This letter is in response to item #3 needing attention/comments on the inspection at our Apex facility, COC #NCG 140073, on August 5, 2015. First of all, thank you for taking the time to perform this inspection with our Operations Manager, Jay Watkins, and myself. Your inspection was thorough and your comments and advice were well taken. I apologize for the delay in responding, but we have tried to set up a time for our internal Environmental Manager and our outside Environmental Consultant to come and visit the site to observe and confirm, but we were not able to set that up before the deadline for a response. Here is the response to item #3 listed: 3. Starting in 2016, we plan to move SD02 to the new location that you noted on the inspection and will start sampling for analytical monitoring at this location. This location is on the east half of the facility and enters the receiving stream at the downstream culvert on the south side of the unpaved road. We plan to eliminate the current SD02 at the southeast corner of the facility as an SDO. We will note the location on our site map in handwriting. In early 2016, we plan to get our Environmental Consultant and Environmental Manager to the plant to visually confirm they are in approval. If they have any questions or concerns about the new SD02 site, we will get with you at that time. If they do not, we will plan to get an updated site map completed to note the new location. Again, thank you for taking the time to inspect our facility and communicate your observations and comments with us. The facility has developed a stormwater pollution prevention plan (SPPP). A review of the plan indicated that it included all of the components required by the general permit. 3. Semi-annual qualitative and analytical monitoring has been conducted as required by the permit at the both stormwater discharge outfalls (SDOs). However, samples from SDO 2 were actually collected in stream at the southeast corner of the facility, where runoff leaves the facility via sheet flow. Runoff from east half of the facility enters the receiving stream near the downstream culvert on the south side of the unpaved road, this stormwater entry point should be identified as a SDO, at which analytical monitoring samples should be collected. 4. Both SDOs were observed during the inspection. SDO1 was maintained and accessible. Williams Street, in Apex, Wake County. From RRO, take the outer Beldine and stay on Hwy I South to the Hwy 55 exit at Apex. Turn left, Hwy 55 South, and go about 1/4 mile. Plant is located on the left side of the road and has a Ready Mixed sign out front. (II) FACILITY DESCRIPTION: Southern Equipment Company - Plant 46 is a truck mix concrete batch plant. At the time of the inspection the facility was in operation. A hard hat is recommended for this inspection. (III) SAFETY: As with all facility visits, standard safety gear (steel -toed shoes, hard hat, safety glasses and hearing protection) is to be worn. (IV) INSPECTION SUMMARY: On August 5, 2015, I, Cheng Zhang, inspected the Southern Equipment Company facility dba Ready Mixed Concrete Company - Apex Plant (as part of a multimedia inspection). I arrived at the location at approximately 9:30 PM and met with Jay Watkins, the Operational Manager, and Mr. James Walters, the Division Manager. Mr. Watkins and I reviewed the permit and the inspection reports for the dust collector. During my visit I observed several batch trucks being loaded. No visible emissions were observed from any of the plant operations. I left the facility at approximately 11:30 AM. At the time of the inspection, the facility appeared to be in compliance with their air permit. An unpaved public road (Prestine Water Dr.) runs through the facility, dust emissions can be caused by all through traffic. *** A.7 FABRIC FILTER REQUIREMENTS including cartridge filters, baghouses, and other dry particulate collection devices - As required by 15A NCAC 2D .0611, particulate matter emissions shall be controlled as described in the permitted equipment list. a. Inspection and Maintenance Requirements - To comply with the provisions of this permit and ensure that emissions do not exceed the regulatory limits, the Permittee shall perform, at a minimum, an annual (for each 12 month period following the initial inspection) internal inspection of each bagfilter system. In addition, the Permittee shall perform periodic inspections and maintenance as recommended by the equipment manufacturer. ***In Compliance. The bag filter system is inspected weekly and noted in the logbook. *** b. Recordkeeping Requirements - The results of all inspections and any variance from manufacturer's recommendations or from those given in this permit (when applicable) shall be investigated with corrections made and dates of actions recorded in a logbook. Records of all maintenance activities shall be recorded in the logbook. The logbook (in written or electronic format) shall be kept on -site and made available to DAQ personnel upon request. * * * In Compliance. All records were complete and up to date and were dated back to January 2014. *** (VIf) EXEMPT EMISSIONS SOURCES: No exempt/insignificant sources were observed during the inspection. (VIII) 112r APPLICABILITY: This regulation is not applicable to this