On March 5, 2008, Autumn Hoban, of the Division of Water Quality (DWQ), Raleigh Regional Office, performed
a stormwater compliance evaluation inspection of the plant. Mr. Bob Messer assisted me with the inspection and
his cooperation was helpful and appreciated. Inspection findings are listed below.

Guilford Mills Inc. prior to 2005 manufactured yarn. In 2005, it was reported that the plant shut down the
manufacturing process. The stormwater runoff and process wastewater generated at such facilities is regulated
by General Permit NCG170000. DWQ has therefore issued Guilford Mills certificate of coverage, NCG170140,
which became effective on August 1, 2004 and expires on July 31, 2009.
The facility has nine stormwater outfalls and a pond on site. Process water is discharged through the sanitary
sewer system to the Fuquay Varina WWTP. The facility has two tanks on -site that hold 3,000 gallons of finish
oil that have secondary containment and lock out as required. Other bulk oil storage was reported to have been
removed last year, tanks emptied, and locked out upon the shut down of the manufacturing process. Waste oil
and 55 gallon drums are stored under cover, with containment, and are locked in fenced area. The waste oil is
removed from the site by Noble Oil,
The facility was in compliance with permit requirements with one exception. It is required that the Stormwater
Pollution Prevention Plan be updated annually and the plant should provide an updated plan that reflects the
current operations of the plant upon future inspections.
The pond on site overflows to an unnamed tributary of Basal Creek. The pond overflow was sampled during the
inspection and the results are attached. pH, Conductivity and Dissolved Oxygen were in normal range.

Comment: The Plan has not been updated since 2000. In 2005, manufacturing ended
at the plant, changing operations significantly. The plan is required to be updated
annually and be reflective of the current operations.
There are two tanks on -site that hold 3,000 gallons of finish oil. These tanks contain
secondary containment as required. Other tanks on the site were reported cleaned out
and are no longer in use.
Waste oil and 55 gallon drums are stored with secondary containment, under cover, and
in a locked, fenced area at the facility and are picked up by Noble Oil.

RAINFALL EVENT.THE ASSOCIATE PERFORMING THESE INSPECTIONS WILL BE TRAINED BY
THE CORPORATE ENVIRONMENTAL MANAGER TO OBSERVE THE OUTFALLS FOR COLOR,ODOR,
SOLIDS,FOAM,OUTFALL STAINING AND VISIBLE SHEENS.THE ASSOCIATE WILL ALSO INSPECT
ALL STORMWATER STRUCTURES AND CONVEYANCES TO MAKE SURE THEY ARE IN WORKING ORDER
A RECORD OF THIS INSPECTION AND ANY SUBSEQUENT MAINTENANCE ACTIVITIES ,
PERFORMED WILL BE MAINTAINED WITH THE STORMWATER POLLUTION PREVENTION PLAN
(SPPP) LOCATED IN THE PLANT MANAGERS OFFICE. THESE RECORDS TO BE MAINTAINED FOR
5 YEARS. A FORM FOR RECORDING INFORMATION FROM THE INSPECTION IS AVAILABLE IN
THE SPPP.DOCUMENT THE DATE AND TIME OF INSPECTION AND A NARRATIVE DESCRIPTION
OF THE PLANT'S STORMWATER SYSTEM, DOCUMENTATION OF THIS VISUAL MONITORING

Dear Permittee:
Payment of the required annual administering and compliance monitoring fee of $80.00 for this year has not
been received for the subject permit. This fee is required by Title 15 North Carolina Administrative Code 2H.0105, under
the authority of North Carolina General Statutes 143-215.3(a)(1), (1a) and (1b). Because this fee was not fully paid within
30 days after being billed, this letter initiates action to revoke the subject permit, pursuant to 15 ncac 2H.0105(b) (2) (k)
(4), and G.S. 143-215.1 (b) (3).
Effective 60 days from receipt of this notice, subject permit is hereby revoked unless the required Annual
Administering and Compliance Monitoring Fee is received within that time. Discharges without a permit are subject to the
enforcement authority of the Division of Water Quality. Your payment should be sent to:

If you are dissatisfied with this decision, you have the right to request an administrative hearing within Thirty (30)

Hearings, Post Office Drawer 27447, Raleigh, North Carolina, 27611-7447. Unless such request for hearing is made or

In response to your renewal application for continued coverage under general permit NCG 170000. the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6. 1983.
The following information is included with your permit package:
* A copy of general Stormwater permit NCG170000
* A Stormwater Pollution Prevention Plan (SPPP) Certification Form. Completion of this form is required to
certify that you have developed and implemented the SPPP as per the conditions of the permit. This form
must be completed and returned to the Division within 30 days of receipt of this letter.
DO NOT SEND the SPPP with the signed form.
* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin for the general permit which outlines changes in the permit, key requirements.

Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order, judgment, or
decree.

herewith the subject certificate of coverage to discharge under the subject state - NPDES general permit. This
permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Environmental Management. The Division of Environmental Management may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal
