Guidelines for Access: The staff of the Raleigh Regional Office is dedicated to making public
records in our custody readily available to the public for review and copying. We also have the
responsibility to the public to safeguard these records and to carry out our day-to-day program
obligations. Please read carefully the following before signing the form.
1. Due to the large public demand for file access, we request that you call at least a day in
advance to schedule an appointment for file review so you can be accommodated.
Appointments are scheduled between 9:00 a.m. and 3:00 p.m. Monday —Thursday. Viewing
time ends at 4:00 p.m. Anyone arriving without an appointment may view the files to the
extent that time and staff supervision are available.
2. You must specify files you want to review by facility name or incident number, as appropriate.
The number of files that you may review at one appointment will be limited to five.
3. You may make copies of a file when the copier is not in use by the staff and if time permits.
Cost per copy is 2.5 cents for ALL copies if making more than 25 copies - there is no charge if
less than 25 copies are made. Payment is to be made by check, money order, or cash in the
administrative offices.
4. FILES MUST BE KEPT IN THE ORDER YOU RECEIVED THEM. Files may not be taken
from the office. No briefcases large totes etc are permitted in the file review area To
remove, alter, deface, mutilate, or destroy material in one of these files is a misdemeanor for
which you can be fined up to $500.00.
5. In accordance with General Statute 25-3-512, a $25.00 processing fee will be charged and
collected for checks on which payment has been refused.
6. The customer must present a photo ID, sign -in, and receive a visitor sticker prior to reviewing
files.

Dear Mr. Smith,
This letter is in response to the site visit preformed by Mitch Hayes on April 14"', 2011, and your
subsequent notification dated May 27", 2011, in regards to compliance with NPDES General Permit
No. NCG15000 at Johnston County Airport. As requested in the May 27'h notification, below is a
point by point response to the listed findings from Mr. Hayes's site visit.
1. The subject permit was issued and became effective January 29, 2010. A copy of the permit
was not available for inspection.
Airport Response: A copy of the permit is now kept on file at the airport as part of the
Stormwater Pollution Prevention Plan (SWPPP).
2. A Stormwater Pollution Prevention Plan (SWPPP) has not been developed and implemented
within the 12 months of the effective date of the initial Certificate of Coverage.
Airport Response: A SWPPP has been developed and implemented at the airport. A copy of
the plan is kept at the Airport and Implementation training for Airport Management, Staff and
SWPPP Team members was complete on July 12, 207 1.
3. Qualitative Monitoring has not been completed.
Airport Response: Qualitative Monitoring has not yet been completed. A document has
been filed with the SWPPP stating that since plan implementation (711212011) no run-off
producing storm events have occurred at a time when airport staff could collect samples as

required in Section B of the general permit. Airport staff is now trained and equipped to
complete wet weather monitoring and will do so as soon as possible.
Other applicable key requirements for the permit as identified in the May 27Ih notification have also
been addressed and are now in place. At this time the SWPPP has been implemented. Best
Management Practices for the facility have been identified and are being implemented as quickly as
possible.

9. Facilities that conduct aircraft and/or runway (including taxiways and ramps) deicing/anti-icing operations
must produce and Implement a plan for the minimization of the release of materials used for de-icing into
the stormwater system.
10.The SPPP should include Best Management Practices (BMPs) to control discharge of pollutants from e
facility's stormwater outfalls.
Please inform this office when a SPPP has been developed and implemented. Respond in writing to this
office to item number 1, 2, and 3 within 90 days receipt of this letter. Include in your response a copy of your
qualitative monitoring results. Please note that failure to abide by the conditions and limitations contained in the
permit may subject the Permittee to a civil penalty up to $25,000 per day per violationYo r appropriate response

In accordance with your application for a discharge.permit received on December 1, 2009,
we are forwarding herewith the subject certificate of coverage to discharge under the subject state
— NPDES general permit. This permit is issued pursuant to the requirements of North Carolina:
General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the
US Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage.
Per the requirements of the Neuse Riparian Buffer Rule, all storrnwater drainage to stream
buffers, from portions of this site that have been constructed afier July 22, 1997, must be
discharged through a correctly designed level spreader or another device that meets diffuse flow
requirements per 15A NCAC 2B .0233. Diffuse flow requirements are described in Chapter 3 of
the North Carolina Stormwater BMP Manual, available at:

This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land Resources,
Coastal Area Management Act or any other federal or local governmental permit that may be
required.
