On November 2, 2012, Thomas Ascenzo of the Raleigh Regional Office (RRO) conducted a
compliance inspection (CEI) of Triangle Brick Company's Merry Oak Facility stormwater
pollution program. The assistance of yourself and Mr. Howard Brown -VP of Production and
Engineering was appreciated as it facilitated the inspection process.
The Triangle Brick Company- Merry Oak facility discharges stormwater and mine dewatering
to receiving waters designated as UT to Shaddox Creek in the Cape Fear River Basin.
The following observations were made:
1. The Stormwater Pollution Prevention Plan (SP3) was reviewed and revealed the
Certificate of Coverage was issued on February 3, 2012. The (SP3) is still under
development.
2. Current (SP3) contains the following components:
• The plan contained the required general location map that included outfall
location and drainage areas.
• Plan included Stormwater Facility Inspection reports
• Best Management Practices summary, current certificate of coverage, and spill
prevention response plan were available for review.
• The SP3 is being updated annually and as needed.

3. Qualitative Monitoring and Analytical Monitoring has not commenced at the time of
inspection. Qualitative Monitoring forms is attached to this inspection report.
4. Employee Training has not commenced at the time of inspection but was slated to be
conducted sometime in November 2012.
5. Responsible Party list is posted at the facility. This information needs to be in a page in
the (SP3) manual.
6. Include a page in the (SP3) with information on any spills that occurred in the past three
years. If none are noted, document this information.
7. Spill Prevention and Response Plan was located in a separate notebook.
8. The site contains two outfails. Outfall #1was dry. Outfall #2 was not evaluated.
