Dear Ms. Berger:
Division of Water Resources' Central and Regional staff has reviewed the application package
received December 2, 2019. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on March 13,
2020.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items,
or failure to provide the additional information on or before the above requested date may result in your
application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy.

A. Application (15A NCAC 02T .0105):
1. Address the nitrate exceedances in monitoring well MW-I.
2. Address the possible ditch or washout area within Field 22.
3. Address if Phase II has been constructed. If yes, provide an engineering certification.
B. Site Map:
The drawings provided showed irrigation on the Weatherington property. The drawings were also were
missing several required items. Verify if irrigation is proposed on the Weatherington property. If not,
verify the field acreages of the surrounding irrigation fields. Provide an updated site map in accordance
with 15A NCAC 02T .0105(d) that shows the following:
1. Legend, north arrow, and scale
2. Distance measurements verifying all setbacks are being met
3. Stormwater drainage controls
4. Groundwater lowering systems
5. Location and ownership of property boundaries within 500 feet of the disposal area (including road/rail
right-of-ways)
6. All facility -related structures and fences within the treatment, storage, and disposal areas
7. All habitable residences and places of assembly within 500 feet of all treatment, storage, and disposal sites
8. Location of all wells, streams (ephemeral, intermittent, and perennial), springs, lakes, ponds, ditches, and
other surface drainage features within 500 feet of all waste treatment, storage, and disposal site(s)
9. The wetted perimeter of all irrigation fields with field names (named according to the approved permit)
10. The delineation of compliance and review boundaries
