of Coverage for your facility. These permits have specific conditions that must be met in order for you
to be in compliance with your permit. It is your responsibility, as the permit holder, to read and
comply with the conditions contained in the permit.
It is our responsibility, as the issuing authority, to make sure that the operation and
maintenance of your facility complies with the conditions contained in your permit. To assist you in
complying with these conditions, we are attaching a Technical Bulletin specific to your permit
requirements. We are currently in the process of developing our inspection schedule. Therefore, you

In response to your renewal application for continued coverage under general
NCG 180000, on May 10, 1999 the Division of Water Quality (,DWQ) is forwarding herewith the
reissued stormwater general permit. This permit is reissued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state
of North Carolina and the U.S. Environmental Protection Agency, dated December6, 1983.
This permit renewal should have been sent soon after your request to renew was received,
and this oversight was not discovered until last week when the permit was brought to the
attention of Ms. Aisha Lau due to a notice of violation (NOV) for the nonpayment of annual fees.
The NOV has now been taken off of the system, and enclosed you will find a new invoice that
Will need to be paid 30 days from the receipt of this letter.
The following information is also included with this permit package:
* A copy of general stormwater permit NCG 180000
* A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this
form is required to certify that you have developed and implemented the SPPP. as per the
conditions of the permit. This form must be completed and returned to the Division within 30
days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and five copies of the Qualitative
Monitoring form
A copy of a Technical Bulletin on the Stormwater program which outlinessprogram
components and addresses frequently asked questions
Your coverage under this general permit is not transferable except after notice to DWQ.
The Division may require modification or revocation and reissuance of the Certificate of
Covera.ge. This permit does not affect the legal requirements to obtain other permits which may

be required by DGNR or relieve the permittee from responsibility for compliance with any other
applicable federal, state, or local law rule, standard, ordinance, order, judgment, or decree.

Your facility is currently covered for Stormwater discharge under General Permit NCG180000. This permit will
expire on August 31, 1999. North Carolina General Statute 143.215.l(c) requires that an application for permit
coverage renewal be filed in a timely manner. A prior notice of renewal was sent for this site on February 3, 1999.
To date, no response has been received by the Division of Water Quality (DWQ). In order to assure your
continued coverage under the general permit, you must apply to DWQ for renewal of your permit coverage.
Enclosed you will find a general permit renewal application form. This form serves two purposes. First, it is an
application for renewal of your permit coverage. Second, it serves as an invoice for the $80.00 permit fee. The
application must be completed and returned with the required fee by May 3, 1999 in order to assure continued
coverage under the general permit.
Recent legislation modified the fee structure for DWQ permits. Your general permit fee has been changed to an
annual fee of $80.00. A copy of the new fee schedule is enclosed in this package.
Failure to request renewal by May 3, 1999 will result in a civil assessment of at least $250.00. Larger penalties
may be assessed depending on the delinquency of the request. Discharge of Stormwater from your facility without
coverage under a valid Stormwater NPDES permit would constitute a violation of NCGS 143-215.1 and could
result in assessments of civil penalties of up to $10,000 per day.
