Dear Mr. Warren:
On December 2, 2010 Myrl Nisely of the Raleigh Regional Office carried out a stormwater inspection at
Seaboard Lumber Mill. The help of Russell Schwartz was greatly appreciated. The mill was in
compliance. Please see the attached inspection checklist for details. Highlights were as follows:
The elements of the program as outlined in the permit were available either as computer files
handled by Mr. Schwartz or as hard copies in his files. We discussed the need for placing this
information into the Stormwater Pollution Prevention Plan manual left from when the mill was
owned by Intemational Paper. He expects to be able to update that manual by the end of
January, 2011.
Outfall 001 carries strong leachate from bark treatment windrows placed immediately upstream
of that location. There has been an understanding that the outfall would be monitored by
Caroline Bark, an impression created during a DWQ inspection in February 2004. Please
understand that West Fraser, Inc. is to conduct semi-annual monitoring of this release in keeping
with current permit requirements.
3. Bark is conveyed to the south side of the mill nearest Carolina Bark, where it is ground and
loaded onto trucks that carry it next door. At times a finely divided bark dust accumulates under
a tum in the conveyor system. This is moved to the grinding operation for incorporation by
Carolina Bark.
4. Outfalls 002 (Boiler blowdown) and 003 (Vehicle Maintenance area and Kiln condensate) each
travel under railroad tracks where theyjoin and eventually dissipate in a Swale along the tracks.
Outfall 002 has a metal screen that effectively holds back any stray wood wastes, resulting in a
clear flow as it starts under the railroad tracks.

5. Housekeeping�was-geherally good with the exception of piles of wood chips accumulated against
the chain.link.fence.beside.the chip truck loading station. It was agreed that these would be
removed to prevent the generation of a leachate from rainwater that could occur if the chips were
allowed to stay there and deteriorate.
6. All fuel storage and hydraulic oil operations are under structures, out of the rain.
This office expects to hold a meeting with representatives of DWQ, Carolina Bark and West
Fraser early in 2011 to review the industrial activities and water quality permits needed (both
stormwater and wastewater) for the combined bark handling operation. Contact will be made
through Mr. Schwartz to establish this meeting. He has offered the use of a mill conference
room.

Comment: A proactive program is in place, but its documentation needs to be
combined with information from the old IP Stormwater Pollution Prevention Plan and fully
updated with the current data.
Hydraulic oil spills were noted on 8/12/10 (3 gallons) and 9/16/10 (100 gallons). These
were properly cleaned up via the response plan.

In response to your renewal application for continued coverage under stormwater General Permit NCG210000 the Division of
Water Quality (DWO) is forwarding herewith the reissued stormwater General Permit. This permit has been reissued pursuant to
the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007.
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of stormwater General Stormwater Permit NCG210000
• A copy of the Technical Bulletin for the General Permit
• Five copies of the Discharge Monitoring Report (DMR) Form
• Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater
discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all
the changes in the reissued permit.
The more significant changes to the General Permit include the following:
• Permit cover page, second paragraph — The General Permit now may cover activities that DWO determines to be similar in
either the process, or in the exposed materials, to the Timber Products Industry.
• Part I Section A — A new provision that facilities draining to 303(d) listed waters, or in watersheds with an approved TMDL,
may not be eligible for continued coverage at the next renewal of the General Permit.
• Part I Section B second paragraph —A new clarification that the permit does not authorize discharges that DWO determines
are wastewaters.
• Part II Section A — The required contents of the Stormwater Pollution Prevention Plan have been expanded and clarified.
• Part II Section B Table 1 — A new requirement to sample stormwater discharges twice per year for those facilities with
exposed piles of certain materials remaining on site longer than seven days.
• Part II Section B Tables 2 and 3 and following — For permittees with exposed piles, there are new provisions requiring the
permittee to execute Tier One and Tier Two response actions, based on the first benchmark exceedence (Tier One) and the
second consecutive benchmark exceedence (Tier Two). Tier Two requires that the permittee institute monthly monitoring
instead of twice -per -year monitoring, until three consecutive monitoring events show no benchmark exceedences.
• Part II Section B — For permittees with exposed piles, a new provision that four exceedences of any particular benchmark
will trigger increased DWQ involvement in the permittee's stormwater management and control actions. DWQ may direct
the permittee to apply for an individual permit, or may direct the implementation or installation of specific stormwater control
measures.
• Part II Section C — Clarification that under the qualitative monitoring provisions of the permit, the permittee is obligated to
respond to repeated observations of stormwater pollution. DWO may impose additional stormwater management
requirements if the permittee is non -responsive, or if the responses are ineffective.
Your coverage under the General Permit is transferable only through the specific action of DWO.

