The assistance of Howard Whitley was gratefully appreciated. Please find the attached Basin Wide Information Management System (BIMS) inspection checklist summarizing each inspection. Below is a list of recommendations and findings developed from the inspection: 1. The subject permit became effective January 19, 2005 and will expire December 31, 2009. A request for permit renewal with fees must be made 180 days prior to expiration in order to discharge after the expiration date. Please submit all information required on the Notice of Intent (NOI) to the address listed at the bottom of the NOI. The website for the NOI can be found at http:1/h2o.enr.state.nc.us.Isuldocuments/SWU217NCG02N01 Rev2008.pdf 2. The facility has two stormwater retention ponds. The upper pond is upslope of the bottom pond and catches most of the sediment runoff. Clarified water in the upper pond is conveyed to the bottom pond by underground piping. There is no discharge point for the bottom pond. There was no indication of sediment deposition around the perimeter of the bottom pond. Please explain in a written response whether: 1) the bottom pond meets the definition of 10-year design storm without discharging or 2) if engineering reports for the upper and lower ponds are unknown. 3. A private consultant, Patrick Shilling, handles monitoring. The last analytical data results were on October 15, 2006. All qualitative monitoring data was up to date and available for review.
