On March 5, 2015, samples were collected from the SDO-2 outfall, and TSS concentrations were 185 mg/L, and the upstream/downstream results were 18.3 NTU/57.8 NTU. Our investigation following receipt of the lab results shows the basin needs cleaning, and the pre -basin measures need repair. Plans are to significantly increase the size of this basin and install a riser with skimmer per our mine permit modification to expand northward. However, until construction of the enlarged basin starts, immediate actions are needed. Hanson has routed all stormwater from the existing basin to drain into the Pit. There are no longer any discharges from SDO-2 because the water now drains into the Pit where it will receive adequate time for sedimentation. Discharges from the Pit are monitored quarterly with the more stringent limits assigned to wastewater. The SDO-2 sediment basin will be cleaned out and the inlet structures refurbished and maintained to minimize sediment entering the Pit. Attached is the Benchmark Exceedance Form with monitoring results. The Crabtree Quarry is now operating under Tier II as required by the permit, including monthly monitoring. The reports will read "No Discharge" as long as the stormwater is diverted to the Pit. The measured concentration was 0.20mg/L with a Benchmark of 0.1 mg/L. Hanson has worked hard to minimize TSS and Turbidity concentrations at SDO-2, and as a result, had no Benchmark exceedances in 2013 for TSS and Turbidity. Our investigation following receipt of the 0.20mg/L settleable solids report showed no apparent reason for the exceedance. There have been no further exceedances since that day, and we will continue our focus on TSS and Turbidity. Attached is the Benchmark Exceedance Form with monitoring results and the exceedance highlighted in yellow. Because we experienced 3 consecutive months with no Benchmark exceedances, Hanson is no longer subject to the Tier 2 monthly sampling initiated in 2012. Sampling will occur on a semi-annual basis. During the stormwater management inspection, a plan was discussed to route the stormwater into the pit; however, quarry wall integrity would possibly be reduced, and gullies would form along side the quarry roads. Inspection results find that sediment basin inlets need maintaining, and installation of a pre -sediment basin may help keep the colloidal solids from reaching the discharge end of the sediment basin. Also, by installing a smaller pre -basin, cleaning it out can be accomplished using our own frontend loader. To further prevent suspended solids from reaching the discharge outlet, baffles will be installed midway in each basin. Hanson personnel have been made aware of our plans to more frequently clean and maintain the sediment basins, basin inlets and outlet, and to apply approved flocculants as needed, to keep colloidal solids from reaching the discharge outlet. Attached are the stormwater management inspection results, the Benchmark Exceedance Form with monitoring results, and the associated qualitative monitoring reports. Sample results from Outfall No. SDO-2 were received on 10/12/12, and the Area Environmental Manager notified the NC Operations manager of the actions to take per Tier Two Steps, including a stonnwater management inspection, as stated in the new General Permit under Ticr Two Steps. The stonnwater management inspection was conducted on 10/17/12 with the Area Environmental Manager and NC Operations Manager. 2. Identify and evaluate possible causes of the benchmark value exceedance. The sediment basin upstream of Outfall No. SDO-2 needed cleaning out. Furthennore, a breach was found in the diversion bean that allowed stormwater from the screenings stockpile to enter the basin at its mid point rather than at the upstream end. 3. Identify potential and select the specific: source controls, operational controls, or physical improvements to reduce concentrations of the parameters of concern, or to bring concentrations within the benchmark range. To clean out the sediment from the basin, a long reach track hoc was scheduled to be delivered to the site on 10/18/12. "The basin needed cleaning out, and it was decided to install a pre -basin that can be cleaned out with a frontend loader. It was also decided to install a baffle midway in the main basin and the pre -basin. This will further prevent solids from reaching the discharge end of the basin. Because the actual cleaning out the basin will cause solids to become suspended, an approved coagulant may be introduced to knock them back down. 4. Implement the selected actions within two montlis of the inspection. Corrective actions will be iinpleinentcd within one month of the inspection. The majority of the corrective actions will be implemented the next day. The baffles will be installed within one month of the 10/17/12 inspection. 