Dear Mr. Buck:
On February 2, 2004 Mr. Myrl Nisely of the Raleigh Regional Office of the Division of Water Quality
made a compliance inspection at the Seaboard facility. A copy of the inspection checklist is attached.
The help provided by you, Eddie Wade and Tyrone Deloatch was greatly appreciated. During the
inspection, Eddie, Tyrone and Myrl also had a discussion with Mr. Robert Flintoff of Carolina Bark, as
discussed below. Observations were as follows:
This facility has a very well written Stormwater Pollution Prevention Plan (SP3) that contains all the
elements required by the general permit. The program is actively followed. Documentation of
stormwater activity is being done. The only change to improve the SP3 would be to place each
document into that binder as it is generated so that eventually five years of records are there, as a
"living document" readily available for inspection. At present all inspections, employee training
records, analytical data and so forth are stored in Mr. Deloatch's office as required by International
Paper (IP) protocols. Please add copies to the SP3.
2. There are seven points from which rainwater leaves the site, but only one, 003 for Vehicle
Maintenance, requires sampling under the newly issued permit (May 2003). The others receive
qualitative observations semi-annually. A tour of the site showed satisfactory housekeeping and
verified BMPs as described in the SP3.
3. This visit was carried out in part to understand stormwater interactions between this site and your
neighbor to the southwest, Carolina Bark. DWQ has been communicating with Carolina Bark that
runoff from their site is unacceptably high in BOD and nutrients. They have been requested to
evaluate engineering alternatives for changing to a non -discharge site, probably by collecting and
recycling leachate from their piles. From this visit it was learned that Carolina Bark leases the
southwest end of your site for bark processing, too. This processing includes the addition of fertilizer
and water sprays on the windrows to break down the bark, much like they do on their own site further
to the SW. Runoff from this activity is likely to pass through discharge point 001 on your site plan.

4. While IP is the owner of the leased area extending to 001, water quality exiting 001 has become the
responsibility of Carolina Bark because of their activity on that portion of the property. During the
inspection, Mr. Robert Flintoff was available, so Eddie, Tyrone and I discussed the fact that a
potentially complex arrangement existed at Point 001 between the two companies at this time. Dialog
between the two companies was encouraged. Since my visit, I can now offer further insight
concerning outfall 001. Once Carolina Bark obtains Division approval to convert their operation to a
non -discharge permit, the existence of outfall 001 will no longer be a concern to either company.
Water passing through that structure will enter their system. There will be no further need for anyone
to monitor it, except to see that solids do not become so excessive that water can no longer flow onto
the Carolina Bark property. Therefore, no action is needed at this time, in anticipation of the
installation of the Carolina Bark system within a few months.

Division within 30 days of receipt of this letter. DO NOT send the SPPP with the signed. form.
■ Five copies of Analytical Monitoring forms.
■ Five copies of Qualitative Monitoring forms.
■ A copy of a Technical Bulletin on the stormwater program which outlines program components
and addresses frequently asked questions.
Your certificate of coverage is not transferable except after notice to DWQ. The Division of Water

