On 4/13/2010, the Division of Water Quality received your request to rescind your coverage under
Certificate of Coverage Number NCG 140379. In accordance with your request, Certificate of Coverage
Number NCG 140379 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to
waters of the State without valid coverage under an NPDES permit will subject the responsible party to a
civil penalty of up to $25,000 per day. It is the intention of DWQ that enforcement proceedings will
occur for persons that have voluntarily relinquished permit coverage when, in fact, continuing permit
coverage was necessary. If, in retrospect „wyou feel the site still requires permit coverage, you should
notify this office immediately. Furthermore, if in the future you wish to again discharge to the State's
surface waters, you must first apply for and receive a new NPDES permit.
If the facility is in the process of being sold,,you will be performing a public service if you would inform
the new or prospective owners of their potential need for NPDES permit coverage.

The Division has reviewed and approved a request for modification to the above
referenced certificate of coverage (COC) received on your behalf from WAZ Engineering on
July 29, 2008. The COC now includes authorization to operate treatment systems and discharge
wastewater associated with aggregate pile wet -down. No other changes have been made.
Please find enclosed the revised COC. The old COC may be kept as reference for the
initial authorization for recycle system construction, stormwater discharges, and originally
proposed wastewater discharges. All other terms and conditions in the general permit remain
unchanged and in full effect. This permit modification is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, Coastal Area Management Act or any other
federal or local governmental permit that may be required.
This approval was based on the demonstration that treatment devices in place will be
adequate to treat wastewater associated with aggregate piles. If current erosion control devices
and/or site configuration prove inadequate to meet effluent limits in NCG140000, you will be
responsible for modifications to achieve compliance. Any future construction, installation, or
modification of wastewater treatment facilities will require an Authorization to Construct (ATC)
prior to construction per 15A NCAC 2H .0138 & .0139. You must submit, in triplicate,
plans/specifications and design calculations, stamped and sealed by a professional engineer, with
a letter requesting an ATC to the Stormwater Permitting Unit, 1617 Mail Service Center,

In accordance with your application for a discharge permit received on April 3, 2008, we
are forwarding herewith the subject certificate of coverage (COC) to discharge under the subject
state — NPDES general permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina
and the US Environmental Protection Agency dated October 15, 2007 (or as subsequently
amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or

An Authorization to Construct treatment facilities has been issued concurrently with
this COC. The Raleigh Regional Office, telephone number [phone], shall be notified at
least forty-eight (48) hours in advance of operation of the installed facilities so that an in -place
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted
facility has been installed in accordance with the NPDES Permit, the associated
Authorization to Construct, and the approved plans and specifications. Please mail the
certification (attached) to the Stormwater Permitting Unit, 1617 Mail Service Center, Raleigh,
NC 27699-1617.

15A NCAC 2H .0138 & .0139. You must submit, in triplicate, plans/specifications and design
