Comment: Secondary containment has two areas of concern. Number 1. The release valve is missing.
There is lust a section of PVC pipe that allows anything in the containment area to empty
on to
the property. No valve. No lock. Number 2. The secondary containment area is filling up
with
process dust (concrete dust). I estimate its holding capacity is reduced by half.
The most recent date in the SPPP is 2007. No documented emDlovee trainina or evidence the
plan has been updated or reviewed since then.
Qualitative Monitoring Yes No NA NE
Has the facility conducted its Qualitative Monitoring semi-annually? 0 ❑ ❑ ❑

Comment: The facility has one documented outfall. It actually has 4 others. However, it's my opinion that
these 4 outfalls ultimately drain into the same location as the documented outfall. The facility
does use a sump pump to drain storm water into a ditch. This pump could be redirected to
pump storm water back to the wash pit for release at the documented outfall.
