Comment: No samples have been taken. The facility staff stated that no discharge had been observed at all
outfalls, therefore, no samples were taken, However, active discharge was observed at the end
of two pipes below the dam of the pond at the time of inspection, although water level in the pond
was about one foot lower than the top of risers. Accumulation of sediment was observed in
Basins_#1 and #3, resulting very limited freeboard._

(I) DIRECTIONS TO FACILITY: From Raleigh Regional Office take 1-440 East and then take exit 11 and
merge onto US-1 N. Continue on US-1 N for 28 miles to the Vance County line, make the first right after the Tar
River onto Sunrock Drive, pass the concrete plant on the right, and the asphalt plant on the left, continue for 0.2
miles and park in front of the weigh station. The main office is in the weigh station.

(Ill) FACITLITY DESCRIPTION: Carolina Sunrock- Southern Vance Quarry is permitted with permit No.
8368R06, to operate a NSPS Subpart 000 applicable portable mineral processing plant.
Safety note: Wheel chocks for your vehicle, steel toe shoes, safety glasses, a safety vest and hearing protection are
required to observe the facility equipment.
(III) INSPECTION SUMMARY: On July 29, 2015, I (Cheng Zhang) met with Mr. Jay Campbell, Plant
Manager, and Mr. Tony Sample, Compliance Manager, to conduct a multi -media compliance inspection at Carolina
Sunrock's Kittrell rock quarry. Upon my arrival at the site, I sat down with Mr. Campbell and Mr. Sample to review
the specific conditions in the facility's current air permit and to evaluate the plant's record keeping procedures.
Afterwards, Mr. Campbell and Mr. Sample directed me on a tour of the quarry to observe the facility's equipment.
Overall, the facility appeared to be operating well from an air quality standpoint. However, a Notice of Deficiency
is recommended because the facility switched out their primary crusher without first requesting modification of their
air permit. Comments about observations made during the inspection are provided below for each of the emission
sources and specific air quality regulations listed in the facility's current air permit.

(V) SPECIFIC CONDITIONS AND LIMITATIONS: Air Permit No. 08368R06 includes the following specific
conditions:
A.1) Applicable Regulations: The permittee shall comply with 15A NCAC Subchapter 2D .0202, .0501, .0510, .0516,
.0521, .0524, .0535, .0540, .1111 and (40 CFR 63, Subpart 000 and ZZZZ).
IN COMPLIANCE - The facility appears to be in compliance with all applicable state and federal regulations.
A.2) 2D .0202 Permit Renewal and Emission Inventory Requirement — The Permittee shall submit their permit
renewal request and emission inventoryfor 2014 at least 90 day s prior to the expiration of the current permit.
IN COMPLIANCE — This source has an Air Quality Permit, No. 0836SR06, that expires on December 31, 2015.
Mr. Campbell was reminded that an emissions inventory would be required as part of the next air permit renewal.
A.3) TSP and PM10 Ambient Air Quality Standards —As required by 15 A NCAC 2D .0403 and 2D .0409,
production shall be limited to 14, 400 tons per day and 3,600, 000 tons per year and shall record the daily and
annual totals. Records shall be maintained for at least 5 years.
IN COMPLIANCE — The daily records show a production rate average of 1,500 tons per day and an average annual
production of less than 540, 000 tons.

A.4) Equipment Reporting— To demonstrate compliance with 15A NCAC 2D .0501 (c), the Permittee shall maintain
on -site an equipment list and plant diagram of all permitted equipment.
IN COMPLIANCE — Both an equipment list and a flow diagram are digitally Stored in an electronic format. Both
the list and the diagram describe the width of the conveyor belt, screen dimensions, rated capacity of the crushers,
identification numbers, manufacture dates, and revision dates.
A.5) Particulate Control Requirement —As required by 15A NCAC 2D .0510, the Permittee shall not cause, allow,
or permit any material to be produced, handled, transported, or stockpiled without taking measures to reduce to a
minimum any particulate matter from becoming airborne. The Permittee shall control process -generated emissions
from crushers with wet suppression and from conveyors, screens, and transfer points such that the applicable
opacity standards are not exceeded.
IN COMPLIANCE — Recent wet ambient conditions have helped to keep fugitive dust under control, but the' facility
operates two water trucks on a routine basis. Sunrock Drive has been paved to further reduce the need for watering.
Spray nozzles are inspected and maintained on a weekly basis and were performing adequately at the time this
inspection.
A. 6) The primary crusher is a jaw crusher rated at 630 tons per hour at a 6-inch crusher setting per the
manufacturer's specifications as submitted. Any changes from this crusher definition, other than like -for -like, will
require a permit application to be submitted. The purpose of this stipulation is to characterize the primary crusher
and not limit production.
DEFICIENCY NOTED — The primary crusher was pulled out for repair in August 2014. A diesel powered portable
crusher rated at 340 tons per hour was put in the pit to serve as primary crusher. The portable crusher was in use at
the time of inspection. Facility personnel were under the impression that because the new crusher was portable, it
was exempt from air quality permit requirements. While this may be true for certain portable quarry equipment, the
replacement of the primary crusher always requires a permit modification unless the new crusher is exactly like the
former crusher in terms of size, type, and function and does not increase emissions. In this case, the new primary
crusher is smaller than the original one and the resultant emissions are expected to be much less. Therefore, it is
recommended that a Notice of Deficiency be issued to the facility requesting an application for permit modification
be submitted to allow operation of the new portable crusher at the Kittrell quarry.
A. 7) Sulfur Dioxide Control Requirement — As required by 15A NCAC 2D .0516, combustion sources shall not
exceed 2.3 pounds per million Btu heat input.
IN COMPLIANCE — Most of the diesel powered emissions sources (pumps) on this permit have been replaced with
electric motors. Compliance with the 2D .0516 emission limit is expected for all remaining combustion sources.
A.8) 2D .0521 Control of Visible Emissions: Visible emissions from the facility (all permitted items), established
after July 1, 1971, shall not be more than 20 percent opacity.
IN COMPLIANCE — V isible emissions were less than 10% from the crushers, screens and conveyors. 1 was able to
view all sources in operation.
A.9) 2D .0521 Control of Visible Emissions: Visible emissions from the facility (all permitted items), established as
of July 1, 1971, shall not be more than 40 percent opacity,
IN COMPLIANCE — Visible emissions were less than 10% from the crushers, screens and conveyors. I was able
to view all sources in operation.
A.10) 15A NCAC 2D .0524 New Source Performance Standards: Pursuant to 15A NCAC 1D .0524, the Permittee
shall comply with all applicable provisions of 40 CFR 60 Subpart 000 for the plant crushers, screens, and
conveyors.

