The Division has received your sample results for the 2020 Calendar year. However, a review of the
analytical results for samples collected on January 30, 2020 revealed the violation(s) indicated below:

Based on the above limit exceedances, you should re -sample your effluent for the above parameters. If
the high Ievels persist, then you should have your treatment system evaluated to determine if corrective
measures are needed to ensure the system functions as intended and meets the conditions of the permit,
including complying with the effluent limits. Within 304ays receipt of this letter, please suhutit a

A Notice of Violation is being issued for the noted violations of North Carolina General Statute (G.S.)
143-2I5.1 and the subject General NPDES Permit. Pursuant to G.S. 143-215.6A, a civil penalty of not
more than twenty-five thousand dollars ($25,000.00) may be assessed against any person who violates or
fails to act in accordance with the terms, conditions, or requirements of any permit issued pursuant to G.S.
143-215.1.
