(III) INSPECTION SUMMARY: On September 25, 2015, I (Thad Valentine) met with Robert Porter, Plant Manager, to conduct a compliance inspection of the facility. Mr. Porter and I first reviewed the permit and then took a tour of the facility. The plant was operating at the time but no trucks were being loaded during the inspection Overall the facility seemed to be run well. The equipment appeared wom, but well maintained. Records were complete and up to date. Because the silos are filled so infrequently, I was unable to observe one being tilled at the time of the inspection. (V) PERMITTED EMISSION SOURCES: 1. One central dust collector (1,433 squarefeet offilter area, CD-B3) attached to a cement storage silo (57 ton capacity, ES- CSI), a fyash storage silo (47 ton capacity, ES-FS2), a cement weigh butcher (10 cubic yard capacity, ES-BAT-CEI) and a truck loading operation with dust shroud (ES-TLP-2).. 2. One aggregate weigh batcher (10 cubic yard capacity, ES-BAT-AGI) (VI) SPECIFIC PERMIT CONDITIONS: A.1 Applicable Regulations: The Permittee shall comply with 15A NCAC Subchapter 2D .0202, 2D .0515, 2D .0521, 213 .0535, 2D .0540, 213.0611 and 2D .1806. In compliance. The facility was in compliance with all permit conditions. A.2 2D. 0202 Registration of Air Pollution Sources: The Director may require an owner or operator of a source of air pollution to register that source. In compliance. The facility has an air permit. A.3 Particulate Emission Limitations: Pursuant to 15A NCAC 2D .0515, particulate matter emissions from the affected emission sources listed in the permit shall not exceed allowable emission rates which are determined based on process throughput rates using the equations provided in the permit. In compliance. Compliance is achieved through the use of a well maintained bagfilter. A.4 2D .0521 Control of Visible Emissions: 'Visible emissions from the facility emission sources, established after July], 1971, shall not be more than 20 percent opacity. Incompliance. No emissions were observed due to the lack of activity during the inspection A.5 2D .0535 Notification Requirements: Facility is required to notify the Division of Air Quality if excess emissions are released for more than four (4) hours. In compliance. Mr. Porter stated that there was no reason to contact the DAQ due to a malfunction. A.6 2D .0540 Particulates from Fugitive Non -process Dust Emission Sources: The Permittee shall not cause or allow fugitive non -process dust emissions to cause or contribute to substantive complaints. Incompliance. There is no history of complaints concerning fugitive dust emissions associated with this facility. A.7 2D .0611 Fabric Filter Requirements: Particulate emissions from emission sources with bagfilters shall be controlled by the bagfilters identified in the permit. At a minimum, an annual internal inspection shall be conducted on each bagfilter. The Permittee shall maintain a bagfilter logbook on site which summarizes all of the bagfilter inspection and maintenance activities. In compliance. The facility conducts weekly inspections, monthly maintenance and an annual internal inspection on the bagfilter. All records were complete and up to date. New filters have recently purchased and installed according to the maintenance log A.8 2D. 1806 Control and Prohibition of Odorous Emissions: The permittee shall employ suitable measures to control odorous emissions from all emission sources. In compliance. There were no odorous emissions beyond the facility's boundries. No complaints have been received concerning odors at the facility. (VII) COMPLIANCE HISTORY: The facility has no history of violations with the RRO. Halifax County Dee Freeman Secretary On October 13, 2010 a stormwater inspection was attempted by Mr. Myrl Nisely of the Raleigh Regional Office. You were away from the plant, but Mr. Jason Rollman provided the Stormwater Pollution Prevention Plan binder (SP3) and a walk through the facility. The SP3 had not been updated since 2009. There is still time in 2010 to do so to maintain compliance with the permit. Changes were being made at the plant, as described in the attached inspection report. These will need to be reflected on a revision of the site plan (map). This office plans to make a return inspection near the end of this year -or early in 2011 to complete this visit with you. An employee from the Rocky Mount plant, Jason Rollman, answered what questions he could and provided a tour of the facility. Construction is underway to build a new containment area for admixture tanks and new cells for the storage of various aggregates. The SP3 had information for 2009 and earlier, but not for 2010. It was agreed that DWQ would make a return visit in a few months, giving time for completion of the new configuration and an update of the manual. Sampling, both Quantitative and Qualitative, employee training and a revision of the site map to reflect the new features should be addressed by the end of this year in order to be in compliance. Halifax County Dear Mr. Porter: Dee Freeman Secretary On June 13, 2011 Mr. Myrl Nisely of the Raleigh Regional Office made a stormwater inspection at the Roanoke Rapids facility. This was a follow-up to the incomplete inspection made on October 13, 2010 when you were away from the plant. The plant was in compliance. Please see the attached inspection report for full details. A few of reminders: • Employee training has not yet been done for 2011. • Two Qualitative Monitoring sessions are needed for 2011, as well. • The current permit expires at the end of June 2011, and new one will be issued early in July requiring stormwater sampling twice a year. Therefore, one more Analytical Monitoring round is need by the end of this year. Be sure the lab supplies the correct bottles for both TSS and Settleable Solids (SS). The round in April of 2011 did not report SS. • Note, too, that the new permit will require sampling four (4) times per year of wastewater. In other words, if truck wash water or other process contaminated water travels to the outfall, more frequent sampling will be required. On the other hand, uncontaminated stormwater sampling is now twice per year. • You may discard records from your Stormwater Pollution Prevention Plan manual that are older than 5 years; in other words, year 2005 and older. In response to item 3, we will be making repairs to the damaged wall in which the truck wash is stored. As for the size of the secondary containment being large enough, I have contacted our admixture supplier about downsizing the storage tank sizes. We were already looking at possibly reworking this area in early 2009 to make it more efficient & practical. In response to item 4, I have contacted Dana Satterwhite in the Laboratory Certification branch to see what our options are on the pH testing within the 15 minute time frame. Dana was very helpful & made me aware of a couple of things we can do to correct this procedure. We will follow up on what is most feasible and efficient to ensure that we are meeting the proper guidelines. We will have this taken care of in a timely manner & I will keep you up to date on our progress. I have begun to research the size of the containment in Roanoke Rapids. Repairs will be made to the secondary containment & if size is a problem that will also be addressed. Mr. Robert Porter's cooperation and assistance with this inspection was appreciated. Please find attached with this letter the Basin Wide Information Management System (BIMS) checklist. Below is a list of findings and recommendations developed from the inspection. 