(1) DIRECTIONS TO FACILITY: From RRO take 1440 E Inner Beltline to 140 E to US-70 E to Princeton. Turn left onto Edwards Rd. The facility is on the left about V2mile past the first intersection. (II) FACILITY DESCRIPTION., Hanson Aggregates operates a stone quarry and crushing facility at this location since 1999. It currently operates 40 hrs per week either four 10-hr days (Mon -Thu) or five 8-hr days (Mon -Fri), depending on demand. Currently, production rate is about 850 tons crushed stone per hour. Safety note: Standard PPE (hard hat, steel -toed shoes, safety glasses and hearing protection) plus a reflective vest are to be worn at this facility. (111) INSPECTION SUMMARY: On November 6, 2015, 1 (Gary Perlmutter) with Thaddeus Valentine (DEMIR-RRO) visited Hansen Aggregates — Princeton quarry to conduct a multimedia inspection addressing air, stormwater and hazardous waste media. This report addresses the air medium component of the inspection. At the facility we met with Mr. Jack Harvey, Environmental Manager and Technical Contact and Mr. Donald Harvey, Plant Manager and Facility Contact., to conduct a compliance inspection. We first reviewed the. air permit stipulations and all required records. All seemed in order We then toured the facility. The plant equipment located above the pit was shut down.as the secondary crusher was shut down for maintenance, but the equipment associated with the primary crusher down iff the pit was operating. I observed water spray at the crusher and the roads appeared wet throughout from recent rains. No VE was observed. Overall, the facility appeared well run and maintained from an air quality standpoint. Further details follow. Only equipment in the pit were In operation; equipment above the pit were down for maintenance during my Inspection. (V) SPECIFIC CONDITIONS AND LIMITATIONS A. 1. The Permittee shall comply with 15A NCAC, Subchapter 2D .0202, 2D .0501, 2D .0510, 2D .0521, 2D .0524 (40 CFR 60, Subpart 000), 2D .0535 and 2D .0540. Appeared to. 6i in compliance — see below. A.2. PERMIT RENEWAL AND EMISSION INVENTORY REQUIREMENT - The Permittee, at least 90 days prior to the expiration date of this permit, shall request permit renewal and submit an air pollution emission inventory report for CY2014. Appeared to be in compliance — Mr. Garvey told us that he had already received notice from DAQ regarding air permit renewal. A.3. EOUIPMENT REPORTING - To demonstrate compliance with 15A NCAC 2D .050 1 (c), the Permittee shall maintain on -site an equipment list and a plant (or flow) diagram of all equipment covered under this permit. Appeared to be in compliance — I reviewed the current equipment list and flow diagram onsite. They appeared complete and up-to-date with the recently replaced jaw crusher and are both dated January 21, 2014. A.4. PARTICULATE CONTROL REQUIREMENT - As required by 15A NCAC 2D .0510 "Particulates from Sand, Gravel, or Crushed Stone Operations," the following requirements apply: a. The Permittee shall prevent exceeding the ambient air quality standards beyond the property line for particulate . matter, both PM10 and total suspended particulates. 1.00035 inspection Report Page 13 b. Fugitive dust emissions from sand, gravel, or crushed stone operations shall be controlled by 15A NCAC 2D .0540 "Particulates from Fugitive Dust Emission Sources." c, The Permittee of any sand, gravel, or crushed stone operation shall control process -generated emissions: i. From crushers with wet suppression; and ii. From conveyors, screens, and transfer points Appeared to be in compliance — during the facility tour we observed spray -bars in operation at the primary crusher with wet material being transported along conveyors and transfer points. A.5. This primary crusher is a 50 x 62 jaw crusher rated at 850 tons per hour at a discharge opening of 7" per the manufacturer's specifications as submitted. Any change from this crusher definition, other than like -for -like, will require a permit application to be submitted. The purpose of this stipulation is to characterize the primary crusher and not limit production. Appeared to be in compliance — the primary crusher Is as described. A.6. VISIBLE EMISSIONS CONTROL REQUIREMENT - As required by 15A NCAC 21) .0521, VE from the emission sources, manufactured after July 1, 1971, shall not be > 20 % opacity when averaged over a six -minute period. Appeared to be in compliance — no VE was observed during the inspection. A.7. 15A NCAC 21) .0524 "NEW SOURCE PERFORMANCE STANDARDS" - For the crushing operations (ID No. ES - Crusher), the Permittee shall comply with NSPS as promulgated in 44 CFR 60, Subpart 000, including Subpart A "General Provisions." [See permit forlengthy details]. Appeared to be in compliance-. Ali equipment except for the new primary jaw crusher, manufactured 2013 (and installed January 2014), and two cone crushers, both built in the late 1950°s,.were manufactured between 19.83 and 2008 as recorded in fo the equipment list. Therere, slightly different limits apply to the plant equipment (see permit). Also in the equipment list were dates of initial performance testing..No. VE was observed as the plant was shut down during the Inspection. Spray -bars were observed in operation at the primary crusher, and I reviewed daily dust log which contained spray -bar maintenance triggered by dust observations. Daily VE recordings were reviewed and found to. be complete and within limits. The jaw crusher was determined to be'not like -for -like in review of the received permit application, and an