On February 2, 2004 Mr. Myrl Nisely of the Raleigh Regional Office of the Division of Water Quality
made a compliance inspection at the Seaboard facility. A copy of the inspection checklist is attached.
The help provided by you, Eddie Wade and Tyrone Deloatch was greatly appreciated. During the
inspection, Eddie, Tyrone and Myrt also had a discussion with Mr. Robert Flintoff of Carolina Bark, as
discussed below. Observations were as follows:
This facility has a very well written Stormwater Pollution Prevention Plan (SP3) that contains all the
elements required by the general permit. The program is actively f6ll6w6dc-'D66uinent5tio— Fof
stormwater activity is being done. The only change to improve the SP3 would be to place each
document into that binder as it is generated so that eventually five years of records are there, as a
"living document" readily available for inspection. At present all inspections, employee training
records, analytical data and so forth are stored in Mr. Deloatch's office as required by International
Paper (IP) protocols. Please add copies to the SP3.
There are seven points from which rainwater leaves the site, but only one, 003 for Vehicle
Maintenance, requires sampling under the newly issued permit (May 2003). The others receive
qualitative observations semi-annually. A tour of the site showed satisfactory housekeeping and
verified BMPs as described in the SP3.
3. This visit was carried out in part to understand stormwater interactions between this site and your
neighbor to the southwest, Carolina Bark. DWQ has been communicating with Carolina Bark that
runoff from their site is unacceptably high in BOD and nutrients. They have been requested to
evaluate engineering alternatives for changing to a non -discharge site, probably by collecting and
recycling leachate from their piles. From this visit it was learned that Carolina Bark leases the
southwest end of your site for bark processing, too. This processing includes the addition of fertilizer
and water sprays on the windrows to break down the bark, much like they do on their own site further
to the SW. Runoff from this activity is likely to pass through discharge point 001 on your site plan.

4. While IP is the owner of the leased area extending to 001, water quality exiting 001 has become the
responsibility of Carolina Bark because of their activity on that portion of the property. During the
r inspection, Mr. Robert Flintoff was available, so Eddie, Tyrone and I discussed the fact that a
potentially complex arrangement existed at Point 001 between the two companies at this time. Dialog
between the two companies was encouraged. Since my visit, I can now offer further insight
r concerning outfall001. Once Carolina Bark obtains Division approval to convert their operation to a
non -discharge permit, the existence of outfall 001 will no longer be a concern to either company.
Water passing through that structure will enter their system. There will be no further need for anyone
to monitor it, except to see that solids do not become so excessive that water can no longer flow onto
the Carolina Bark property. Therefore, no action is needed at this time, in anticipation of the
installation of the Carolina Bark system within a few months.

been received for the subject permit. This fee is required by Title 15 North Carolina Administrative Code 2H.0105, under
the authority of North Carolina General Statutes 143-215.3(a)(1), (1 a) and (1 b). Because this fee was not fully paid within
30 days after being billed, this letter initiates action to revoke the subject permit, pursuant to 15 ncac 2H.0105(b) (2) (k)

This letter is in response to your letter of May 19, 1998 to Bradley Bennett
regarding two types of discharges at the Subject Lumber Mill and the applicability of
permits required for these discharges. There are several air compressor discharges of
condensed water that do not discharge to the surface waters except in a storm event.
These discharges do not require further permitting; they are considered a part of the
discharge of stormwater from the site, which is discharge under the provisions of the
General Permit for stormwater, NCG210000. The other type of discharge is
regeneration water from boilers at the site. These discharges are covered under the
provisions of General Permit for cooling water, boiler blowdown, etc. in NCG500000,
rather than in NCG210000. We believe that your Certificate of Coverage, NCG500141,
covers these discharges.
In summary, the coverages under NCG210078 and NCG500141 are sufficient to
provide permit coverage for the discharges described in your May 19 letter. No other
permits should be needed for these discharges. If you have any questions please give

explained to him two circumstances that exist at Union Camp's Seaboard Lumber Mill
for which we would like to request coverage under General Permit No. NCG500000 if it
is required. He advised submitting the information to you. A detailed description of the
two water sources is attached.
Neither water source was specifically named in the general permit notice of intent.
Union Camp would like the DEHNR to give either approval to discharge or exemption
from coverage for each of the two sources. If you need further information, please feel

