Inspection Summary:
Reason for Visit: Other
NCG020601 expired in 2004, but the permit fees were continued to be paid. The Owner has not operated the St. Delight
Mine in several years, but believed the COC to be for Antioch Mine - 25-76. Owner is finishing up at Antioch and
requested the permit to be rescinded. Allison Ward and Pat McClain state that Robert Jones is a good operator and that
they have not had any difficulties. I have discussed in detail wl in house staff (DWQ & I-Q), Jan Piner, Robert Jones (252)
244-2805, and Ann Henrys about the situation so that they understand the program better and continue compliance with
all sites. My recommendation is to resind the NCG020601 permit as St. Delight Mine (25-67) has been reclaimed for
years and the Antioch Mine (25-76) is about to be released, as well. The owner and his staff understands this situation
and will highten their awarness and impliment all adjustments required to maintain site/permit compliance.

I checked with Mr. Jones on this to clarify which permit site it was for. This is for his Antioch Mine 25-76. The site was
inspected 6/20/12. He is no longer pumping out there and is only using an excavator to remove a small amount of material left
at the haul road. The site has been reclaimed other than where he is actively working. The most recent aerial photo google
earth is an accurate representation of the site. Also, attached are some photos from the inspection.

On 4/25/2012, the Division of Water Quality received your request to rescind your coverage under
Certificate of Coverage Number NCG020601. In accordance with your request, Certificate of Coverage
Number NCG020601 is rescinded effective immediately.
Operating a treatment facility, discharging wastewater or discharging specific types of stormwater to
waters of the State without valid coverage under an NPDES permit will subject the responsible party to a
civil penalty of up to $25,000 per day. It is the intention of DWQ that enforcement proceedings will
occur for persons that have voluntarily relinquished permit coverage when, in fact, continuing permit
coverage was necessary. If, in retrospect, you feel the site still requires permit coverage, you should
notify this office immediately. Furthermore, if in the future you wish to again discharge to the State's
surface waters, you must first apply for and receive a new NPDES permit.
If the facility is in the process of being sold, you will be performing a public service if you would inform
the new or prospective owners of their potential need for NPDES permit coverage.

Inspection Summary:
NCG020601 expired in 2004, but the permit fees were continued to be paid. The Owner has not operated the St. Delight
Mine in several years, but believed the COC to be for Antioch Mine - 25-76. Owner is finishing up at Antioch and
requested the permit to be rescinded. Allison Ward and Pat McClain state that Robert Jones is a good operator and that
they have not had any difficulties. I have discussed in detail w/ in house staff (DWQ & i_Q), Jan Piner, Robert Jones (252)
244-2805, and Ann henrys about the situation so that they understand the program better and continue compliance with
all sites. My recommendation is to resind the NGG020601 permit as St. Delight Mine (25-67) has been reclaimed for
years and the Antioch Mine (25-76) is about to be released, as well. The owner and his staff understands this situation
and will highten their awarness and implement all adjustments required to maintain site/permit compliance.

I checked with Mr. Jones on this to clarify which permit site it was for. This is for his Antioch Mine 25-76. The site was
inspected 6/20/12. He is no longer pumping out there and is only using an excavator to remove a small amount of material left
at the haul road. The site has been reclaimed other than where he is actively working. The most recent aerial photo google
earth is an accurate representation of the site. Also, attached are some photos from the inspection.
