On February 4, 2020, Brandon Wise and Zac Lentz, inspectors with the North Carolina Department of
Environmental Quality(NCDEQ)—Division of Energy, Mineral and Land Resources, visited the subject site located at
194 Red Marshall Road in Pelham, Caswell County,North Carolina. The site visit and file review revealed that the
facility was not in compliance with the conditions of the National Pollutant Discharge Elimination System (NPDES)
permit NCG 100112. The site currently discharges stormwater affected by processes on site to Tardy Branch and Little
Mill Creek class"C" rivers of the Roanoke River basin.
A Stormwater Pollution Prevention Plan(SWPPP) is required by Part II, Section A of the NPDES NCG100000
permit. The SWPPP is to be created and implemented within a year of permit coverage. This facility has been permitted
since 1998 therefore the SWPPP should have been implemented prior to the inspection. Analytical and Qualitative
(Visual) monitoring are required by Part II Section B & Section C of the NPDES NCG100000 permit. While monitoring
had been completed in the past no monitoring had been completed as of January of 2019. Beginning as soon as possible
monitoring should be completed with a Discharge Monitoring Report being sent to NCDEQ annually. A response noting
the receipt of this Notice along with a copy of a SWPPP should be submitted within 30 calendar days of receipt of this
Notice. It is requested that this submittal be emailed to Brandon Wise at [e-mail], however should a

Thank you for your attention to this matter. This office requires that the violations,as detailed above, be
properly resolved. These violations and any future violations are subject to a civil penalty assessment of up to
$25,000 per day for each violation. Your efforts to undertake activities to bring the subject site into compliance are not
an admission, rather they are actions that must be taken in order to begin to resolve ongoing environmental issues. The
timeliness with which you submit a SWPPP,the degree and extent of harm to the environment, and the duration and
gravity of the violation(s)will be considered in any civil penalty assessment process that may occur. Should you have any