DESCRIPTION OF PROCESS WATER SOURCES
REQUESTING COVERAGE OR EXEMPTION
The Seaboard Lumber Mill operates several small air compressors as part of the
lumber making process. Three of the compressors have an automatic purge system
which eliminates condensed water from the compressor. Several other small
compressors are purged manually as needed.
The compressor water soaks into the ground and evaporates. The volume is too small
to create a discharge. During storm events, it is possible that the water could mix with
rainwater and be discharged to the storm ditch. The storm water is sampled for pH, oil
and grease, and total suspended solids as required by General Permit NCG210000.
Because there is no discharge except that associated with storm water, and the storm
water is covered under a general permit, Union Camp does not feel that coverage and
sampling is required under NCG500000. We would like your concurrence on this issue.
If coverage is required under NCG500000, we would like guidance on how and when to
sample since there is no discharge except during a storm event.
The Seaboard Lumber Mill operates two small industrial boilers that generate steam for
the lumber drying process. On approximately a monthly basis, each boiler goes
through a regeneration process of the water softening system. During that time,
approximately 650 to 700 gallons of salt (sodium chloride) water is purged from the
system. The water temperature is ambient with a pH of seven.
Union Camp believes that this water is covered by General Permit NCG210000 under
"similar wastewaters" because it is similar to boiler blowdown water. We would like
your concurrence on this issue.

idea of stormwater flow. I had to shrink it down to a smaller scale in order to email to you. I can mail you a larger scale
copy if needed. As I stated before our tenant Carolina Bark as well as West Fraser has water intermingling at Outfall 001
and we had a high COD issue earlier this year in which we feel was due to pine from the runoff of water on the bark piles.
We would like to look at and discuss our options with you, as well as plan to move outfall 001 if need be. We do semi
annual testing on this outfall. As I stated before we would like to have a meeting in January preferably the week of 1/14/13

Part II - General Facility Information
7.0 Drainage Basins & Outfalls
Refer to Figure 2 for details on the drainage ditches and Outfall locations. Also refer to Figure 3 for an
aerial photo of the facility in which the major features of the facility are visible. Potential pollutant
sources are included in the Basin and Outfall descriptions presented below. Note that the numbers
assigned to the basins and outfalls in this Plan are for internal purposes and do not imply that all outfalls
must be monitored or sampled under the stormwater permit. The facility monitors the discharge points
from the industrial activity areas of the site per Part 11, Paragraph B of General Permit NCG21000..
Appropriate monitoring points are selected where the industrial discharges exit the property. Some of the
monitoring points may include combined discharges of several of the basins as defined in this Plan. The
facility also samples the discharge from the area that contains the vehicle maintenance activities per the
requirements of Part II, Paragraph C of the General Permit.
Drainage Basin 1: Drainage Basin 1 is shared with the tenant bark composting operation. The
westernmost portion (the down -slope portion) is occupied by the tenant operation, which also utilizes
additional land not owned by International Paper beyond the small road that is the western boundary
of the International property. This tenant operation is not covered by the International Paper
stormwater permit, as this operation is not entirely within the International Paper Property and is not
under operational control of International Paper. The portion of the operation on the lease property is
used for composting piles and has little or no storage of petroleum products or other chemicals. This
operation does have on -site loaders to move the bark, and also stages trucks for loading of the
mulched bark for shipping as a finished product. The trucks are apparently fueled remotely and the
loaders are fueled on -site by a fuel delivery service.
The easternmost portion of Basin I includes the Saw Mill scale house and the bark bagging building
now used for storage (electric motors, spare parts, etc.). No petroleum or chemical storage or usage
occurs in the Saw Mill portion of Drainage Basin 1. Basin Idrainage is generally from east to west,
with most stormwater collecting in a shallow Swale through the center of the basin. However, there
are also minor amounts of sheet flow to the west and to the south that do not flow through the central
swale. Westward flowing water enters the ditch, which form the western boundary. The two halves
of this ditch flow southward and northward, respectively, to the approximate center of the western
boundary where they join at Outfall 001. This outfall includes a large sediment trap that intercepts
solids (primarily finely ground bark and sawdust) prior to discharge to a culvert beneath the road.
An additional ditch runs westward west along the southern boundary of Basin I and also combines
into Outfall 1. It receives small quantities of sheet flow from Basin 1. This same ditch also conveys
stormwater from Basin 2 (see below) and also from the railroad. It should be noted that a portion of
the Saw Mill runoff from Basin 1 leaves Saw Mill operations, enters the tenant operation where it co -
mingles with storm waters from the tenant operation and then re -combines at Outfall 001 with storm
waters from Basin 2, Basin 3 and from the railroad into Outfall 001.
Integrated Spill Response Plan 19 Seaboard Saw Mill
Part II - General Facility Information
Basin Size: Approximately 11.94 acres
Percent Impervious: Approximately 20% (paved or impervious building).
Potential pollutants =: TSS, primarily in the form of wood dust (bark and sawdust), possible soil -based
sediment, and minor oil & grease burdens from use and staging of trucks and industrial
vehicles (but none from vehicle maintenance or fueling).
Outfall: Outfal 1001
Drainage Basin 2: Basin 2 is located immediately east of Basin I, and includes the wood piles,
the sawdust shed, the boiler, the chip screen and a portion of the saw mill building. Basin 2
discharges via a culvert (Outfall 2) to an offsite ditch between the parallel tracks of the CSX railroad.
This railroad ditch flows westward approximately 450 feet to a culvert that conveys the combined
Basin 2 and railroad flow back under the northernmost track to the ditch mentioned above in the
Basin I description (see also Figure 2).
Basin Size: Approximately 7.67 acres
Percent Impervious: Approximately 75% (paved or impervious building).
Potential pollutants: TSS (bark dust, sawdust, boiler ash), oil & grease, pH (from boiler treatment
acids).
Outfall: Outfall 002 (which flows to Outfall 001)
Drainage Basin 3: Basin 3 is located immediately east of Basin 2, and includes a portion of the
east wood pile, the motor fuels slab, the oil slab, the wash pad, most of the maintenance shop
(including the apron in front of the high -bay doors to the maintenance shop), the kilns, the water tank
and its diesel fueled emergency pump and emergency lead -acid batteries, the Saw Mill office, outdoor

