On November 7, 2013, [,along with Chris Pul linger (previously of the Raleigh Regional Office),
carried out a stormwater compliance evaluation inspection (CEI) for the above permit. In the previous
letter regarding this inspection, sent December 19, 2013, Mr. Pullinger confirmed that you were
authorized to return to "Pier One sampling for pH. However, the letter should have also specified that
you were authorized to return to Tier One for settleable solids as well. Thank you for bringing this
oversight to our attention. Please consider this letter as confirmation that you may return to Tier One
sampling for both settleable colds and pH.
It is apparent that the Stormwater Program is being carried out for the Martin Marietta Materials Inc. —

On November 7, 2013, I, Chris Pullinger, of the Raleigh Regional Office, along with Ashley Rodgers,
PE, also of the Raleigh Regional Office, carried out a stormwater compliance evaluation inspection
(CEI) for the above permit. The availability of Mr. Nuwan Wijesuriya to assist me, was appreciated as
it facilitated the inspection. A synopsis of the inspection is discussed below:
1. The Stormwater Pollution Prevention Plan (SPPP) was available during the inspection. The
SPPP was reviewed and updated annually during this permit cycle. All components of the plan
were evaluated during the CEI, and the plan was found to be complete and being put into active
use.
2. The Stormwater Discharge Outfalls (SDO) were inspected during the inspection. The SDO
appear to be adequately capturing all of the stormwater coming from the production area.
3. Qualitative and quantitative monitoring results were reviewed during the CEI. All monitoring
results were available and LIP to date. At the time of the site visit, the facility was sampling
Outfall 92 (SDO #2) monthly under the Tier Two self -monitoring requirements due to a single
exceedance of the pl-I benchmark, and had been doing the monthly sampling required by Tier
Two for the previous 8-plus months (since February). At your request, 1 evaluated whether
your facility could return to the Tier One sampling. Based on discussions about the probable
reasons for the exceedance (pH meter failure), and noting that you had undertaken duplicate
sampling to resolve this question, I gave you verbal authorization during the meeting that you
could return to Tier One sampling, and this letter is also meant to confirm that you are
authorized to return to Tier One.

of your permit is required to be received a minimum of six months prior to its expiration.

Comment: Plan is in place and being implemented. Everything looks good, all data
are being collected, and on during the site visit, we discussed the plant having to go to
Tier 2 (for pH benchmark exceedance), and that they'd done monthly sampling for the
last 5-6 months with no further exceedances (and discussed how pH meters can be
fickle, and by Chan--e that the exceedance event was the only sampling event where
they hadn't double -sampled with both a meter and lab analysis), so permission was
granted (by Chris P.) to go back to Tier 1 sampling, and was noted in the compliance
letter issued on 11/14/2013.

On September 14, 2009, I, Vicki Webb, of the Raleigh Regional Office conducted a compliance
evaluation inspection of the subject facility. You represented the facility during the inspection. All of
yourhelpwas appreciated as it eased the inspection process.
The following observations were made:
1. The Martin Marietta Materials is permitted to discharge stormwater into the receiving waters
designated as Mullato Branch, classified C waters in the Cape Fear River Basin.
2, All components of the Stormwater Pollution Prevention Plan (SP3) were present including:
detailed site map with labeled storm outfall, a list of potential contaminants, a spill and response
plan, a narrative describing BMPs, and a contact list of whom to contact in an emergency.
3. Employee training records were on -site. This facility maintains good housekeeping practices.
4. Qualitative and quantitative inspection records were available for review and appeared complete.
5. The facility has most of the stormwater directed to their close loop system to utilize this water
coming off site. The facility looks great.
The overall condition of Martin Marietta Materials is compliant with Division standards. If you have

Mr. Chad Coburn of the Division of Water Quality (DWQ), Raleigh Regional Office, conducted a compliance evaluation inspection of
the Lemon Springs Quarry for coverage under the NPDES permit NCG020094 on October 17, 2006, Mr. Nuwan Wijesuriya
represented Martin Marietta Materials Inc. during the inspection and his assistance was greatly appreciated. Findings during the
inspection are as follows:
1) The Stormwater Best Management Practices (BMP's) Plan for the site was reviewed and found to be in excellent order.
A general location map and site map identifying all SDO's, a copy of the general permit, records of spills, emergency
contact information, and records of in house site inspections, were on file.
2) Qualitative monitoring is being performed according to the permit, and records are kept on site in accordance to the
permit requirements. Analytical monitoring is being performed according to the permit, and laboratory records kept at
the company's central office in Raleigh were made available to the inspectors upon request. Monitoring records are kept
on site as well.
3) The site plans for an 87.1 acre expansion to the existing 386.1 acre facility were submitted to DWQ through the Division
of Land Resources. The existing NPDES permit was reviewed along with the proposed plans for expansion and an issue
was raised regarding the existence of a stream feature shown on the USDA soil survey map of the area. This feature is
not evident on the corresponding USGS topographic map of the area. Since the site lies within the Cape Fear River basin
there are no existing riparian buffer rules that governing this stream feature. However, 1 strongly recommend that Martin
Marietta Materials Inc. discuss this issue with the U.S. Army Corps of Engineers and/or hire an environmental consultant
to evaluate the need for possible wetland delineation or the need of a 401 certification/404 permit before expansion in
areas that could impact the stream feature or wetlands on or adjacent to the site.

