On September 15, 2011, Dave Parnell of the Raleigh Regional Office (RRO), Surface Water Protection
Section, and Jennifer Jones of the Stormwater Permitting Unit (SPU) conducted a site visit in order to
address the possibility of this facility (NCG200354) being granted Representative Outfall Status (ROS).
At that time, new stormwater discharge outfalls (SDO) were designated. We agreed at that time, that
sampling during the next two sample periods would be conducted at all of the newly designated outfalls,
in order to determine the makeup of cacti sample. After the two sample periods, a determination would
be made by the RRO, of whether your facility could be granted ROS.
There are currently six stormwater discharge outfalls (SDO) being sampled. Five are
discharging to the stormwater system, which flows to the drainage ditch which passes under
much of your facility. The sixth SDO is the outfall that has been sampled historically and is the
referenced drainage ditch that flows into and out of your property.
During the site visit, it was noted that you have a clear understanding of the requirements of the
NCG200000 permit and are currently monitoring all of the outfalls as required. An evaluation of each
SDO was conducted and sampling techniques at each SDO were agreed upon. After reviewing sample
results and outfall locations, it is understandable that the similarities among the outfalls would allow for
ROS designation.
Your most recent sample results from May 9, 2012, were received by this office on .June 4, 2012. SDO
#I, 92, #3 and #4 were found to be within the required parameters set forth by the permit. SDO 95
continues to have parameters above the acceptable range for the parameters of Copper and Total
Suspended Solids. These elevated sample results are similar to the results that you have been reporting to
its in past years - that resulted in your facility being placed on Tier 2 found in Section 2, Part 13 of your
stormwater permit.

Therefore; it is determined that the following outfall can be representative of other outfalls at this time:
The drainage areas of Outfall # 1, 2, 3 and 4 are contiguous. All of the ou[f1tllS have similar activities and
contain similar materials in their drainage areas. SDO # I is granted ROS for SDO # 1, 2, 3 and 4.
Outfall 45 (the former SDO) is influenced by outside sources of contaminants and will no longer be
required to be sampled.
Analytical sampling is required at the frequency and for the parameters set forth in the pennit for SDO #5
— formerly the only SDO designated.
In the event that topography and activities change at the facility, the above approval of the representative
outfall may be revisited/re-opened by the Division of Water Quality. Sampling is required if new outfalls
are added or if outfall locations are changed. In addition, after analytical data is obtained from these
locations, DWQ may revisit the request for representative outfall status.
Please note that the representative outfall designation is only for analytical monitoring requirements of
the General Stonmwater Permit. The qualitative monitoring requirements are still effective for all
stormwater outfalls.
In addition to this ROS certification, it is permitted that Rocky Mount Recyclers be taken off of Tier 2
Response Actions, of Section 2, Part I3 of your permit. You may begin sampling as required by the
permit when there is no Tiered Response Actions required.

The Mooresville Regional Office staff have reviewed your request dated February 22, 2012 for a
determination that stormwater discharge outfall (SDO) 001 be granted representative outfall status for
stormwater outfalls 005, 006, 003, 009, 008, 001, and 007. Based on site visit conducted, we are
approving this request. In accordance with 40 CFR §122.21(g)(7), you are authorized to sample outfall
number 001 as a representative outfall. This approval is effective with the next sampling event. We also
want to remind you that the permit still requires Qualitative Monitoring be performed at all SDOs,
regardless of representative status.
Please remember that any actions you initiate in response to benchmark exceedences as directed in the
tiered response provisions of your permit must address all drainage areas represented by SDO 001,
where appropriate.
Please append this letter to your Stormwater Pollution Prevention Plan (SPPP) or permit to document
that representative outfall status has been approved. If changes in drainage areas, structures,
processes, storage practices, or other activities occur that significantly alter the basis of this approval,
representative ouffall status may no longer be valid. You should either resume sampling at all SDOs, or
reapply to this office for representative outfall status based on updated information. If you have any