Part II - General Facility Information
Drainage Basin 4: Basin 4 is located immediately east of Basin 3, and includes the planer mill,
indoor lumber storage, a portion of the maintenance shop, and refuse dumpsters. Basin 4 discharges
via a culvert (Outfall 4) to an offsite ditch between the parallel tracks of the CSX railroad. This
railroad ditch flows eastward approximately its discharge off the eastern end of the International
Paper Property. The end of the culvert is considered as Outfall 004. Note that the flow from Outfall
004 co -mingles with the flow from Outfall 003 in the ditch, but can be observe and/or sampled
separately, as necessary.

Basins 5 & 6: Basins 5 and 6 are located east of Basin 4. These basins have no industrial activity,
and are mostly inactive land except for the communications tower within Basin 5. Basin 5 is drained
by outfall 005 and also by sheet flow that crosses the property boundary directly. Basin 6 is drained
by outfall 006 and also by sheet flow that crosses the property boundary directly. The exact boundary
between these two basins has not been specifically defined in Figure 2, as there is no petroleum
product stored by International Paper within these basins and there is no industrial activity regulated
under the stormwater permit within these basins.

Basin 7. Basin 7 is located along the northern margin of the Saw Mill and discharges to the north.
The majority of Basin 7 discharges as sheet flow to the ditch along the south site of Highway 186.
The paved employee parking lot flows to a catch basin at the north edge of the parking lot that flows
via a culvert to this same ditch. The culvert is Outfall 007. This basin has no industrial activity, and
consists of mostly landscaped grass except for the parking lot. There is no storage, transfer or use of
petroleum products, except for the fuels and crankcase oils used in the parked cars (a non -industrial
activity). There likewise is no storage, transfer, or use of non -petroleum chemicals.
Basin Size: Approximately 2.56 acres
Percent Impervious: Approximately 3 1 % (pavement)