NOV was issued on March 12, 2014. However, it was later determined to be like -for -like and the NOV was rescinded on June 13, 2014. A.8. NOTIFICATION REQUIREMENT - As required by 15A NCAC 2D .0535, the Permittee of a source of excess emissions that last > 4 hr and results from a malfunction, a breakdown of process or control equipment or any other abnormal conditions shall notify DAQ by the next business day. Appeared to be In compliance — no such emissions have occurred per Mr. Garvey and Mr. Harvey. I reminded them of this condition. A9. FUGITIVE DUST CONTROL REQUIREMENT - As required by 15A NCAC 2D .0540, the Permittee shall not cause or allow fugitive dust emissions to cause or contribute to substantive complaints or excess visible emissions beyond the property boundary. Appeared to be in compliance — no dust was observed leaving the facility. I reviewed the daily water truck log, which appeared complete and up-to-date. No complaints are on file against the facility. {VI) GENERAL PERMIT CONDITIONS: B.14. PERMIT RETENTION REQUIREMENT - The Permittee shall retain onsite and make available to DAQ personnel a current copy of the air permit. Appeared to be in compliance — Mr. Harvey showed me a copy of Air Permit 020651321. B.15. CLEAN AIR ACT SECTION 112(r) REQUIREMENTS - Pursuant to 40 CFR Part 68 "Accidental Release Prevention Requirements: Risk Management Programs Under the Clean Air Act, Section 112(r)," if the Permittee is required to develop and register a risk management plan pursuant to Section 112(r) of the Federal Clean Air Act, then the Permittee is required to register this plan in accordance with 40 CFR Part 68. (VIM EMISSION INVENTORY REVIEW: Between El years 2005 and 2010 emissions of TSP and PM1o, the only pollutants listed, had decreased. (IX) SOURCE TEST REVIEW: Per I -Beam records, one performance test was conducted, on November 11, 1998, with no results in DAQ files. Review of the facility equipment list shows that all required NSPS testing of equipment was performed. The current permit has a stipulation for NSPS Subpart 000 that includes a source testing requirement. The equipment list has a due date of July 12, 2014 for the new jaw crusher, installed January 2014. A test protocol was received by DAQ on June 5, 2015 with a proposed test date of June 15, 2014. However, this was'never performed as a result of the crusher being determined as a like -for -like replacement. The equipment 11st'dates the test for this crusher as before July 21, 2014. (X) COMPLIANCE HISTORY: A review of I -Beam has revealed the following compliance actions taken against the facility: • March 12, 2014 — an NOV was issued for replacing a jaw crusher with a unit deemed not like -for -like without first applying for an air permit modification. The crusher was subsequently determined to be like -far -like, and the NOV was rescinded. How6vcr, the permit was still modified to address the dimensions of the new crusher. (XI) CONCLUSIONS/RECOMMENDATIONS: Overall, this facility appears to be in compliance with their air quality requirements. The following observations were noted during the Division of Water Quality inspection: SITE DESCRIPTION Stone and gravel products are currently being mined at this facility. There is one stormwater discharge outfall (SDO). The process wastewater produced from the quarry flows to a series of four ponds and then is pumped from the fourth pond, known as the "freshwater pond", back to the stone washing operation. There is no discharge of process wastewater from the site, as all wastewater is recycled. This type of operation is known as a closed loop recycle system. Stormwater from the quarry pit is pumped to either a settling pond, which then flows through a canal to SDO 001; or to the freshwater pond, which would be re -used in the stone washing process. Vehicle maintenance is conducted at this facility and therefore analytical monitoring is required for this function. The Stormwater General Permit was onsite during the inspection. The permit expires on December 31, 2009. Please note that an application for renewal is required 6 months prior to expiration of the permit. 2. The Best Management Practices (BMP) plan was available during the CEI. Documented records of BMP inspections were evaluated. All records that are currently required by the permit were available to us during the inspection and were in compliance. Though not required of this permit, a record of employee stormwater training and a list of spills was within the BMP plan manual. The possibility that permit requirements could change in the next permit cycle was pointed out to you and Mr. Bizzell during the site visit. 3. Qualitative and analytical monitoring was conducted and was documented. The records were evaluated during the CEI and found to be compliant. Analytical monitoring for the vehicle maintenance areas is being conducted, however, sampling must include the required grease and oils in 2009 as required for facilities conducting vehicle maintenance. 4. The SDO (001) was visited during the CEI. There was considerable flow present, which showed no sign of an oily sheen and was not considered to be sediment laden. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the Memorandum of Agreement between North Carolina and the US Environmental Protection agency dated December b, 1983. If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to request an individual permit by submitting an individual permit application. Unless such demand is made, this certificate of coverage shall be final and binding.
