Resources (DI -MLR) is forwarding herewith the reissued Certificate of Coverage and
Stormwater/Was(ewater General Permit. This permit is reissued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between the state of North
Carolina and the U.S. Environmental Protection Agency, dated October 15. 2007 (or as Subsequently
amended.) The following information is included with your permit package:

The General Permit authorizes discharges of stormwater and/or wastewater (as applicable to your site),
and it specifies your obligations with respect to discharge controls, management, monitoring and record
keeping. Please review the new permit to familiarize yourself with all changes in the reissued permit.
Significant changes to the General Permit are outlined in the "Technical Bulletin.
Your coverage under the General Permit is transferable only through the specific action of DE-MI-R. This
permit does not affect the legal requirements to obtain other permits which may be required by the N.C.
Department of Environmental Quality, nor does it relieve (lie permittee from responsibility for -
compliance with any other applicable federal, state, or local law, rule, standard, ordinance, order,
judgment or decree.

Inspection Summary:
Wilson Stone was issued a stormwater permit in January 2012. The site has not been inspected for stormwater since the
permit was issued. Due to lack of business - the mine was not operating for the entire 2013 year.
We discussed the stormwater permit requirements with Josh Hughes at the site. They have a copy of their permit and site
map. The map shows 5 outfalls for the quarry. The map needs to be updated to show 2 outfalls instead of 5. They do not
have a stormwater pollution prevention plan. We told Josh he could get with his consultant to have one made or he could
do the plan himself. We also told him to read the sampling requirements and to start qualitative/analytical monitoring.
They are maintaining measures on site and the site looks good otherwise.

subject state — NPDES general permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land

dredging or dipping operations)
For new mining sites that require an authorization to construct (ATC) wastewater treatment facilities: This
NOI is also an APPLICATION FOR an ATC for new wastewater treatment facilities. ATC requests must be
submitted at least 90 days prior to construction'.
For exi-- sting mining sites that require an Authorization to Operate (A TO) wastewater treatment facilities:
This NOI is also an APPLICATION FOR an ATO for wastewater treatment facilities in place.
Construction modification or installation of anV new treatment comi2onents at an existing site requires an
ATC. (If the site is already covered by NCG020000, the ATC request may be submitted without an NOI).
Some wastewater treatment facilities used to treat or recycle wastewater at mining sites will require an ATC rp for
to construction per 15A NCAC 2H .0138. Please seethe NCG020000 General Permit for details about which
treatment facilities require an ATC (summarized in Table 1 below). The authorization to construct or continue
to operate will be issued at the same time as the Certificate of Coverage (COC) for the NCG02 permit. Design of
treatment facilities must comply with requirements of 15A NCAC 2H .0138 & .0139.
Construction of wastewater treatment facilities subject to the ATC/ATO requirements in the NCG02 permit
at new orexisting mining sites requires submission of three (3) sets of plans and specifications signed

SILT FENCE DETAIL.
NOT TO SCALE---
1. Silt fence slail be constructed before upslope land disturbance begins.
2. All silt fence shall be placed as close to the contour as possible so
that water vill not concentrate at low points in the fence and so that
small swales or depressions which may carry small concentrated flows to
the silt fence are dissipated along its length.
3. To prevent hater ponded by the silt fence from flowing around the
ends, each tnd shall be constructed upslope so that the ends are
at a higher elevation.
4. Where possble, silt fence shall be placed on the flattest area available.
5. Where possble, vegetation shall be preserved for 5 ft. (or as much as
possible) upslope from the silt fence. If vegetation is removed, it shall
be reestablidned within 7 days from the installation of the silt fence.
6. The height -3f the silt fence sham be a minimum of 16 in. above the nrigino
ground surface.
7. The silt fence shall be placed in a trench out a minimum of 6 in. deep.
The trench shall be cut with o trencher, cable laying machine, or other
suitable device which will ensure an adequately uniform trench depth.
8. The silt feriae shall be placed with the stakes on the downslope side of
the geotextie and so that 8 in. of cloth are below the ground surface.
Excess mots"ial shall lay on the bottom of the 6-in.-deep trench. The
trench shall be backfilled and compacted.
9. Seams betveen section of silt fence shall be overlapped with the end stakes
of each section wrapped together before driving into the ground.
10. Mointenonee---Silt fence shill allow runoff to pass only as diffuse flow through
the geotextie. If runoff overtops the silt fence, flows under or around the ends,
or in any oher way becomes a concentrated flow, one of the following shall be performed,
as appropride:
1) The layott of the silt fence shall be changed,
2) Accumulated sediment shall be removed, or
3) Other prlctiees shall be installed.
Criteria for Sit Fence Materials
1. Fence Posts --The length shall be a minimum of 32 in. long. Wood posts will be 2-by-2in.
hardwood o sound quality. The maximum spacing between posts sholl be 1 0 ft.
