Acid is leaking onto a 55 gallon drum of an unknown substance. Does not need any assistance at this time, but
might request RRT 6 once he gets on scene and evaluates.
1537- LEMC requested Technical Advise RRT 6
1643 - Eric Wiseman request a state mission from RRT 6 due to substance leaking and it is out of capabilities of
locals.
1720 - Mission cancelled per DDO Taylor after phone conversation with J. Cardwell. Locals no longer require
state assistance.

1538- LEMC patched to RRT 6 advised 1 gallon glass container of Perchloric acid leaking onto a 55 gallon metal
drum poss containing a hydo carbon product labeled "Ashefill glaze". RP has been contacted and waiting on
chemist to return call. RRT 6 advised would contact LEMC back with further information shortly.
1558 - T. Miller sent page that J. Cardwell enroute to scene.
1643 - E. Wiseman called and requested a state response from RRT 6.
1644 - J. Cardwell paged.
1647 - J. Cardwell briefed of request. Stated that he is on scene with LEMC an it is a good mission. Not sure of
tier response level required.
1650 - Called to brief RRT of request. Will have District Chief call.
1659 - DDO Taylor paged.
1700 - SEC Rudisill briefed and approved mission.
1704 - Notified LEMC of mission approval. Stated that mission had been put on hold because responsible party
might be on scene before RRT 6 can be. This decision made approx 4 minutes ago between LEMC and District
Chief.
1707 - J. Cardwell paged.
1717- NCEOC made contact LEMC Cell phone to have AC call NCEOC, Placed AC on phone.
1718 - J. Cardwell and DDO Taylor phone conversation.

Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the slate of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
* A copy of general stormwater permit NCG180000
* A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
must be completed and returned to the Division within 30 days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin on the Stormwater program which outlines program components and
addresses frequently asked questions
* A corrected Certificate of Coverage if you indicated a name change on the Renewal Application
Your coverage under this general permit is not transfefable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law rule, standard, ordinance, order, judgment, or
decree.

Your facility is currently covered for stormwater discharge under General Permit NCG180000. This permit
expires on August 31, 1999. We are currently in the process of rewriting this permit and are scheduled to have the
permit reissued by the summer of 1999. Once the permit is reissued your facility would be eligible for continued
coverage under the reissued permit.
Irm order to assure your continued coverage under the general permit, you must apply to the Division of Water
Quality (DWQ) for renewal of your permit coverage. To make this renewal process easier, we are informing you
in advance that your permit will be expiring. Enclosed you will find a general permit renewal application form.
This form serves two purposes. First, it is an application for renewal of your permit. Second, it serves as an
invoice for the $80.00 permit fee. The application must be completed and returned with the required fee by March
1, 1999 in order to assure continued coverage under the general permit.
Recent legislation modified the fee structure for DWQ permits. Your general permit fee bas been changed to an
annual fee of $80-00. A copy of the new fee schedule is enclosed in this package.
Failure to request renewal within this time period may result in a civil assessment of at least $250.00. Larger
penalties may be assessed depending on the delinquency of the request Discharge of stormwater from your
facility without coverage under a valid stormwater NPDES permit would constitute a violation ofNCGS
143-215.1 and could result in assessments of civil penalties of up to $10,000 per day.