1. The current permit was reissued 08/01/2004 and expires 07/31/2009. Please request permit renewal no later than 180 days prior to permit expiration to avoid enforcement action being taken. 2. According to conservation with Mr. Porter, all roller washwater is discharge to a wash pit. The concrete settles and solidifies, and then hauled away. The washwater is pumped to a previously used polyethylene tank that was previously used for chemical storage. The chemical label is still affixed to the container. Mr. Dickens' help was greatly appreciated. verview This program shows generally good attention to the documentation requirements in the permit, but deficiencies in properly handling stormwater/process wastewater. The SPPP manual contained records that showed excellent program planning, inspections and monitoring, housekeeping and other details, thereby demonstrating that implementation of the program has begun. A few "tune-up" points for the SPPP manual are listed below. Regarding physical conditions at the plant, however, there is a need for engineered Best Management Practices (BMPs) to control a combination of process water plus a 6" rain in a 24 hours period (the definition of a 10 year storm in most of NC). Details The SPPP is very well done and contains forms for all the required elements. Most sections have data, but the following changes still are needed: 1. A checklist has been placed under the front cover of the manual as a reminder of when actions are needed. This helpful tool might as well be filled in as inspections are made, sampling done and so forth. For example, the 7/11/02 activities. Reminder —the Fall sampling and other semi-annual activities are now due (September — November). Ready Mix Concrete, October 7, 2002 p. 2 2. The page designating the corporate official responsibleforthe program needs to be filled in and signed. This is apparently Mr. Larry Spence. 3. There is a page in the back of the book that further lists the Regional Manager, the Division Manager, and the Plant Manager positions. Please fill in the details for these positions. It is recommended that this page, needed for section A(6), be placed immediately after the page discussed above. 4. Employee Training was the one "empty" section lacking documentation. For instance, the session held with Mr. Dickens on 7/11/02 appeared to be a training meeting, so should have been noted by his signature as well as that of the trainer. Please note — general awareness training is to be given annually to all employees, not just the Plant Manager. See the topics in Part II Section A(4) 5. The USGS map for Part II Section A (I.)(a) lacks some detail requested by the Division. Please prepare a USGS-type map "zoomed in" somewhat to show the facility's location in reference to transportation routes (as the present map does) and the route of all stormwater discharges all the way to the receiving water(s). Clearly identify the name of receiving stream(s). 6. The plant site map gives better than average detail, but please add lines showing the underground piping from the grates to the discharges. There are numerous arrows showing the directions of flow. Please designate the approximate areas being served -by the runoff arrows (Section A(1)(c). These areas will help provide estimates of total flow passing through a sample point during a sampling event (see footnote 4 under Table 3.). Add a map for the lot across the street from the office building. This is where most of the process activity is done, such as the discharge of unused concrete into blocks and the drum wash dump pond, yet this area is largely ignored in the SPPP. It was unclear where vehicle and equipment cleaning takes place. Before drafting this map, consider the comments in bold below. 8. A means of showing that an update of the SPPP was accomplished at least annually should be provided (and more often, if major changes are made at the plant). The Division does not see evidence that engineered measures have been taken to control stormwater from a 10 year storm. Physical changes needed at this plant are as follows: a. Secondary containment is needed for storage of admixture chemicals per Section A(2)(b). This must be addressed as soon as possible. b. pH control must be assured to meet the 6 to 9 units specified in the permit. Measurement of the water in the culvert just before the railroad track was 9.4 pH. Waters collected across the street for eventual discharge (if the area is redesigned to do so) must also meet the upper limit of 9 pH. c. The entire lot across the street shows potential for release of contaminated rainwater during a heavy storm. The holding areas are designed for temporary storage and evaporation of process water during a dry period, but appear inadequate for a big rain. The Division of Water Quality is working with several of the larger readymix concrete companies to help establish what is needed to meet General Stormwaler Permit 140000. This letter is to document the topics we covered. The Roanoke Rapids stormwater manual (SP3) is quite well organized. Your letter indicates that the SP3 now contains the beginning of a growing body of data for the numerous areas covered by the permit — regular inspections, employee training, records of draining secondary containment and stormwater sampling results to name a few. These records are to be retained for five years, so on year six, the oldest can be discarded and so on into the future. Federal regulations provide that the public has the right to request a review the SP3 through the pennitting authority, if they so desire. The biggest problem we are finding is the lack of engineering design in older plants. Existing pits are often used, or new ones created, without much consideration for the design requirements scoped out in 15A NCAC 2H.1000. Contracting with a reputable engineering firm is required by 15A NCAC 2H.1008 0) to review the needs of a given site and provide appropriate planning and design. The high pH waters typical of your process are potentially bad not only for surface water, but for groundwater, as well. Pits are to have liners able to meet percolation of no more than 1 x 10` cm/sec, for example. There are limits on how close the bottom of a pit can be to the highest groundwater level. Your willingness to better plan actions to be taken "across the street" is appreciated.