facility. (DC) STACK TEST REVIEW: There are no stack tests on record for this facility (X) COMPLIANCE HISTORY: The facility was issued a Notice of Deficiency on January 9, 2015 for late submittal of their air permit renewal application and emission inventory for calendar year 2013. (XI) CONCLUSIONS AND RECOMMENDATIONS: At the time of this inspection, Southern Equipment Company Apex Plant 46 appeared to be in compliance with all permitting requirements. I recommend the facility be re -inspected within two years. The helpful cooperation of Mr. Clavon Wiggins was greatly appreciated. Overview As noted at other Ready Mix locations, this program shows generally good attention to the SPPP documents required in the pernut, but there are deficiencies in properly handling stormwater/process wastewater. Violations of the TSS limit at every sampling point have occurred since at least 1999. Sampling practices will be discussed below. The SPPP manual contained records that showed good program planning, inspections, monitoring, housekeeping and other details that demonstrate implementation of the program. A few "tune-up" points for the SPPP manual are listed below. Physical conditions at the plant still need modification to provide engineered Best Management Practices (BMPs) to control both process water and stormwater as per the permit. Details The SPPP is very well done and contains forms for all the required elements. Most sections have data, but the following changes still are needed: The good detailed description of the site found in an early section needs to be updated. For instance, secondary containment has been added, and other details will need to be revised, especially in light of changes requested below. A page near the beginning of the manual declaring the dates of annual updates is needed, as well. 2. The USGS map for Part II Section A (l.)(a) lacks some detail requested by the Division. Please prepare a USG S-type map "zoomed in" somewhat to show the facility's location in reference to transportation routes (as the present map does) and the route of all stormwater discharges all the way to the receiving water(s). For example, where does the culvert under Hwy 55 eventually go? Clearly identify the name(s) of receiving stream(s). Please send a copy of this map to the Division. Please also send a copy of this map to the Division. a) Please designate the approximate areas being served by each discharge point (Section A(1)(c). These areas are apparently being used to calculate the total flow passing through each sample point during a sampling event. b) Add the lot across the street from the office building. This is where vehicle and equipment cleaning takes place, so show the runoff flow patter. c) Show the new admixture chemical containment structure. 4. The Division wishes to make sure that control of process wastewater and stormwater is adequate to meet permit limits at every discharge point: a. The drum wash pit is being used as a detention basin. At present, water in this pit evaporates and may contribute to impacts to groundwater. Therefore, any pit/containment structure should be built such that groundwater as well as water quality standards are protected. b. pH control must be assured to meet the 6 to 9 units specified in the permit at every discharge/sampling point. Limits for TSS and settleable solids must likewise be met. c. The lot across the gravel road shows potential for release of contaminated rainwater. This vehicle cleaning area must be designed to provide sampling during a rain per Section C. Provide engineered structures for this part of the property to fully control/treat the stormwater and process wastewater. At present some or all of this flow goes to Middle creek on that side of the gravel road. d. Sheet flow to Point #3 has no particular structural control and a small vegetative buffer before entering Middle Creek. This vegetation is inadequate to reduce solids. Investigate methods for assuring removal of solids to less than 30 mg/I in the flow to the creek. 5. Sampling methods being used are probably not providing analytical results representative of water quality that reaches Point #3 Middle Creek, nor water quality from the Drum Wash Pit. You may want to pursue installing a more permanent pit from which to collect samples, versus cutting a temporary pit on the day of sampling. The second Statement was about a zoomed in USGS-type map. Our plan includes a USGS map of the site. We have a narrative in our plan that provides the information you have asked for. I have tried to blow up a USGS map to the scale that you have asked for and the information that you asked for is still not shown. I have included a copy for your review. The only way to give you the information that you are asking for is to hand writes it on the USGS map. The wash pit is going to be evaluated by Mid Atlantic. I will send you a copy of the updated site plan and their recommendations for this wash pit once they are completed. The lot across the street is only used to pour recycled concrete block forms and for drivers to mix their loads on busy days. We will add this to our site plan and start testing this water. Discharge point # 1 will be removed from our site plan since it is sheet flow that is filtered by 175' of vegetation.