5. Record each instance of a Ticr One response in the Stormwater Pollution Prevention Plan. Include the date and value of the benchmark cxceedaIlee, the inspection date, the personnel conducting the Inspection, the selected actions, and the date the selected actions were implemented. On 03/19/12, Outfall SDO-2: TSS = 276 mglL with a benchmark of 100 ing/L. Turbidity upstream was 35.2 NTU, and downstream was 208 NTU. On 10/08/12, Outfall SDO-2: TSS = 194 Ing/L with a benchmark of 100 mg/L. Turbidity upstream was 22.7 NTU, and downstream was 156 NTU. 6. Send all monitoring results for that sample period to the Raleigh Regional Office. Sample results and Qualitative Monitoring Reports will be submitted with an excecdance letter. Tier Two Steps: Two consecutive sampling periods have resulted in concentrations above the Benchmark limits set for TSS and Turbidity. Hanson has instituted Tier Two steps Ior TSS and Turbidity. As a result, the 1,000 gpm pump sprayed spent process water into Crabtree Creek for about 10 minutes before the operator was able to cut the pump and valve "off'. We estimate that 625 gallons went into the creek causing a very slight plume for 300 to 500 feet. No evidence of sedimentation could be found anywhere. We researched conditions further downstream and could not see any trace reaching the first observation point near Oak Park. The creek was flowing very slowly that day due to the lack of rain, but by 3:00 pm, we could find no trace of the wastewater anywhere. Hanson will use training and signage to help prevent such an occurrence from happening again. On March 5, 2015, samples were collected from the SDO-2 outfall, and TSS concentrations were 185 mg/L, and the upstream/downstream results were 18.3 NTU157.8 NTU. Our investigation following receipt of the lab results shows the basin needs cleaning, and the pre -basin measures need repair. Plans are to significantly increase the size of this basin and install a riser with skimmer per our mine permit modification to expand northward. However, until construction of the enlarged basin starts, immediate actions are needed. Hanson has routed all stormwater from the existing basin to drain into the Pit. There are no longer any discharges from SDO-2 because the water now drains into the Pit where it will receive adequate time for sedimentation. Discharges from the Pit are monitored quarterly with the more stringent limits assigned to wastewater. The SDO-2 sediment basin will be cleaned out and the inlet structures refurbished and maintained to minimize sediment entering the Pit. Attached is the Benchmark Exceedance Form with monitoring results. The Crabtree Quarry is now operating under Tier II as required by the permit, including monthly monitoring. The reports will read "No Discharge" as long as the stormwater is diverted to the Pit. NPDES General Permit COC No. NCG020052 Benchmark Exceedance: Stormwater Samples for 2nd 6-Month Period 2010 Dear Manager: Hanson Aggregates is notifying the Department that during the 2nd 6-month period 2011, SDO-2 outfall stormwater samples collected on 916111 resulted in concentrations for Total Suspended Solids (TSS) above the Benchmark stated in the newly issued General Permit. Upstream samples for Turbidity measured 318 NTU, while the downstream sample measured 209 NTU. Our contracted consultant conducted a stormwater management inspection and concluded that the basin upstream of the SDO-2 outfall needed cleaning to better settle out the colloidal material to meet the benchmark for TSS and reduce Turbidity. Erosion control measures will be installed upstream of the basin to provide pre-treatment. The watershed for SDO-2 consists mainly of the gravel stockpile area and it is quite often wetted for dust suppression. Rain allows the dusfto flow into the basin prior the SDO-2 outfall. A landscaper was contracted to create and maintain pathways to the outfalls and beyond to the receiving stream, so that upstream and downstream samples could be collected. It was found that the stormwater exiting the SDO-2 outfall, for the most part, discharges into the pit. Thus, not all rain events result in a discharge from SDO-2. This is a first time exceedance for TSS, and we have initiated steps found in the Tier I Action Plan. Item #6 under Tier One instructions states to include monitoring results for that sampling period. Since there was not a Tier One form included in the newly issued General Permit, I have recorded the sample results on the attached old DMR form.
