Dear Mr. Parker:
In accordance with your application for a discharge permit received on November 20, 2008, we
are forwarding herewith the subject certificate of coverage to discharge under the subject slate — NPDES
general permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-
215.1 and the Memorandum of Agreement between North Carolina and the US Environmental Protection
Agency dated October 15, 2007 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to the
Division of Water Quality. The Division of Water Quality may require modification or revocation and
reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required
by the Division of Water Quality or permits required by the Division of Land Resources, Coastal Area
Management Act or any other federal or local governmental permit that may be required.

STORMWATER DISCHARGES
NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM
In compliance with the provision of North Carolina General Statute 143-215.1, other lawful
standards and regulations promulgated and adopted by the North Carolina Environmental
Management Commission, and the Federal Water Pollution Control Act, as amended,

is hereby authorized to discharge stonnwater from a facility located at

170 Knobbs Creek Road
Elizabeth City, NC
Pasquotank County
to receiving waters designated as Pasquotank River, a class SC water in the Pasquotank River
Basin, in accordance with the effluent limitations, monitoring requirements, and other conditions
set forth in Parts I,.II, III, IV, V, and VI of General Pen -nit No. NCG 160000 as attached.
This certificate of coverage shall become effective January 30, 2009.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

Hello,
I copied you on my reply to Jason Mizelle, Project Manager of Albemarle & Associates,
Ltd., for your information. Mr. Al said for me not to hold up the issuance of the
COC, at this point, for their SPPP plan. The permit text states that the plan must be
utilized prior to discharge, so I will be visiting the site shortly after plant
opening and ringing any bells necessary. Therefore, I have no objection to the
issuance of the COC.
Please let me know if you need more information or this notification in another form.

Al,
We've received an NOI from RPC Contracting, Inc for their site in Elizabeth City
(Pasquotank County) for coverage of their Asphalt Paving Mixtures and Blocks facility
under NCG16. The site discharges stormwater into Pasquotank River(SC). The NOI is
attached.
Does the Washington Regional Office have any concerns about issuing this facility a
COC for this general permit, and are there any potential impacts to wetlands? If we
don't receive any objections, we'll issue the CDC in 30 days.

Mr. Mizelle,
As I mentioned to you on the phone at the beginning of the week, I am going to inform the Raleigh Central Office
that I have no objection to the issuance of the NCG160194 general stormwater permit. Please see the note on the
front of the application (that was submitted) that an acceptable stormwater pollution prevention plan must be
submitted and utilized prior to operation of the plant or a discharge of stormwater from the site. The plan that was
submitted along with the application to Raleigh did not meet the requirements of the plan. Therefore, please send
me and the Raleigh office the plan prior to a discharge. The details that are expected in the plan are outlined in
the permit text, found by clicking the 2nd water drop, beside NCG 16, within the attached link, that we also
discussed:

Please make sure that the owner is aware of this requirement.
