Resources (DEMLR) is forwarding herewith the reissued Certificate of Coverage and
Stormwater/Wastewater General Permit. This permit is reissued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15, 2007 (or as subsequently
amended.) The following information is included with your permit package:

The General Permit authorizes discharges of stormwater and/or wastewater (as applicable to your site),
and it specifies your obligations with respect to discharge controls, management, monitoring and record
keeping. Please review the new permit to familiarize yourself with all changes in the reissued permit.
Significant changes to the General Permit are outlined in the Technical Bulletin.
Your coverage under the General Permit is transferable only through the specific action of DEMLR. This
permit does not affect the legal requirements to obtain other permits which may be required by the N.C.
Department of Environmental Quality, nor does it relieve the permittee from responsibility for
compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order,
judgment or decree.

In accordance with your application for a discharge permit received on October 7,
2013, we are forwarding herewith the subject certificate of coverage to discharge under the
subject state - NPDES general permit. This permit is issued pursuant to the requirements
of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between
North Carolina and the US Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
This certificate of coverage is not transferable. If the facility changes ownership or is
closed, the Division of Energy Mineral & Land Resources may require modification,
revocation or reissuance of the certificate of coverage.
Per the requirements of the Neuse Riparian Buffer Rule, aii storaiwater drainage to
stream buffers, from portions of this site that have been constructed after July 22, 1997,
must be discharged through a correctly designed level spreader or another device that
meets diffuse Flow requirements per 15A NCAC 2B .0233. Diffuse Flow requirements are

effect of pumping and to establish any pumping regime necessary to reduce impacts, and (2) Detailed
plans to maintain surrounding hydrology and respective monitoring to demonstrate compliance. See

(3) For a municipality, state, federal, or other public agency: by either a principal executive officer or ranking elected official.

GENERAL PROJECT INFORMATION
The Stone Creek Sand & Gravel Mine (Permit No. 51-45) is located approximately 10
miles south of Smithfield on Hwy 701 in Johnston County. Currently approximately
10.09 acres of land is within the mine permit boundary with 8.7 acres to be disturbed
within the next 10 years. The proposed modification will add 21.5 acres to the permitted
acreage, with 18.9 acres to be disturbed within the next 10 years. The mine areas are
connected by an existing road with an easement agreement.
Total land disturbance within the next 10-years will be 27.6 acres and includes the access
road within the easement agreement. Total permitted land area will be 31.59 acres. The
proposed mine expansion area will use wet detention ponds for erosion control runoff.
The existing mining operation uses front end loaders to excavate for fill dirt, sand, and
gravel. The proposed mine expansion area will be conducted in a similar fashion. Mined
material may be washed and screened periodically at the mine. A recirculation system
will be installed for use in the screening process. There will be no structures except the
screen at the proposed plant area.
Water usage will be approximately 400 gallon Is per minute (0.89 cfs). The water will be
pumped to the recirculation system from the wet detention pond for makeup water. The
recirculation system will not discharge to the wet detention pond.
HydroCAD was used to evaluate the runoff volumes and peak flow rates for a 1-inch
rainfall, as well as the 10-year and 25-year rain events. Erosion control for the proposed
mining area will consist of wet detention basins with perimeter berms to divert the
stormwater runoff to the basins.
The proposed basins will have corrugated metal riser pipes with two (2) holes (2"
diameter) drilled in each riser to slowly drain the basin when the water level goes above
normal pool. A trash rack which covers the top of the riser m well as the 2" holes is
provided for each riser. The system will allow the water to slowly drain out of the
structure for a rainfall event greater than I -inch. The CMP riser pipe inverts will be
placed 6" above the drilled holes. Based on the pipe elevations, watersheds, and basin
storage data, the maximum rainfall depth before the CMP risers begin to discharge is
2.24" for Basin 4 and 2.12" for Basin 5. The basins can pass the 25-year rain event with
one foot of freeboard.
The recirculation system is designed to settle the 0.017 millimeter particle size. Both basins
will have a surface area of 2,000 sf. An 8" Schedule 40 PVC pipe will connect the two
basins.
Upon completion of mining, two (2) lakes will be formed. Basin 3 will be formed into an
approximately 0.9 acre lake. Basin 5 will be an approximately 3.0 acre lake. The
perimeter berms will be removed. Along the perimeter of the mine, ground will be cut to
3:1 (H: V) or flatter slopes. AM areas will be sloped for positive sheet water flow to the
lakes.
