PLT Concrete Services, Inc. 12/27/06 Notification of material release at our concrete plant located at: 4541 Dozier Rd Rocky Mount, N.C. I, Rick Proctor, was notified of a Portland Cement release at our Plant 42 in Rocky Mount, N.C. on 12/21/06 at 8:15 am by our employee, Kenny Parrish. I was told that a cement tanker truck, owned by Sterling Transport Co.. was unloading into Out - cement silo and his hose came off our blow tube and released an estimated '/ to 1 ton of Portland cement on our plant yard. I first notified our Spill Response Team, Eastern Lnvironrnental Management and talked with Johnny Baines about our situation at about 8:20 a.m. I asked him to meet me at our plant site to access the spill and determine what action needed to be taken. I was at our Wilson N.C. office at the time and told him that I would meet him there in about 30 minutes. I then notified the Division ol'Air Quality and the Division of Storm Water at or about 8:30 a.m. The person with Air Quality, whose name 1 didn't get, said he would notify their staff by e-mail and if there were any concerns, 1 would be contacted. I talked to a woman in Water Quality and she said someone would call me shortly. I arrived at our plant at 9:00 am and met with Johnny Baines and another member of Eastern Environmental Management. We discussed the proper procedures for cleaning up and they left to mobilize their crew and equipment. It was decided that they would use a dust control material similar to `oil dry' to keep the dust particles at a minimum during clean up. I met with our employce. Kenny Parrish and asked him to inspect the two houses and vehicles across the street from our plant to determine if any cleaning up would be needed. He stated that the light breeze blowing toward their properties could have "dusted" their houses and cars. I instructed Kenny to take photos of those properties and autos, along with photos of our property from all angles. Upon my inspection, I found one eighth to one quarter inch of cement on the ground at the tanker truck unloading area, his rear facing Dozier Rd. Our plant yard was dusted also from the truck to our front wash down area. In talking with our plant manager, Dexter Clark, he estimated that it took the driver Steve Strickland of Sterling Transport, about 10-15 seconds to shut down his blower to cut off the cement discharge. I -le also stated that the driver had not properly fastened his hose to the truck and that likely caused it to come off during unloading.l then notified Sterling Transport's dispatcher about our problem and he said he would have someone contact me shortly. Kenny Parrish returned from our neighbors property across the street and stated that there was a "light dusting' on the 2 houses and 6 vehicles. He talked with both residents and they were in agreement and appreciative of our efforts to clean everything up. Both cleaning services arrived within 1 hour. About 9:45 am, Eastern Environmental Management returned to the plant. Bud Pridgen was the supervisor, along with 3 employees and equipment. We discussed clean up procedures and we both determined that the best way to control dusting was to spread "concrete sand" all over our yard on the paved areas; so the moisture in the sand could almost completely stop any dusting that might occur. I used our front end wheel loader to spread sand on our entire yard prior to their using the broom -tractor for sweeping and it worked great. There was no dusting at all during the sweeping. They swept our entire yard into the designated collection areas and cleaned up the waste material for disposal on our "returned concrete pile". The wet sand collected the cement dust better than expected and now will become part of our " recycled fill material' when our waste pile is crushed in January. This minimized using water to wash down our yard during cleanup. I was later contacted by the safety director and also the insurance company from Sterling Transport. I went over all the details of the spill and cleanup procedures with them both and they agreed that the situation was being handled correctly. I informed them that we had taken plenty of photos of all the dusted areas prior to, during and after the cleanup. I was also contacted by Ron Boone with the Division of Water Quality. He stated that he was calling in response to my report of a spill and we discussed what had happened and our plans for cleanup. He stated that ifthey needed any additional information, 1 would be contacted. I also discussed the proper procedures in the event of this kind of spill in the future with our "responsible parties" team. They include: Dexter Clark -plant manager. Kenny Parrish -area sales, Paul Strickland -quality control and environmental services and myself. We discussed all of the proper chain of contacts during this situation and the proper procedures for cleanup. I'm confident that our cleanup procedures worked well and was done as quickly as possible, to eliminate any additional problems due to this situation.