On October 14, 2011, the Division of Water Quality (DWQ), Raleigh Regional Office (RRO), received your
documented laboratory results from analysis at specified sample sites taken on October 6, 2011. The
accompanying map had the sample sites noted and labeled as Jennifer Jones and I had requested during our
technical support visit on September 15, 2011. During the visit, we had agreed that you should sample at five
points within your facility, in addition to your current Stormwater Discharge Outfall (SDO). All of the sample
sites are now considered to be SDO and must be sampled as directed by your permit.
After reviewing your sample results and comparing them with the baseline upstream and downstream values
you had provided from prior sampling, it is apparent that the high copper values you have been receiving, are
coming upstream from your facility. All five of the sample sites within your facility had values below the
copper benchmark and the SDO continued to exceed the benchmark.
Therefore, it is permissible for you to resume sampling as required by your permit, prior to placing yourself into
the tier system. As stated above, there will now be six SDO. The site map of your facility should reflect the
SDO, numbered 1-6. Please sample all SDO as required by your permit. After receiving another set of sample
results, and if they are consistent with the October 6, 2011 results, you may apply for representative outfall
status (ROS), which would allow you to sample at one site which would represent the production area of your
facility.

In response to your renewal application for continued coverage under general permit NCG200000 the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwaler general permit. Due to resource constraints,
the Division has been unable to adequately consider appropriate modifications to the permit. Therefore, the permit
has been reissued without changes from the previous permit, so all conditions remain the same. The permit term for
the reissued permit is two years, expiring on September 30, 2008, The general permit may be modified and reissued
prior to its expiration. The permit is reissued pursuant to the requirements of North Carolina General Statute 143-
215.1 and the Memorandum of Agreement between the state of North Carolina and the U.S. Environmental
Protection Agency, dated December 6, 1983.

Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal

compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.

Your facility is currently covered for stormwater discharge under General Permit NCG200000. This permit expires on
October 31, 2005. The Division staff is currently in the process of renewing this permit. When a draft version is
available for notice and public comment, it will be posted on our website at http://h2o.enr.state.nc.us/su/. Once
the permit is reissued, your facility would be eligible for continued coverage under the reissued permit.
In order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, enclosed you will find a
Permit Coverage Renewal Application Form. The application must be completed and returned by August 19, 2005 in
order to assure continued coverage under the general permit. Letters confirming our receipt of the completed
application will not be sent.
Failure to request renewal within the time period specified may result in a civil assessment of at least $250.00.
Larger penalties may be assessed depending on the delinquency of the request. Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and
could result in assessments of civil penalties of up to $10,000 per day.
Please note that "no exposure exclusion" is now available to all operators of industrial facilities in any of the
categories of "storm water discharges associated with industrial activity," (except construction activities). If you feel
your facility can certify a condition of "no exposure", i.e. the facility industrial materials and operations are not
exposed to stormwater, you can apply for the no exposure exclusion. For additional information contact the Central
Office Stormwater Staff member listed below or check the Stormwater Permitting Unit Web Site at
http://h2o.enr.state.nc.us/su/.
If the subject stormwater discharge to waters of the state has been terminated, please complete the enclosed
Rescission Request Form. Mailing instructions can be found on the bottom of the form. You will be notified when
the rescission process has been completed.

If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you,
you have the right to request an individual permit by submitting an individual permit application. Unless such
demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division of Water
Quality. The Division of Water Quality may require modification or revocation and reissuance of the certificate of
coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the Division of
Water Quality or permits required by the Division of Land Resources, Coastal Area Management Act or any other
Federal or Local governmental permit that may be required.
Enclosed is a permit package which contains the following information
* A stormwater pollution prevention plan (SPPP) implementation certification form
* A copy of general stormwater permit NCG200000
* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin for the general permit which outlines key requirements and addresses
frequently asked questions
* A Certificate of Coverage

This certificate of coverage shall become effective January 19, 2001.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

The general stormwater permit for scrap metal recycling industries, NCG200000, was issued on
November 1, 2000. Since there has been a considerable time lag since your permit application was received on
July 28, 1995 this letter is being sent to ensure that the contact information that we have is correct.
Enclosed with this letter you should find a contact information update form with the information that the
Division has on your facility. Please check to see whether or not this information is correct. If any of the
information has changed from what our records show, please make the appropriate corrections in the empty
blanks to the right of the information and mail the corrected form to us by December 8, 2000. Sign and mail the
information form regardless of whether or not any changes need to be made.
Upon receipt of your information form you will be mailed a Certificate of Coverage with a new number of the
format NCG200***. If you were one of the few facilities that received coverage under the NCG030000 general
permit prior to the issuance of this permit your existing NCG030000 permit coverage will be rescinded
concurrent with the issuance of your new Certificate of Coverage.
