However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on March 9, 2020. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through D, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Please reference the subject application number when providing the requested information. All revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper copies and one electronic copy submitted to my attention at the address below. Salmon February 6, 2020 Page 2 of 2 A. Signature Authority 15A NCAC 02T .0106 : 1. The Distribution of Class A Residuals (DCAR 06-16) application was signed by Michael P. Wagner, the Public Utilities Director for the City of Goldsboro. According to 15A NCAC 02T .0106(a)(4), the application shall be signed by either an executive officer, elected official in the highest level of elected office, or other authorized employee in the case of a municipal, state, or other public entity. The attached signature authority delegation letter shall be signed in accordance with 15A NCAC 02T .0106(d) to authorize Mr. Wagner to sign this permit application on behalf of the City of Lincolnton. Alternatively, the DCAR 06-16 form shall be signed by a signature authority and resubmitted. B. Residuals Quality Information: 1. The Hazardous Waste Determination section of the Residuals Source Certification (RSC 06-16) form references results from a March 15, 2018 toxicity characteristic leaching procedure (TCLP) analysis from Fairway Laboratories. The results are included, but the laboratory report was not. The laboratory results from the analysis shall be submitted for review. C. Patho yen and Vector Attraction Reduction Reg uirements 115A NCAC 02T .1106 & .1107 : 1. Section 1113 of the RSC 06-16 form indicates that composting is being used as the Process to Further Reduce Pathogens (PFRP) in order to meet the Class A pathogen reduction requirements as listed in 15A NCAC 02T .I 106(a)(3)(E). However, the operational data to verify that this rule is met was not submitted with the application. Operational data shall be submitted that fulfill the following requirements for composting i. The biological residuals shall be composted using either the within -vessel method or the static aerated pile method, during which the temperature of the biological residuals is maintained at 55 degrees Celsius or higher for three consecutive days or longer. Alternatively, the biological residuals shall be composted using the windrow method, during which the temperature of the biological residuals is maintained at 55 degrees Celsius or higher for 15 consecutive days or longer. The windrow shall be turned five times during the period when the biological residuals are maintained at 55 degrees Celsius or higher. Natural decay of the biological residuals under uncontrolled conditions shall not be deemed to comply with these composting requirements 2. Section I1.8 of the RSC 06-16 form indicate that a 14-day aerobic process is being used for Vector Attraction Reduction in accordance with 15A NCAC 02T. 1107(a)(5). However, the operational data to verify that this rule is met was not submitted with the application. Operational data shall be submitted that fulfill the following requirements for a 14-day aerobic process: i. The biological residuals shall be treated in an aerobic process for 14 days or longer. During that time the temperature of the biological residuals shall be higher than 40 degrees Celsius, and the average temperature of the biological residuals shall be higher than 45 degrees Celsius. D. Product Information Sheet: 1. The submitted product information sheet does not include information on all the applicable setbacks in 15A NCAC 02T .I 108(b) or applying residuals at agronomic rates. The information sheet shall be updated to include these required items.
