NPDES Permit NCS000250 Example Inspection and Maintenance (I&M) Training Location Type:County Maintenance Yard (CMY) Date Inspection 09/04/2016 Performed: Reporter: sujit_ekka LOS Ranking: B - Minor maintenance needs found but functioning properly. ■ Report Type: Inspection ■ Questions:Y N N/A Inlet and outlet drainage systems and flaw bypass structure are structurally sound and functional. Pretreatment BMP and filter strip/swale are structurally sound and functional. Basin, embankment, and emergency spillway (if present) are structurally sound and functional. Emergency outlet control structure, sluice gate, and trash rack are structurally sound and functional if present. All components are free of sediment, leaves, trash, and other debris. Sediment was removed from forebay if capacity was reduced to 500/0 orless. Basin sidewalls and surrounding areas are stabilized with a dense cover of turf grass maintained at a height of 6-15 inches. Basin bottom has adequate cover (4" of sand or dense turf grass 5-15' ill height). Sediment was removed from basin if soil media appeared to be clogged. Areas of erosion have been repaired.
