4a. Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application:*
The UT to Kenneth Creek located at the Sequoia Ridge Drive culvert is eroding. With an almost 90 degree turn near the culvert, the stream bank parallel to the road is undercutting. As
erosion progresses, the stream bank nearest to the the public ROW is in danger of undermining the road. A wooded buffer is present for this stream as Fuquay-Varina required riparian
buffers in the past for the Cape Fear watershed. Adjacent to the project area and on the same parcel, the Town of Fuquay-Varina recently constructed a public sewer parallel to this
stream. Access for stream bank maintenance can be provided off of this public sewer easement. The 0.87 parcel is open space for Parker Pointe Subdivision and is owned by the
Parker Pointe FICA.

In an effort to repair stream bank erosion prevent further slope failure in the vicinity of Sequoia Ridge Drive, The Town of Fuquay-Varina proposes to install approAmately 75 feet of
TXT gabion baskets to repair and armor the stream bank in this area.
4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used:*
The proposed maintenance of stream banks at Sequoia Ridge Drive will consist of installation of approAmately 25 3')G' gabion baskets to repair ongoing stream bank erosion. The
installation of gabion baskets includes foundation preparation, the filling of rip rap, and the backfilling of #57 stone. Once the gabion wall is completed, the stream bank slope will be laid
back and reestablished with straw blankets and grass. A mini -excavator may be utilized from Sequoia Ridge Drive to drive to deliver the rip rap to the gabion installers below and to dig
out the banks as needed for installation. An adjacent Town of Fuquay-Varina public sewer line is also available for access to the the culvert area. Any other areas disturbed as part of
this work will be stabilized immediately following disturbance.

The Town is applying for a 404/401 permit to perform maintenance and repair stream bank erosion. Significant erosion has occurred over the years
due to the developing watershed. The current stream bank is sloughing and is in a failing state. Public safety is considered as the bank erosion
encroaches further into the public ROW. Water quality improvement is a)pected with stabilization of these banks.
