form from you on July 5, 2001, for some reason we
are not sure of. We did not ask for this form insofar as we can determine. Your stormwater from the
facility would appear to be covered under Certificate of Coverage NCG2t0143 issued to Wood Waste
Energy Inc, - A Railworks Co. in 1999 (copy attached). This coverage is good until March 2003. If
your were attempting to change the name and/or ownership of the Certificate of Coverage you need to
complete and submit the attached form. We are not sure why you submitted the Short C Form. It is
being returned to you.
