Management Commission, and the Federal Water Pollution Control Act, as amended,
South Fork Industries, Inc.
is hereby authorized to discharge stormwater from a facility located at:

Catawba County
to receiving waters designated as Shady Branch, a class C water, in the Catawba River Basin; in
accordance with the effluent limitations, monitoring requirements, and other conditions set forth
in Parts 1, IL 111, and IV of General Permit No. NCG170000 as attached.
This certificate of coverage shall become effective November 30, 2016.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

assigns, and shall include singular, plural, masculine, feminine or neuter as required by context.
WITNESSETH, that the Grantor, for a valuable consideration paid by the Grantee, the receipt of which is
hereby acknowledged, has and by these presents does grant, bargain, sell and convey unto the Grantee in fee simple,
all that certain lot or parcel of land situated in Catawba County, North Carolina and more particularly described as
follows (the "Property"):

TO HAVE AND TO HOLD the aforesaid Property and all privileges and appurtenances thereto belonging
to the Grantee in fee simple.
And the Grantor covenants with the Grantee, that Grantor has done nothing to impair such title as Grantor
received, and Grantor will warrant and defend the title to the Property against the lawful claims of all persons
claiming by, under or through Grantor only, except for the exceptions set forth on EXHIBIT'B", attached hereto
and made a part hereof. The Property being conveyed hereby is not the primary residence of Grantor.

In accordance with your application for a discharge permit received on April 22, 2009 we
are forwarding herewith the subject certificate of coverage (COC) to discharge under the subject
state — NPDES general permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina
and the US Environmental Protection Agency dated October 15, 2007 (or as subsequently
amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, Coastal Area Management Act or any other
federal or local governmental permit that may be required.
Please note that the attached permit expires on July 31, 2009. Your COC will be
automatically renewed to cover the full 5 year term of the soon to be reissued permit. In the
interim you must abide by the terms of the expiring permit. You can review the proposed
general permit changes once they are posted at(www.newaterquality.org/su/pLiblicnotice.htm).
Per the requirements of the Catawba Riparian Buffer Rule, all stormwater drainage from
portions of this site that have been constructed after June 30, 2001 must be discharged through a
correctly designed level spreader or another device that meets diffuse flow requirements per 15A
NCAC 2B .0243. Diffuse flow requirements are described in Chapter 8 of the North Carolina
Stormwater BMP Manual, available at: http://112o.enr.state.nc.us/su/bmp forms htn3

Please see NOI attached.
We have received an application for coverage of an existing facility under NCG 17 from Delta Apparel, Inc. The facility
drains to Shady Branch (class C) in Maiden (Catawba County)..No vehicle maintenance or haz waste. They currently also
have the following permits: NC0006190 and WQ0006984.. Does the MRO have any concerns about issuing this facility a
COC for this general permit?