IN COMPLIANCE — The visible emissions were less than 10% from the crushers, screens and conveyors. Daily
(workday) inspections are performed on wet suppression spray nozzle and maintained in a logbook in the main
office.
A.11) Notification Requirement— As required by 15A NCAC .0535, the Permittee for a source of excess emissions
that last more than four hours that results from a majunction shall notify the Division of Air Quality.
IN COMPLIANCE — Mr. Campbell stated that no excess emissions have occurred.
A.12) 2D .0540 Fugitive Dust Control Requirements: Facility shall not allow fugitive dust emissions to cause or
contribute to substantive complaints.
IN COMPLIANCE —No fugitive dust was noted at the facility.
A.13) 15A NCAC 2D .1111 "Maximum Achievable Control Technology"for the following equipment:

(VI) ENFORCEMENT HISTORY: 5/4/2004 — NOV — Permit stipulation
(VII) PERMIT EXEMPT EMISSION SOURCES: No exempt emission sources were noted during the inspection
that would require listing on an insignificant / exempt activities attachment to the facility's air permit.
(VIII) 112(r) APPLICABILITY: Carolina Sunrock has no emissions sources that are subject to the Risk
Management Plan - 112(r) program.
(IX) STACK TESTING HISTORY: The facility is not required by the air permit to do stack testing.
(X) EMISSIONS INVENTORY: The facility has submitted emissions inventory reports for 2005 and 2009, An
emissions inventory will be required for 2014 with the air permit renewal before the 2015 expiration date.
(XI) CONCLUSIONS/RECOMMENDATIONS: Overall, the Carolina Sunrock's Kittrell quarry appeared to be
operating in compliance with their air quality requirements at the time of this inspection. However, it was
determined that in accordance with Specific Condition A.6. of their air permit, the facility should have applied for
and received a modified air permit prior to switching out their primary crusher. A Notice of Deficiency is
recommended to address this issue. It is also recommended the facility be re -inspected in two (2) years.

Dear Mr. Pfohl:
Staff from the Department of Environmental and Natural Resources conducted a stormwater inspection
on July 29, 2015, as part of a multimedia inspection of your facility. The assistance provided by Mr. Jay
Campbell, Plant Manager, and Mr. Tony Sample, Compliance Manager, during the inspection was
greatly appreciated. The inspection report is attached. Findings during the inspection were as follows:
Certificate of Coverage (COC) No. NCG020439 under NPDES General Permit NCG020000
was issued to Carolina Sunrock LLC — Kittrell Quarry to discharge stormwater runoff and
process wastewater to an unnamed tributary to Tar River, a WS-IV, NSW stream, in the Tar -
Pamlico River Basin.
2. The facility has developed a stormwater pollution prevention plan (SPPP). A review of the
plan indicated that it included all components required by the General Permit.
3. Semi-annual qualitative monitoring has been conducted as required by the permit at the all
five stormwater discharge outfalls (SDOs). Stormwater Best Management Practices (BMPs)
have been inspected weekly. Sediment accumulation was noted in the settling basins at
SDO 1, and SDO 2, which significantly reduces the capacities of both basins. Sediment
in these basins needs to he removed periodically to ensure that they function as
designed.
4. Carolina Sunrock — Kittrell Quarry was found to have violated Part III,:Section B, of
the General Permit for failure to perform analytical monitoring for stormwater
discharge from one SDO (Pond 1). The facility has been reporting no flows at all SDOs.

However;,during the inspection, discharge from the pond was observed from the two
corrugated pipes below the dam of the pond; although the water level in the pond was
bout one foot lower than the top the risers. Instead of inspecting the risers, semi-
annual qualitative monitoring at this outfall shall be performed at the ends of discharge
pipes.
All five SDOs were observed during the inspection. The outfalls were found to be maintained
and accessible.
