Enclosed please find the Semi-annual Stormwater Discharge Monitoring Report (DMR) for
North Carolina DEMLR General Permit No. NCG200458. The facility had no representative discharge
of stormwater in 2015 during business hours. The facility inspects the site during rain events to verify
there is no discharge. Stormwater is directed to a series of pools on the property where it infiltrates and

Management Commission, and the Federal Water Pollution Control Act, as amended,

to receiving waters designated as Little Sugar Creek, a class C water in the Catawba River Basin,
in accordance with the effluent limitations, monitoring requirements, and other conditions set
forth in Parts I, II, III, and IV of General Permit No. NCG200000 as attached.
This certificate of coverage shall become effective April 11, 2014.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
