Thank you for sending the correspondence on Friday regarding the air quality permit but since the rescission was for an air quality
permit and not your stormwater permit — I'll need for you to submit a rescission request for stormwater by filling out the attached
form.
For plant k61, your stormwater permit is p NCG170421. I checked with the region and they will contact you before sending an
engineer out to inspect the facility. The form has our central office address so mail in to my attention and once we receive the
signed form, the region will contact you.

Thank you for the follow up. The requested rescission documentation is attached. I cannot locate my signed copy of the
request letter, but a signed version was submitted to MRO back in February.
To explain a bit - this particular building is home to three different operations. One is a raw materials and finished goods
warehouse known as Fiber Distribution Center (FDC), which is the largest of the three - John Reynolds is the FDC
Manager. The second is a textiles quality laboratory known as Fiber Research Center (FRC). The third is a small
production operation that was previously Plant 61, but now houses a different operation which belongs to the US Cotton
side of our business (Parkdale and US Cotton are separate companies with shared ownership).
So, the facility is still in operation, but the small part of it that was permitted as Plant 61 is operating under a different
permitting status (exempt, I understand, with respect to Air Quality).

In accordance with your application for a discharge permit received on May 27, 2015, we are forwarding
herewith the subject certificate of coverage to discharge under the subject state - NPDES general permit.
This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection Agency dated
October 15, 2007 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to the Division of
Energy, Mineral, and Land Resources. The Division may require modification or revocation and
reissuance of the certificate of coverage.
Per the requirements of the Catawba Riparian Buffer Rule, all stormwater drainage to stream buffers,
from portions of this site that have been constructed after June 30, 2001, must be discharged through a
correctly designed level spreader or another device that meets diffuse flow requirements per 15A NCAC
2B .0243. Diffuse flow requirements are described in Chapter 8 of the North Carolina Stormwater BMP
Manual, available at: http://portal.ncdenr.orglweb/Ir/bmp-manual.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Energy, Mining, and Land Resources, or permits required by the Division of Water
Resources, Coastal Area Management Act, or any other federal or local governmental permit that may be
required.

This certificate of coverage shall become effective July 7, 2015.

Concerning the issuance of NCG170421 for the Parkdale Mills facility in Gaston County -

Based on yesterday's site visit, the facility is very close to no exposure and may prefer to try and achieve that status. At this time one
of the site managers, Mr. John Reynolds, was on vacation and is needed to make the final determination. Because all interested
parties need to meet and review the information to determine the best course for the facility, it is recommended that the permit not
be issued until Parkdale makes a decision.
Lisa and John --Attached please find the No -Exposure information (NPDES-NCGNE file). I have also attached the permit that will be
issued in case Parkdale decides to not to apply for No Exposure. If you have any questions, please do not hesitate to contact me -I
will be back in the office next Thursday
Ken and Bradley —This was forwarded to our office through Mr. Rick Reynolds. When I tried to copy him on this email I received a
,no longer valid email' notice. I am not sure who is tracking it in his stead, but I though both of you would be adequate for Central
Office notification. Parkdale will make a decision within approximately 2 weeks, depending on vacation times.