This writer, along with Mr. Kenneth Schuster (Surface Water Quality Regional Supervisor), of the NC Division of
Water Quality, conducted a Stormwater Permit Compliance Evaluation Inspection of Martin Marietta — Lemmon Springs,
on ,tune 17, 2005. Mr. Steve Whitt and Mr. Chance Allen represented Martin Marietta during the inspection. Findings are
as follows:
1) The quarry sump collects process and storm water within the drainage area of the quarry proper. Water is pumped
from the quarry sump to the primary sump, also located within the quarry, for further settling. Water in the primary sump
is pumped as required to either a closed loop recycling system or to the effluent outfall. Quarterly effluent sampling for the
first quarter of 2005 had been performed. Ali results met permit discharge limitations.
2) A new process area has been constructed at the Lemmon Springs facility and is separated from the quarry by the
former process area. Three riser basins, strategically located around the new process area, collect Stormwater runoff.
The riser basins were formerly used as settling basins during construction of the new process area. Annual stormwater
sampling for the current permit, collected at the riser basin effluents, has not yet been performed. (The current permit
became effective on 1/19/05.) Within the process area is the closed loop recycling system where water is distributed to
sprinklers (for dust suppression) and wash screens (for product wash). Primary sump water (from the quarry) and wash
screen slurry is returned to a series of settling cells where solids are settled out and the water is reused.
3) During this Inspection, this writer and Mr. Schuster noted a breach in a perimeter berm along Willett Rd. Stormwater
had washed sediment through the breach to a catch basin below. The catch basin was completely full of sediment
rendering it useless for sediment control. The berm must be repaired and sediment in this basin removed immediately.
BMP inspections as required by the stormwater permit should have caught this breach and repairs made immediately.
(Mr. Whitt reported that the berm was repaired and sediment removed from the catch basin the week of June 20, 2005.)
4) It is recommended that stormwater permit information be incorporated into the training already presented annually to
Martin Marietta — Lemmon Springs personnel under the Spill Prevention Control and Countermeasure Plan.
5) It was noted that qualitative monitoring of stormwater outfalls was being performed weekly while the permit requires
semi-annual monitoring. It is recommended that the State of NC Qualitative Monitoring Report form be utilized as the
basis for Martin Marietta's qualitative monitoring. (A copy was emailed to Mr. Allen subsequent to this Inspection.) One
form should be completed for each stormwater discharge outfall for each monitoring event.

In response to your renewal application for continued coverage under general permit NCG020000 the Division of Water
Quality (DWQ) is forwarding herewith the reissued stormwater general permit. Please review the new permit to familiarize
yourself with the changes in the reissued permit. The general permit authorizes discharges of stormwater and some types of
wastewater. You must meet the provisions of the permit for the types of discharges present at your facility. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983,

federal, stale, or local law, rule, standard, ordinance, order, judgment, or decree.

Water Quality (DWQ) is forwarding herewith the reissued stormwater general permit. This permit is reissued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:

* Five copies of the Analytical Monitoring form and five copies of the Qualitative Monitoring form
* A copy of a Technical Bulletin on this permit which outlines permit components and
addresses frequently asked questions
* A Certificate of Coverage for your facility
* DWQ fee schedule
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require
modification or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal
requirements to obtain other permits which may be required by DENR or relieve the permittee from responsibility
for compliance with any other applicable federal, state, or local law rule, standard, ordinance, order, judgment, or
decree.
Please note that in 1998 Senate Bill 1366 established changes to the permit fee structure for DWQ permits effective
January 1, 1999. This change requires that you pay an annual fee to assure continued coverage under this permit.
You will be invoiced for this fee beginning next year. A copy of the current fee schedule is included with this letter.

NPDES general permit. Issuance of this certificate of coverage supercedes the individual NPDES

Quality (DWQ) is forwarding herewith the reissued stormwater general permit. Please review the new permit to familiarize
yourself with the changes in the reissued permit. The general permit authorizes discharges of stormwater and some types of
wastewater. You must meet the provisions of the permit for the types of discharges present at your facility. This permit is
reissued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between the state of North Carolina and the U.S. Environmental Protection Agency, dated December 6, 1983.
The following information is included with your permit package:
A new Certificate of Coverage
• A copy of General Stormwater Permit NCG020000
• A copy of a Technical Bulletin for the general permit
• Five copies of Discharge Monitoring Report (DMR) Forms - wastewater and stormwater
Five copies of Qualitative Monitoring Report Form
Your coverage under this general permit is not transferable except after notice to DWQ. The Division may require modification
or revocation and reissuance of the Certificate of Coverage. This permit does not affect the legal requirements to obtain other
permits which may be required by DENR or relieve the permittee from responsibility for compliance with any other applicable
federal, state, or local law, rule, standard, ordinance, order, judgment, or decree.

Please note that in 1998 Senate Bill 1366 established changes to the permit fee structure for DWQ permits effective

If any parts, measurement frequencies or sampling requirements contained in this permit are
