Hey Bethany,
We were going through our Certificate of Coverage (s) an noticed another possible error regarding water
classification for Penrose Quarry. The COC indicates class B;Tr water, however the NC Surface Water
Classification Map indicates a "B" classification. Please see attached Penrose Quarry COC and a screenshot of
the NC Surface Water Classifications Map for your reference.
Please lets us know your findings, again as we are working on our revisions for all our SPPP/BMP plans.

Inspection Summary:
Met with Jason Hall for a Multimedia inspection for stormwater and air permits. I also did an inspection of the quarry (mining
permit). The facility is following their SPPP and conducting monitoring as required. The stormwater permit for the concrete
plant and the quarry are the same NCG020408. We will not be requiring that the facility get a separate permit for the quarry.
The facility has a stormwater/washwater treatment/water reuse system. The final 2 basins are located beside the entrance
road to the quarry/concrete plant. This area also has a spring water diversion structure that routes water from slope drains to
a ditch that flows to the first pond (pond is used for dust supression). The next pond is a detention pond for
stormwaterlwashwater and leads to Outfall "B" (pipe installed flowing to the horsepasture river).

Inspection Summary:
Shawna Riddle and Rick Brooks met with Don Mason and Jeff Arnette of Harrison Construction to conduct the inspection.
Employee training has not yet been completed for this year. SPPP is up-to-date. Sampling is being conducted as required
except for Outfall #1. Process water from the crusher is being dumped onto the ground and flowing into the series of ponds
leading to the sampling outfall. Until this has been corrected the sampling for Outfall #1 is wastewater. Outfall #3 is
wastewater. Outfall #002, and SD06 are stormwater. General housekeeping for the areas around the shop and the
secondary contaiment needs to be performed on a regular basis. Continue to monitor and repair the crack in the secondary
contaiment as discussed onsite.

Comment: Facility is considering withdrawing water from French Broad during times of
drought. If they can get landowner permission, it is not an issue for DWQ. Explained
removal rate would be an issue if they withdrew too much of the flow, however FrB is a
large river and this should not be an issue.
OUTFALL- The facility is not using an approved DWQ flocculent. They were informed
that testing had to be submitted for DWQ approval. The facility has requested the
manufacturer produce testing result sufficient for DWQ approval. This will be an issue
for all Vulcan facilities.

work to control stormwater in the short period of time in which Vulcan has owned the site.

continued operation of the subject stone washing wastewater recycle facility.
The Division has received your letter dated October 17, 1995, requesting the return of the subject
permit renewal application. Therefore the Division is hereby returning your application and supporting
documents. Your letter brought to the Division's attention that Permit No. WQ0004824 issued June 19,
1991, is no longer necessary because General Permit Certificate of Coverage NCG020046 issued
September 10, 1993, has been approved to cover the subject recycle facility. Therefore, Permit No.
WQ0004824 is hereby voided effective immediately. Your application fee will be retumed to you under
separate cover.
Please be advised that construction and/or operation of wastewater collection, treatment; and/or
disposal facilities without a valid permit is a violation of North Carolina General Statute 143-215.1 and
may subject Macon Construction, Inc, to appropriate enforcement actions in accordance with North
Carolina General Statute 143-215.6. Civil penalties of up to $10,000 per day per violation may be
assessed for failure to secure a permit required by North Carolina General Statute 143-215.1.
