Dear Mr. Hackler:
Staff from the Department of Environmental Quality (DEQ) conducted a stormwater inspection on
December 16, 2015, as part of a multimedia inspection of your facility. The assistance provide by Mr.
Jerry Storms and Mr. Johnny Williams during the inspection was greatly appreciated. The inspection
report is attached. Findings during the inspection were as follows:
1. Certificate of Coverage (COC) No. NCG 160080 under NPDES General Permit NCG 160000
was issued to Lane Gresham Lake Asphalt Plant to discharge stormwater runoff to Burdens
Creek, a WS-V, NSW stream, in the Neuse River Basin.
2. The facility has developed a stormwater pollution prevention plan (SPPP). A review of the
plan indicated that it included all components required by the general permit. It was noted
that the settling basin/pond should be included in the BMP summery and clearly
depicted on the site map. The General Permit in the file was the expired version and
needs to be replaced with the most recent version.
Semi-annual qualitative and analytical monitoring has been conducted at the only stormwater
discharge outfall (SDO1). However, no monitoring records were available for the last
quarter of 2014, and the first half of 2015 at the time of inspection, please explain why
monitoring was conducted during this period in your response to this letter. It was also
noted that TSS result from July 2015 sampling exceeded bench mark.
4. The lone outfall was observed during the inspection, which was found accessible and well
maintained.

On May 9, 2013, Thomas Ascenzo of the Raleigh Regional Office (RRO) conducted a compliance
inspection (CEI) of -Lane Construction Company's Gresham Lake Asphalt plant's stormwater pollution
program. The assistance of Johnny Williams, Asphalt Plant Manager and Kenneth P. Cates Assistant
District Manager was appreciated as it facilitated the inspection process.
The Lane Construction Company (Gresham Lake) Asphalt facility discharges stormwater to receiving
waters designated as UT of Gresham Lake in the Neuse River Basin.
The following observations were made:
1. The Stormwater Pollution Prevention Plan (SP3) was reviewed and revealed the
following :
• The plan contained the required general location map that included outfall
location and drainage areas.
• Employee training documents were included in the SP3. The last training was
conducted on February 2013.
• List of responsible parties for emergency spill response notification was available
for review.
• The plan included stormwater facility inspection reports, best management
summary, current certificate of coverage, narrative description of practices and a
spill prevention and response plan.
• The SP3 is being updated annually (or more frequently if needed) as required by
the permit.

We received your inspection report on January 28 and this letter will address items 2 and 3 of the report.
2. The facility has developed a stormwater pollution prevention plan (SPPP). A review of the plan indicated that it
included all components required by the general permit. It was noted that the settling basin/pond should be
included in the BMP summary and clearly depicted on the site map. The General permit in the file was the expired
version and should be replaced with the most recent version.
The settling basin/pond has been added to the BMP summary list attached to this letter. Also the map
now clearly depicts the settling basin on site. The valid General Permit has been placed into the binder and staff
have been updated once again on the revisions of the new general permit.
3. Semi-annual qualitative and analytical monitoring has been conducted at the only stormwater discharge outfoll
(SDO1). However no monitoring records were available for the lost quarter bf 2014, and the first half of 2015 at the
time of inspection, please explain why monitoring was conducted during this period in your response to this letter.
It was also noted that TSS results were high for July 2015 sampling exceeded benchmark.
This facility is currently shutdown and has been closed since 2014. This facility is visited by personnel
occasionally and staff have been sent to other facilities for the interim. A reminder that semiannual sampling is
required throughout the permit life alerted staff to restart sampling. The latest sample was high for TSS and is
being looked at for sampling improvement methods. We expect the next sample to be below the benchmark
taking us out of "TIER 1" status.

Staff from the Department of Environmental Quality (DEQ) conducted a stormwater inspection on
December 16, 2015, as part of a multimedia inspection of your facility. The assistance provide by Mr.
Jerry Storms and Mr. Johnny Williams during the inspection was greatly appreciated. The inspection
report is attached. Findings during the inspection were as follows:
1. Certificate of Coverage (COC) No. NCG160080 under NPDES General Permit NCG160000
was issued to Lane Gresham Lake Asphalt Plant to discharge stormwatcr runoff to Burdens
Creek, a WS-V, NSW stream, in the Neuse River Basin.
2. The facility has developed a stormwater pollution prevention plan (SPPP). A review of the
plan indicated that it included all components required by the general permit. It was noted
that the settling basin/pond should be included in the BMP summery and clearly
depicted on the site map. The General Permit in the file was the expired version and
needs to be replaced with the most recent version.
3. Semi-annual qualitative and analytical monitoring has been conducted at the only stormwater
discharge outfall (SDO1). However, no monitoring records were available for the last
quarter of 2014, and the first half of 2015 at the time of inspection, please explain why
monitoring was conducted during this period in your response to this letter. It was also

4. The lone outfall was observed during the inspection, which was found accessible and well
maintained.