PART H MONITORING, CONTROLS, AND LIMITATIONS FOR
PERMITTED DISCHARGES
SECTION A: STORMWATER POLLUTION PREVENTION PLAN
The Permittee shall develop a Stormwater Pollution Prevention Plan, herein after referred to as
the Plan. This Plan shall be considered public information in accordance with Part 111, Standard
Conditions, Section E, Paragraph 3 of this general permit. The Plan shall include, at a minimum,
the following items:
Site Plan. The site plan shall provide a description of the physical facility and the
potential pollutant sources which may be expected to contribute to contamination of
stormwater discharges. The site plan shall contain the following:
(a) A general location map (USGS quadrangle map or appropriately drafted
equivalent map), showing the facility's location in relation to transportation
routes and surface waters, the name of the receiving water(s) to which the
stormwater outfall(s) discharges, or if the discharge is to a municipal separate
storm sewer system, the name of the municipality and the ultimate receiving
waters; and accurate latitude and longitude of the point(s) of discharge.
(b) A narrative description of storage practices, loading and unloading activities,
outdoor process areas, dust or particulate generating or control processes, and
waste disposal practices.
(c) A site map drawn to scale with the distance legend indicating location of
industrial activities (including storage of materials, disposal areas, process areas
and loading and unloading areas), drainage structures, drainage areas for each
outfall and activities occurring in the drainage area, building locations, existing
BMPs and impervious surfaces, and the percentage of each drainage area that is
impervious. For each outfall, a narrative description of the potential pollutants
which could be expected to be present in the Stormwater discharge.
(d) A list of significant spills or leaks of pollutants that have occurred at the facility
during the 3 previous years and any corrective actions taken to mitigate spill
impacts.
(e) Certification that the stormwater outfalls have been evaluated for the presence of
non-stormwater discharges. The certification statement will be signed in
accordance with the requirements found in Part Ill, Standard Conditions, Section
B, Paragraph 5.
2. Stormwater Management Plan. The Stormwater management plan shall contain a
narrative description of the materials management practices employed which control or
minimize the exposure of significant materials to Stormwater, including structural and
nonstructural measures. The stormwater management plan, at a minimum, shall
incorporate the following:

(a) Feasibility Study. A review of the technical and economic feasibility of changing
the methods of operations and/or storage practices to eliminate or reduce exposure
of materials and processes to stormwater. Wherever practical, the permittee shall
prevent exposure of all storage areas, material handling operations, and
manufacturing or fueling operations. In areas where elimination of exposure is
not practical, the stormwater management plan shall document the feasibility of
diverting the stormwater runoff away from areas of potential contamination.
(b) Secondary Containment Schedule. A schedule to provide secondary containment
for bulk storage of liquid materials, storage of Section 313 of Title III of the
Superfund Amendments and Reauthorization Act (SARA) water priority
chemicals, or storage of hazardous substances to prevent leaks and spills from
contaminating stormwater runoff. if the secondary containment devices are
connected directly to stormwater conveyance systems, the connection shall be
controlled by manually activated valves or other similar devices [which shall be
secured with a locking mechanism] and any stormwater that accumulates in the
containment area shall be at a minimum visually observed for color, foam, outfall
staining, visible sheens and dry weather flow, prior to release of the accumulated
stormwater. Accumulated stormwater shall be released if found to be
uncontaminated by the material stored within the containment area. Records
documenting the individual making the observation, the description of the
accumulated stormwater and the date and time of the release shall be kept for a
period of five years.
(c) BMP Summary. A narrative description shall be provided of Best Management
Practices (BMPs) to be considered such as, but not limited to, oil and grease
separation, debris control, vegetative filter strips, infiltration and stormwater
detention or retention, where necessary. The need for structural BMPs shall be
based on the assessment of potential of sources to contribute significant quantities
of pollutants to stormwater discharges and data collected through monitoring of
stormwater discharges.
3. Spill Prevention and Response Plan. The Spill Prevention and Response Plan (SPRP)
shall incorporate an assessment of potential pollutant sources based on a materials
inventory of the facility. Facility personnel (or team) responsible for implementing the
SPRP shall be identified. A responsible person shall be on -site at all times during facility
operations that have the potential to contaminate stormwater runoff through spills or
exposure of materials associated with the facility operations. The SPRP must be site
stormwater specific. Therefore, a SPCC plan may be a component of the SPRP, but may
not be sufficient to completely address the stormwater aspects of the SPRP. The common
elements of the SPCC with the SPRP may be incorporated by reference into the SPRP.
4. Preventative Maintenance and Good Housekeeping Program. A preventative
maintenance program shall be developed. The program shall document schedules of
inspections and maintenance activities of stormwater control systems, plant equipment
and systems. Inspection of material handling areas and regular cleaning schedules of
these areas shall be incorporated into the program.

