Dear Ms. Kay:
On behalf of Boggs Materials, Inc. (Boggs), please find attached results of stormwater outfall
sampling that was conducted on October 11, 2018 for the Stafford Street Asphalt Plant facility in
Monroe, Union County, North Carolina. Water samples were collected from the outfall SDO-3 by
Boggs personnel on October 11, 2018 in accordance with the SWPPP and sampling requirements of
the permit. Please note that per Boggs personnel, there was no discharge from outfalls SDO-1,
SDO-2 and SDO-4 through SDO-8. Therefore, no samples could be collected from these outfalls.
Water samples were analyzed for total suspended solids. The October 2018 sampling results
indicated that total suspended solids marginally exceeded the permitted levels in the sample
collected from SDO-3. The Stormwater Discharge Outfall Monitoring Report is included in
Appendix A, the Stormwater Discharge Outfall Qualitative Monitoring Report is included in
Appendix B, and a complete laboratory analytical report is included as Appendix C.

subject state -. NPDES general permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
Please take notice that this certificate of coverage is not transferable except upon
approval of the Division of Energy, Mineral, and Land Resources. The Division of Energy,
Mineral, and Land Resources may require modification or revocation and reissuance of the
certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Energy, Mineral, and Land Resources, the Coastal Area
Management Act, or any other federal or local government permit that may be required.

to receiving waters designated as unnamed tributary to Stewarts Creek, a class WS-1II water in
the Yadkin River Basin, in accordance with the effluent limitations, monitoring requirements,
and other conditions set forth in Parts I, II, III, IV, V, and VI of General Permit No. NCG160000
as attached.
This certificate of coverage shall become effective May 12, 2014.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
