On 6/30/2009, the Division of Water Quality received your request to rescind you coverage under Certificate of Coverage
Number NCGO20649. ]n accordance with your request, Certificate of Coverage Number NCG02O649 is rescinded
effective immediately.
Operating a treatment facility, discharging waste%eater or discharging specific types of stormwater to waters of the State
without valid coverage under an NPDF.S permit will subject the responsible party to a civil penalty of up to $25,000 per
day. It is the intention of DWQ that enforcement proceedings will occur for persons that have voluntarily reiinquislted
permit coverage when, in fact, continuing permit coverage Nvas necessary. If, in retrospect, you feel the site still requires
permit coverage, you should notify this office immediately. Furthermore, if in the future you wish to again discharge to
the State's surface waters, you must first apply for and receive a new NPI)ES permit.
Ifthe facility is in the process of being sold, you will be performing a public service if YOU Would inform the new or
prospective owners of their potential need for NPDES permit coverage.

In accordance with your application for a discharge permit received on December 10,
2004, we are forwarding herewith the subject certificate of coverage to discharge under the
subject state -NPDES general permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North
Carolina and the US Environmental Protection Agency dated May 9, 1994 (or as subsequently
amended).
Please take notice that this certificate of coverage is not transferable except after notice to
the Division of Water Quality. The Division of Water Quality may require modification or
revocation and reissuance of the certificate of coverage.
This general permit allows the discharge of stormwater and certain types of wastewater
from your facility. In your Notice of Intent (NOI), you specified that only mine dewatering .
would be discharged from this facility. If point -source discharges of stormwater occur, you will
be required to adhere to the conditions in this permit for stormwater discharges.
In addition, please be aware that mine dewatering at your site has the potential to impact
adjacent wetlands. The specific requirements for mine dewatering at facilities near wetlands can
be found in Section C. Part 1 of your permit (Part III Page 5 of 9).. You must have an approved
Operation and Monitoring Plan before mine dewatering discharges occur at your facility.
Discharging mine dewatering wastewater without Plan approval will be subject to enforcement
action. An Operation and Monitoring Plan for your facility must be submitted to the Washington
Regional Office within 60 days of your receipt of this permit. Please Contact Al Hodge at the

3. 1 agree that bulk storage of petroleum products and other chemicals shall have adequate
protection so as to contain all spills on the site.
4. 1 agree that solid wastes will be disposed of in accordance with N.C. statutes and rules
governing solid waste disposal.
5. 1 agree that maintenance activities for vehicles and heavy equipment will be performed so as to
not result in contamination of the surface or ground waters.

As discussed with DWQ Supervisor Mr. Al Hodge at the Washington Regional Office on
June 3, 2005, the certificate of coverage (COC) sent to you on May 20, 2005 included the
incorrect location for the above site because the wrong map had been submitted with your
application. With Mr. Hodge's approval, the location map has been revised per your fax to us
this morning, and the COC has been revised to more accurately describe the discharge stream
("an unnamed tributary to Knobbs Creek"). The COC has also been post-dated to reflect the
original coverage date. Please discard your original COC and replace it with this corrected copy.

PROCESS WATER RECYCLE SYSTEMS DISCHARGES
NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM
In compliance with the provision of North Carolina General Statute 143-215.1, other lawful
standards and regulations promulgated and adopted by the North Carolina Environmental
Management Commission, and the Federal Water Pollution Control Act, as amended,
Barnhill Contracting Company
is hereby authorized to discharge stormwater and mine dewatering wastewater from a facility
located at
Winslow Pit
0.2 miles off Halstead Blvd, Extended
Pasquotank County
to receiving waters designated as an unnamed tributrary to Knobb's Creek, class C-Sw water, in
the Pasquotank River Basin in accordance with the effluent limitations, monitoring requirements,
and other conditions set forth in Parts 1, 1I, III, IV, V, VI and VII of General Permit No.
NCG020000 as attached.
This certificate of coverage shall become effective May 20, 2005.
This Certificate of Coverage shall remain in effect.for the duration of the General Permit.
