I) DIRECTIONS TO FACILITY: Facility is located on Stone Quarry Road off US 1, Moncure, Lee
County. Take US 1 South into Lee County. Take Exit 78/Deep River Road and turn right onto Deep River
Rd. Go about a quarter mile and turn Right onto Stone Quarry Road. The facility is at the end of the road.
H) FACILITY DESCRIPTION:, Facility crushes rock which is uted primarily as a roadbed. A hard hat
and safety glasses are recommended for this inspection.
III) INSPECTION SUMMARY: On'July 15, 2015, I, Thad Valentine met with Mr. David Lee,
Environmental Supervisor and Robbie Bogan the plant Superintendent to conduct a compliance inspection
of the Wake Stone-Moncure Quarry. We reviewed the current permit and all of the permit stipulations.
After this review I toured the facility with Mr. Lee and Mr. Bogan and observed the entire operation..

V) SPECIFIC CONDITIONS: AND LIMITATIONS
1 Any,air emission sources oi'control devices authorized'to construct and operate above must be
operated and inaintaihed in accordance with the provisions contained herein. The Permittee shall
comply with applicable�Environmental Management Commission Regulations, including Title
15A North Carolina Administrative Code (NCAC), Subchapter 2D .0202; 2D .0501, 213.0510, 2D
.0521', 2D :0524 (40-CFR 60;Subpart 000); 2D-.0535 and 21) .0540:;,*. In Compliance: The
facility appears to be in compliance 'with all'applicable state and federal regulations.
2. PERMIT RENEWAL AND EMISSION INVENTORY REOUIREMENT - The Permittee, at least
90 days prior to the expiration date of this permit, shall request permit renewal by letter in
accordance with 15A NCAC 2Q .0304(d) and (f). Pursuant to 15A NCAC 2Q .0203(i). * In
Compliance. Mr. Lee was reminded of this permit requirement.
3. EOU7PMENT REPORTING - To d .,
_ emonstrate compliance with 15A NCAC 2D .050.1(c}, the:_:,
Permittee shall maintain on -site an equipment list and a plant,(or flow) diagram of all equipment
covered under this permit. * In Compliance. The equipment list and plant flow diagram were
complete and up to date. Since the last compliance inspection no additional equipment has
been installed or relocated.
4. PARTICULATE CONTROL REQUIREMENT - A$ required by 15A NCAC 2D A510..- .
"Particulates from Sand, Gravel, or Crushed Stone Operations," the following requirements apply:
a. The Permittee of a'sand, gravel, or crushed stone operation shall not cause, allow, ofpermit
any material 'to be produced, handled, transported, 'or stockpiled'without taking measures to
reduce to a minimum any particulate matter from becoming airborne to prevent exceeding
the ambient air quality standards beyond the property line for particulate matter, both PMjo
and total suspended particulates:
b. Fugitive dust emissions from sand, gravel, or crushed stone operations shall be controlled
by 15A NCAC'213 .0540 "Particulates from Fugitive Dust Emission Sources,"
i The Permittee of any.sand, gravel, or crushed stone operation shall control
process -generated emissions From crushers witli wet suppression; and
ii. From conveyors, screens, and transfer points such that the applicable opacity
standards in 15A NCAC 2D .0521 Control of Visible Emissions," or 15A NCAC 2D .0524
"New Source Performance standards" are not exceeded. * In Compliance. Due to the use
of wet suppression all emission points were 0-5% opacity.
This primary crusher is a jaw crusher rated at 1082 tons per hour at a 15 inch opening per the
manufacturer's specificatio"ns' as submitted. Any change froin this crusher' definition, other than
like -for -like, ,will require a permit application to be submitted. The purpose, of this stipulation is to
characterize the run crusher and not limit rod..

"Control of Visible Emissions," visible emissions from the emission sources, manufactured as of
July 1, 1971, shall not be more than 40 percent opacity. ** NIA **

comply with all applicable provisions in 40 CFR 60, Subpart 000.

Dear Mr. Bogan:
On September 14, 2009, I, Vicki Webb, of the Raleigh Regional Office conducted a compliance
evaluation inspection of the subject facility. You represented the facility during the inspection, All of
your help was appreciated as it eased the inspection process.
The following observations were made:
1. The Wake Stone Corp. is permitted to discharge stormwater into the receiving waters designated
as UT of the Deep River, classified WS IV waters in the Cape Fear River Basin.
2. All components of the Stormwater Pollution Prevention Plan (SP3) were present including:
detailed site map with labeled storm outfall, a list of potential contaminants, a spill and response
plan, a narrative describing BMPs, and a contact list of whom to contact in an emergency.
3. EmpIoyee training records were on -site. This facility maintains good housekeeping practices.
4. Qualitative and quantitative inspection records were available for review and appeared complete.
5. The facility contains eight stormwater outfalls. All eight of the facility's stormwater outfalls
were observed during the inspection.
The overall condition of Wake Stone Corp. is compliant with Division standards. If you have any

This writer, of the NC Division of Water Quality, conducted a Stormwater Permit Compliance Evaluation
Inspection of Moncure Quarry, on June 23, 2005. You represented Moncure Quarry during the inspection. Findings are
as follows:
Process and stormwater within the drainage area of the quarry proper is directed to a preliminary solids settling area.
Water from this settling area flows through a gravel filter to the quarry pit sump where additional settling takes place.
Water in the pit sump is pumped as required to either a closed loop recycling system or to the effluent outfall. Quarterly
effluent sampling for the first quarter of 2005 had been performed. All results met permit discharge limitations.
Five stormwater discharge outfalls (SDO) are strategically located around the process area. Gravel filter dams have been
constructed at all of the discharge outfalls with the exception of SDO #3, which is a riprapped swale adjacent to the
entrance roadway. The filter dams appear to be preventing any significant sediment discharge. Annual SDO sampling
and semi-annual qualitative monitoring for the current permit had not yet been performed. (The current permit became
effective on 1/19/05.) The inspection of stormwater BMP's at least every seven calendar days and after storm events, as
required by the permit, is documented.
Within the process area is the closed loop recycling system where water in the process water reservoir is distributed to
rinse screens for product wash. Rinse screen slurry is returned to a process water settling basin where solids are settled
out. Water from the settling basin flows to the process water reservoir. Water from the quarry pit sump is added to the
process water reservoir as required.
Applicability of EPA's Oil Pollution Prevention Regulations as they relate to the Moncure Quarry should be investigated by
Wake Stone Corp. personnel. (This writer noted facility oil and fuel storage in excess of 1320 gallons — the threshold for
which the regulations apply to facilities meeting certain criteria.) There appeared to be a gap around the drain in the
sidewall of the fuel tank secondary containment basin. This potential containment leak source should be checked and
repaired if necessary.

If any parts, measurement frequencies or sampling requirements contained in this permit are
