Within 30 days of receiving this notice, you shall do one of the following two options: 1. Submit civil penalty assessment payment: Payment shall be made in the form of a check, money order, or electronic funds transfer made payable to the North Carolina Department of Environmental Quality in the amount of $378.63. Payment resolves this civil penalty; however, it does not prevent further enforcement actions for continuing or new violations. Submit a request for oral presentation: If you desire to make an oral presentation to the Committee on why your civil penalty remission request meets one or more of the statutory factors, please complete and return the attached form within 30 days of receiving this letter. All documentation shall be submitted to: Division of Water Resources Non -Discharge Branch ATTN: COMPLIANCE OFFICER 1617 Mail Service Center Raleigh, NC 27699-1617 The EMC Chairman will review your oral presentation request and supporting documentation to determine if an oral presentation is warranted. If approved, you will be notified via certified mail of the date, time, and place of your oral presentation.
