The assistance provided by Mr. Jay Watkins, Operations Manger, and you during the inspection was greatly appreciated. The inspection report is attached. Findings during the inspection were as follows: 1. Certificate of Coverage (COC) No. NCG140071 under NPDES General Permit NCG140000 was issued to Ready Mixed Concrete — Louisburg Plant #9 to discharge stormwater runoff and process wastewater to an unnamed tributary to Tar River, a WS-IV, NSW stream, in the Tar -Pamlico River Basin. 2. The facility has been operated on demand (concrete was batched for only two days in 2015). The facility has developed a stormwater pollution prevention plan (SPPP). A review of the plan indicated that it included all of the components required by the general permit. 3. Semi-annual qualitative and analytical monitoring has been conducted as required by the permit at the both stormwater discharge outfalls (SDOs). 4. Both SDOs were observed during the inspection. The outfalls were found maintained and accessible. From FRO, take Hwy. 401 North to Louisburg. At the intersection with Bickett Blvd and Hwy. 56 continue straight as if going to the downtown area. Go past McDonald's (which is on the right) and go through the next stoplight at the shopping plaza, then immediately turn left onto Hill Street. Take the first right onto Averitte Street which tuns into Perry St. at a sharp left turn. Immediately after this turn, bear right onto Seaboard Street. The facility office is on the left and the parking lot is on the right. PF (11) FACILITY DESCRIPTION: Ready Mixed Concrete Company — Plant #9 is a concrete batch plant. This facility operates under Air Permit No. 6585R06, issued on July 29, 2013 and expires on June 30, 2018. (III) INSPECTION SUMMARY: On August 4, 2015, I (Cheng Zhang) arrived at this facility to conduct a compliance inspection. Upon arrival I met with Mr. James Walters, and Mr. Jay Watkins where we reviewed the conditions of the facility's current air permit and the facility's logbook. Afterwards, I toured the facility, which was not in operation at the time. Overall, the facility was operating on demand; it was in operation for only 2 days in 2015. Include the following as part of your personal protective equipment: a hard hat, blaze orange safety vest, earplugs and safety glasses for personal protective equipment. Fugitive Dust Control Requirement - As required by 15A NCAC 2D .0540 "Particulates from Fugitive Dust Emission Sources," the Permittee shall not cause or allow fugitive dust emissions to contribute to substantive complaints or excess visible emissions beyond the property boundary. ***IN COMPLIANCE — Mr. Watkins said whenever the facility is dusty; they water the lot with a hose. No dust complaints have been reported or noted regarding this facility.*** A.7. As required by 15A NCAC 2D .0611, Bagfilter Inspection and Maintenance Requirements: Particulate emissions from emission sources with bagfilters shall be controlled by the bagfilters identified in the permit. At a minimum, an annual internal inspection shall be conducted on each bagfilter. The Permittee shall maintain a bagfilter logbook on site which summarizes all of the bagfilter inspection and maintenance activities. ***IN COMPLIANCE _Mr. Watkins and I reviewed the maintenance logbook, which included monthly bag filter inspections. The last internal bag filter inspection was on July 24, 2015.*** (VI) COMPLIANCE HISTORY: The are no violations recorded for this facility in (BEAM. (VII) STACK TEST REVIEW: There are no stack tests recorded for this facility in (BEAM. (VIII) CONCLUSIONS/RECOMMENDATIONS: At the time of the inspection, Ready Mixed Concrete Company appeared to be in compliance with all permitting requirements. Seaboard St. Facility, Louisburg, NC NPDES Stormwater Permit NCG140071 Franklin County Dear Mr. Walters: On June 20, 2008 Myrl Nisely of the Raleigh Regional Office of Division of Water Quality made a compliance inspection at the Louisburg facility. The help provided by driver Jim Jones in the absence of Donald Starling was appreciated. Plant Manager Mr. Starling was on vacation. Our time was somewhat limited because Mr. Jones had delivery commitments. See the attached checklist. Comments are: The SPPP manual had been updated in March 2008. However, a list of responsible parties with phone numbers could not be located and the most recent employee training records were for 2005. After a telephone conversation since the inspection, Mr. Lyman Austin provided employee training records, the latest being March 2008. Please insert these pieces of information into the manual in Louisburg to maintain compliance. 2. This location had reported high TSS results in past years. The latest data for 2/13/2008 showed compliance values for TSS of 25.6 mg/I and 73 mg/I for the two outfalls, well within the benchmark of 100 mg/l. pH results were reported to be 7.8 and 7.2 standard units. The outfall immediately across Seaboard St. from the office has an exit pipe that delivers the flow to a 5 or 6 ft long bed of crushed stone. At the end of the bed is a crushed stone berm through which rainwater'filters before being released into a broad channel. The bed was coated with cementous solids (see attached picture). The channel did not show evidence of that kind of material, so the filter system appears to be effective for removing solids (TSS). However, attention should be given to regular maintenance of this filter. Also, be reminded that pH of stormwater leaving the site is to be 6 to 9 pH units. 4. The vehicle rinse and drum washout pit had about 1.5 feet of freeboard. It is used very little (about four washouts per week). At the lowest point on the berms, and estimated 1.5 to 2ft of freeboard are left. DWQ recommends cleaning out the pit soon to maintain 1.5 ft as the minimum freeboard, to be ready for heavy rains. The pit is used for about 4 truck washouts per week, fewer than it used to be. Delvo is used much of the time, eliminating the need for a washout. The outfall across the street from the office shows evidence of collecting gray cement -like solids, but there are no such solids further downstream. So TSS is controlled, but this facility has no method of attenuating high pH. Seaboard St. Facility, Louisburg, NC NPDES Stormwater Permit NCG140071 Franklin County Dear Mr. Spence: On April 19, 2004 Jonathan Diggs of the Central Office and Myrl Nisely of the Raleigh Regional Office of Division of Water Quality made a compliance inspection at the Louisburg facility. The help provided by John Wegener was appreciated. See the attached checklist. Comments are: 1. The SPPP manual contained all the required components. Some data sheets were not filed in their proper sections, resulting in searches for evidence of the most recent activities. Mr. Wegener agreed to refile and organize the records. Eventually records for at least 3 years were located. 2. This location had reported high TSS results in past years. The latest data for 10/9/03 showed TSS = to 8.5 and 9.0 mg/l for the two outfalls, well within the permit limit of 30 mg/l. The largely dirt/crushed stone surface at this site creates dust and fines that will be transported to the first grate by a heavy rain. The company is urged to keep the concrete drain leading to that grate swept regularly, and to use other Best Management Practices to assure all runoff is less than the 30 mg/l TSS limit. The 1000 gallon tank of diesel fuel is exempt from a secondary containment requirement. There was a 2002 revision of the rule that changed the threshold from 660 gallons to 1320 gallons. Nevertheless, we suggest protecting the tank from being hit by heavy equipment by placing a row of the concrete blocks at the front and along the south side of the tank. Those blocks are conveniently produced across the street. 4. The site map showed only the production facility on the west side of the street, with no mention of activities on the other side of the street. Even though your company does not own the land across the street, please revise the map to include the activities taking place there. We recommend that it be placed inside the containment structure, along with any 55 gallon drums containing chemicals. A change is soon to occur in the vats within the structure (Grace is being replaced by another company), so there may be more room in the future. The vehicle rinse and drum washout pit is functioning as a retention pit from which water evaporates and travels into the ground. Usage is of a low enough volume that overflows do not occur.
