permit coverage when, in fact, continuing permit coverage was necessary. If, in retrospect, you feel the
site still requires permit coverage, you should notify this office immediately. Furthermore, if in the future
you wish to again discharge to the State's surface waters, you must first apply for and receive new
coverage under an NPDES permit.

This annual fee is required by the North Carolina Administrative Code. It covers the administrative costs associated with
your permit. It is required of any person holding a permit for any time during the annual fee period, regardless of the facility's
operating status. Failure to pay the fee by the due date will subject the permit to revocation. Operating without a valid
permit is a violation and is subject to a $10,000 per day fine. If the permit is revoked and you later decide a permit is
needed, you must reapply, with the understanding the permit request may be denied due to changes in environmental,
regulatory, or modeling conditions.

Inspection Summary:
Site inspection due to rescission request.. At this time the rescission request cannot be approved due to the following
reasons:
1. There are three, single -walled laquer thinner tanks which have not been professionally cleaned out. The walls of the
tanks have several areas of leakage and a strong odor of thinner.
2. A single -walled diesel fuel tank without any secondary containment was located in an area exposed to storwater.
3. There remains an active forklift on the site for maintenance needs, stored in a location which is exposed to stormwater.
4. There is an old backhoe with a. blown motor on the site, stored in a location which is exposed to stormwater.. There
were no active leaks noted at the time of this inspection.
Until these items are addressed, the rescission request cannot be approved.

Zahid,
I was able to do a site visit for NCG180118 Southern Furniture Plant 2 on 9/11/14. I would recommend NOT approving
their request for permit rescission at this time. During the inspection it was noted there were three, single -walled Laquer
Thinner tanks that had significant rust,.evidence of leaking and had not been professionally cleaned out (still contained
the Thinner). There was also a single -walled diesel fuel tank with no secondary containment on site which is still used to
fill up their fork lift. There was an old fork lift they still use as well as a back hoe with a blown motor still on the
site. Until these items are addressed, the facility needs to continue to follow all steps within their current permit.

In response to your renewal application for continued coverage under general permit NCG180000, the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
* A copy of general stormwater permit NCGI80000
* A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
must be completed and returned to the Division within 30 days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin on the stormwater program which outlines program components and
addresses frequently asked questions
* A corrected Certificate of Coverage if you indicated a name change on the Renewal Application
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law rule, standard, ordinance, order, judgment, or
decree.
