Dear Mr. Davis:
On November 19, 2007, the Division of Water Quality received you request to rescind your coverage
under stormwater discharge Certificate of Coverage Number NCG 160105. In accordance with your
request, NPDES General Permit NCG 160105 is rescinded, effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to
waters of the State without valid coverage under an NPDES permit will subject the responsible party to a
civil penalty of up to $25,000 per day.
If the facility is in the process of being sold, you will be performing a public service if you would inform
the new or prospective owners of their potential need for NPDES permit coverage.

Dear Mr. Davis:
Enclosed please find a copy of the Stormwater Inspection Report for the
inspection, which was conducted by Larry Frost and I on June 27, 2006. The facility was
found to be in Compliance with permit NCG160105; however, the facility is no longer
located at the permitted site. APAC should offically request that Permit number
NCG160105 issued for the old site, be rescinded. It should also be noted, that the new
facility located off of Grovestone Road does not currently have a stormwater permit as
required. It is recommended that the attached Notice of Intent be completed and
submitted to the address on the NOI as quickly as possible.
Please refer to the enclosed inspection report for additional observations and