5. Employee Training. Training schedules shall be developed and training provided at a
minimum on an annual basis on proper spill response and cleanup procedures and
preventative maintenance activities for all personnel involved in any of the facility's
operations that have the potential to contaminate stormwater runoff. Facility personnel
(or team) responsible for implementing the training shall be identified.
6. Responsible Party, The Stormwater Pollution Prevention Plan shall identify a specific
position(s) responsible for the overall coordination, development, implementation, and
revision to the Plan. Responsibilities for all components of the Plan shall be documented
and position assignments provided.
Plan Amendment. The permittee shall amend the Plan whenever there is a change in
design, construction, operation, or maintenance which has a significant effect on the
potential for the discharge of pollutants to surface waters. The Stormwater Pollution
Prevention Plan shall be reviewed and updated on an annual basis.
The Director may notify the permittee when the Plan does not meet one or more of the
minimum requirements of the permit. Within 30 days of such notice, the permittee shall
submit a time schedule to the Director for modifying the Plan to meet minimum
requirements. The permittee shall provide certification in writing (in accordance with
Part III, Standard Conditions, Section B, Paragraph 5) to the Director that the changes
have been made.
8. Facility Inspection Program. Facilities are required to inspect all stormwater systems on
at least a semiannual schedule, once in the fall (September -November) and once during
the spring (April - June). The inspection and any subsequent maintenance activities
performed shall be documented, recording date and time of inspection, individual(s)
making the.inspection and a narrative description of the facility's stormwater control
systems, plant equipment and systems. Records of these inspections shall be
incorporated into the Stormwater Pollution Prevention Plan.
Stormwater discharge characteristic monitoring as required in Part 11 of this permit shall
be performed in addition to facility inspections.
9. Implementation. The permittee shall document all monitoring, measurements,
inspections and maintenance activities and training provided to employees, including the
log of the sampling data and of activities taken to implement BN1Ps associated with the
industrial activities, including vehicle maintenance activities. Such documentation shall
be kept on -site for a period of five years and made available to the Director or his
authorized representative immediately upon request.
SECTION B: QUALITATIVE MONITORING REQUIREMENTS
Qualitative monitoring requires a visual inspection of each stormwater outfall regardless of
representative outfall status and shall be performed as specified below in Table I. Qualitative
monitoring is for the purpose of evaluating the effectiveness of the Stormwater Pollution
Prevention Plan (SPPP) and assessing new sources of stormwater pollution. No analytical tests
are required. Qualitative monitoring of stormwater outfalls does not need to be performed during
a representative storm event.

Monitoring Location: Qualitative monitoring shall be performed at each
stormwater discharge outfall (SDO) regardless of representative outfall status.
SECTION C: ON -SITE VEHICLE MAINTENANCE MONITORING REQUIREMENTS
Facilities which have any vehicle maintenance activity occurring on -site which uses more than
55 gallons of new motor oil per month when averaged over the calendar year shall perform
analytical monitoring as specified in Table 2. This monitoring shall be performed at all outfalls
which discharge stormwater runoff from the vehicle maintenance areas.
For each parameter, the arithmetic mean of all analytical sampling results collected during the
coverage of the permit shall be calculated for each individual outfall. The computed arithmetic
mean is then compared to the cut-off concentrations listed in Table 3. If the arithmetic mean is
less than the specified cut-off concentration for a parameter other than pH or within the specified
range for pH only, then the facility is not required to continue annual analytical monitoring for
that parameter (at that outfall) during the remaining coverage of the permit unless a significant
change in facility operations or configuration occurs. If a cut-off concentration results in
discontinued analytical monitoring at an individual discharge outfall, the permittee is required to
maintain facility operations that ensure the continuation of stormwater runoff quality.
The Dermittee must perform analvtical sampling durine the first vear that the facility becomes
subiect to this Section C and last vear of hermit coveraee reeardless of cut-off concentration
conditions. Analytical results from sampling during the final year of permit coverage must be
submitted with the permit renewal application. All analytical monitoring shall be performed
during a representative storm event.
