site - sample point for 001 should be moved up to the pond discharge on the property line. If this basin can be made large
enough so that there is no discharge - then the outfall can be removed. The water flowing into this basin is sheet flow from
the parking lot.

Inspection Summary:
Susan Wilson and Tim Fox performed a site inspection on November 29, 2012. Don Mason (APAC-Harrison) assisted
with the inspection. The quarry is beginning its slower business time and doing various maintenance activities.
Much of the flow from the haul roads is now diverted tdthe old pit - with no discharge, which is beneficial (and 001 has
recently been eliminated and diverted to the pit).
Outfall 004 is from the basin near the entrance road and should be maintained (ditch lines should be maintained in this
area and diverted to this basin - and the basin should be cleaned out).
DWQ will allow APAC Harrison to eliminate monitoring from Outfall 002 - near the VMA. APAC should ensure that the
contractor has appropriate spill kits and is aware of who to contact should a spill occur in that area. APAC should also
ensure that other activities take place within the covered building. Although monitoring can be eliminated - routine
inspection and maintenance on the basin should still be conducted and outlined in the SWPPP.
Road washing is a source of turbidity to Mack Town Branch and the Tuckaseegee River (and was observed the day of
inspection). DWQ is aware of the balance between dust control and keeping the road clean. The recommendation at this
point - to try and alleviate the turbidity impact - is to outline a schedule for routine road maintenance (post schedule -
make sure employees know when to conduct). The turbidity impact is not a chronic one - just a fairly acute one for a short
duration.
Periods of "No flow" have been reported during sampling periods. APAC may wish to re-evaluate the sampling protocol to
ensure that stormwater samples can be taken during discharge events; this may require that personnel at the plant site
grab samples during a discharge event (instead of Don having to come from a different location to grab a sample).

Comment: The SWPPP is well put together and includes all major elements. A better
site plan with drainage areas (and what outfall they go to) would be beneficial.
Good housekeeping practices (especially with regard to the roadways) should likely be
more specifically outlined (and posted). Annual training should be documented and
included in the plan.

Comment: All outfalls were observed. Outfall 001 - has just been eliminated (by
enlarging/rerouting flow). Outfall 003 was eliminated (used to be pit dewatering).
Reason for Visit: Routine
Outfall 002 - this is an outlet from a trap near the vehicle maintenance area (VMA).
However - all changing of oil is contracted out (and they remove all fluids). Any other
work is conducted inside a covered building. DWQ will allow this outfall to be eliminated
(but maintained - cleaned out as needed).
Outfall 004 - from basin at the main entrance road. The ditchline and basin for this
outfall need maintenance - with flow directed to this basin.

In compliance with the provision of North Carolina General Statute 143-215.1, other lawful standards and
regulations promulgated and adopted by the North Carolina Environmental Management Commission, and the
Federal Water Pollution Control Act, as amended,
Harrison Construction Company
is hereby authorized to operate a treatment system for mine dewatering and/or recycle systems process wastewater
overflow with the discharges of treated wastewater and stormwater from a facility located at
Dillsboro Quarry
Macktown Road, intersection of SR 1377 and US Hwy 441
Dillsboro
Jackson County
to receiving waters designated as the Tuckaseegee River in the Little Tennessee River Basin
in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, I1, I1I
and IV of General Permit No. NCG020000 as attached.
This Certificate of Coverage shall become effective May 6, 1994.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

In response to your renewal application for continued coverage under general permit NCG020000 the Division of Water
Quality (DWQ) is forwarding herewith the reissued stormwater general permit. Please review the new permit to familiarize
yourself with the changes in the reissued permit. The general permit authorizes discharges of stormwater and some types of

with mine dewatering wastewater and process wastewater from a facility located at

Jackson County
to receiving waters designated as Tuckasegee River (East Fork Lake, Bear Creek Lake, Cedar Cliff Lake),
a class WS-III,B;Tr stream in the Little Tennessee River Basin in accordance with the effluent limitations,
monitoring requirements, and other conditions set forth in Parts I, 11, III, IV, V, and VI of General Permit No.
NCG020000 as attached.
This certificate of coverage shall become effective February 7, 2005.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

In accordance with your application for discharge permit received on March 28, 1994, we are
forwarding herewith the subject certificate of coverage to discharge under the subject state - NPDES general
permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated
December 6, 1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, this certificate of coverage shall be final and binding.
Please take notice that this certificate of coverage is not transferable except after notice to the Division
of Environmental Management. The Division of Environmental Management may require modification or
revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Environmental Management or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local governmental permit that may be required.

to receiving waters designated as the Tuckaseegee River in the Little Tennessee River Basin
in accordance with the effluent limitations, monitoring requirements, and other conditions set forth in Parts I, II, III
and IV of General Permit No. NCG020000 as attached.
This Certificate of Coverage shall become effective May 6, 1994.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.
