On February 17, 2016, the Division of Energy, Mineral and Land Resources received your request to
rescind your coverage under Certificate of Coverage Number NCG050335. In accordance with your
request. Certificate of Coverage NumberNCG050335 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to
waters of the State without valid coverage under an NPDES permit is against federal and state laws and
could result in fines. If something changes and your facility would again require stornnvatcr or
wastewater discharge permit coverage, you should notify this office immediately. We will be happy to
assist you in assuring the proper permit coverage.
If the facility is in the process of being sold, you will be performing a public service if you would inform
the new or prospective owners of their potential need for NPDES permit coverage.

This letter is in follow-up to the NPDES Stormwater Permit Compliance Inspection conducted on January 8, 2016.
The facility was found to have ceased all manufacturing operations and to have only office activities ongoing until
April 2016.
Based on these findings, a Rescission Request Form has been enclosed. On or before March 1, 2016, please complete
this form and return it to my attention at ARO — NCDEQ, 2090 US Highway 70, Swannanoa, NC 28778.

Inspection Summary:
DWQ staff was met on site by David Nix and others. A tour was conducted of the exterior of the facility, observing the
stormwater conveyances and outfalls and site conditions. During the visit, the SPPP manual was reviewed, operational
activities and existing stormwater exposures were discussed. Permit requirements were also reviewed as pertaining to
the most recent permit version, dated June 1, 2008. At the time of the inspection the site was found to be in compliance
with the permit. The inspector noted that the facility appeared to maintain good housekeeping practices and records were
found to be in good order.
Some discussion was held regarding the type of manufacturing conducted at the facility (metal seals), pertinent SIC
coding and permit applicability of the NCG05 permit. Upon further research, DWQ considers the NCG05 permit to be the
proper industrial stormwater permit for the facility.
The inspector noted that some materials were exposed to stormwater on the site. including, metal components, recycling
bins and certain disused equipment/materials. Exposure to stormwater could be reduced by providing additional covered
areas for these items.

In response to your renewal application for continued coverage under stormwater General Permit NCG050000 the Division of
Water Quality (DWQ) is forwarding herewith the reissued stormwater General Permit. This permit is reissued pursuant to the
requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently amended.)
The following information is included with your permit package:
• A new Certificate of Coverage
• A copy of stormwater General Permit NCG050000
• A copy of a Technical Bulletin for the General Permit
• Five copies of the Discharge Monitoring Report (DMR) Form
• Five copies of the Qualitative Monitoring Report Form
The General Permit authorizes discharges of stormwater only, and it specifies your obligations with respect to stormwater
discharge controls, management, monitoring, and record keeping. Please review the new permit to familiarize yourself with all
the changes in the reissued permit.
The more significant changes in the General Permit since the your last Certificate of Coverage include the following:
• Permit cover page — Coverage is expanded to include industrial activities similar to the categorically identified industries.
• Part I Section A — This part explains that the No Exposure Exclusion from permitting option may be available to the
permittee.
• Part II Section A — Several minor expansions and clarifications of the required content of the Stormwater Pollution
Prevention Plan have been added. Please be sure that your plan is updated to comply with the new requirements.
• Part II Section B — Qualitative monitoring must now be conducted during a representative storm event.
• Part II Section B — The permittee is required to keep a written record of his response to problems identified by his
qualitative monitoring observations.
• Part II Section C - For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provisions require that discharges from the vehicle maintenance areas be sampled twice per year, and that the
sampling results be compared to new numerical benchmark values. (The previous cutoff concentrations have been
eliminated.)
• Part 11 Section C — For facilities with vehicle maintenance activities using more than 55 gallons of new motor oil per month,
new provisions require the permittee to execute Tier One and Tier Two response actions, based on the first benchmark
exceedence (Tier One) and the second consecutive benchmark exceedence (Tier Two). Tier Two requires that the
permittee institute monthly monitoring instead of twice -per -year monitoring, until three consecutive monitoring events
show no benchmark exceedences.
• Part II Section C — This part contains a new provision that four exceedences of any particular benchmark will trigger
increased DWQ involvement in the permittee's stormwater management and control actions. DWQ may direct the
permittee to apply for an individual permit, or may direct the implementation or installation of specific stormwater control
measures.

Your coverage under the General Permit is transferable only through the specific action of DWQ.
This permit does not affect the legal requirements to obtain other permits which may be required by DENR, nor does it relieve
the permittee from responsibility for compliance with any other applicable federal, state, or local law, rule, standard, ordinance,
order, judgment, or decree.

to receiving waters designated as Cartoogechaye Creek, a class WS-III;Tr,CA waterbody in the Little
Tennessee River Basin in accordance with the effluent limitations, monitoring requirements, and other
conditions set forth in Parts I, II, III, IV, V, and VI of General Permit No. NCG050000 as attached.
This certificate of coverage shall become effective July 17, 2008.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

On February 17, 2016, the Division of Energy, Mineral and Land Resources received your request to
rescind your coverage under Certificate of Coverage Number NCG050335. In accordance with your
request, Certificate of Coverage Number NCG050335 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to
waters of the State without valid coverage under an NPDES permit is against federal and state laws and
could result in fines. If something changes and your facility would again require stormwater or
wastewater discharge permit coverage, you should notify this office immediately. We will be happy to
assist you in assuring the proper permit coverage.
If the facility is in the process of being sold, you will be performing a public service if you would inform
the new or prospective owners of their potential need for NPDES permit coverage.
