Erosion and sedimentation control measures Will be installed, including the installation of impervious dikes around the existing bridge abutments in order to work in dry conditions. Bridge
demolition will include the removal of the bridge deck and the removal of the top portion of the bridge abutments, leaving the Tooter and approximately three feet of the concrete
abutment to match the excavation elevation and temporary impacts to 85 linear feet to Little Richardson Creek may result. Bridge replacement construction using heavy equipment from
the upland roadway approaches. No deck drains are proposed on the bridge and runoff from the bridge will be routed to inlets located on both sides of the road to the northeast of the
bridge and discharged to the stream floodplain both upstream and downstream of the bridge. The proposed bridge will be built on the same alignment with a temporary off -site detour.
4j. Please upload project drawings for the proposed project.

31. Total stream and ditch impacts:

3i. Total temporary stream impacts:

3j. Comments:
Temporary impacts are associated with the demolition of the eAsting bridge and the overlapping dewatering impacts required to remove the ebsting
bridge abutments in dry conditions for a total of 85 linear feet of temporary impact to Little Richardson Creek.
E. Impact Justification and Mitigation
1. Avoidance and Minimization
la. Specifically describe measures taken to avoid or minimize the proposed impacts in designing the project:
Replace bridge in -place was incorporated to minimize impacts. The bridge span length was a#ended to avoid impacts. Riprap protection of the bridge
abutments is being placed outside of the jurisdictional stream boundary of the perennial stream to avoid impacts. Work will be conducted from the
upland roadway approaches.
1b. Specifically describe measures taken to avoid or minimize the proposed impacts through construction techniques:
NCDOT Best Management Practices for Construction and Maintenance Activities Will be employed. Demolition and construction methods from the
upland roadway approaches will be used to minimize potential temporary demolition related impacts and avoid construction related impacts.
2. Compensatory Mitigation for Impacts to Waters of the U.S. or Waters of the State
2a. Does the project require Compensatory Mitigation for impacts to Waters of the U.S. or Waters of the State?
r Yes r No
2b. If this project DOES NOT require Compensatory Mitigation, explain why:
No permanent impacts are proposed. Minimal temporary impacts of 85 linear feet may occur to Little Richardson from demolition activities and no part of the stream bed will be
impacted. The proposed 69 linear feet and 75 linear feet of dewatering impacts to Little Richardson Creek overlap the 85 linear feet of potential demolition related impacts and will not
require mitigation.
F. Stormwater Management and Diffuse Flow Plan (required by DWR)
1. Diffuse Flow Plan
la. Does the project include or is it adjacent to protected riparian buffers identified within one of the NC Riparian Buffer Protection Rules?
r' Yes r No
If no, explain why:
Project is located in the Yadkin -Pee Dee River Basin and is not subject to NC Riparian Buffer Protection Rules.
2. Stormwater Management Plan
2a. Is this a NCDOT project subject to compliance with NCDOT's Individual NPDES permit NCS000250?*
r Yes r No
Comments:
Stormwater runoff from the proposed bridge will be routed to inlets located on the northeast side of the bridge on both sides of the roadway. Please see attached Stormwater
Management Plan for details on the minimization of water quality impacts.
