APPROVAL of 401 Water Quality Certification

Dear Sirs:
You have our approval, in accordance with the attached conditions, to temporarily impact Quankey Creek in Halifax County
for the purpose of constructing a sewer line crossing, as you described in your application received by the Division of Water Quality
on August 1, 2000. After reviewing your application, we have determined that this action is covered by General Water Quality
Certification Number 3288. This Certification allows you to use Nationwide Permit Number 12 when issued by the U.S. Army
Corps of Engineers. In addition, you should get any other federal, state or local permits before you go ahead with your project
including (but not limited to) Sediment and Erosion Control, Non-Discharge and Water Supply Watershed regulations. Also this
approval will expire when the accompanying 404 permit expires unless otherwise specified in the General Certification.
This approval is only valid for the purpose and design that you described in your application except as modified below. If you
change your project, you must notify us and you may be required to send us a new application. If the property is sold, the new
owner must be given a copy of this Certification and-approval letter and is thereby responsible for complying with all conditions.
If total wetland fills for this project (now or in the future) exceed one acre, compensatory mitigation may be required as described
in 15A NCAC 2H .0506 (h) (6) and (7). This approval shall expire when the corresponding Nationwide Permit expires or as
otherwise provided in the General Certification. For this approval to be valid, you must follow the conditions listed in the
attached certification (NW 12).
If you do not accept any of the conditions of this certification, you may ask for an adjudicatory hearing. You must act within
60 days of the date that you receive this letter. To ask for a hearing, send a written petition, which conforms to Chapter 150B of the
North Carolina General Statutes to the Office of Administrative Hearings, P.O. Box 27447, Raleigh, N.C. 27611-7447. This
certification and its conditions are final and binding unless you ask for a hearing.
This letter completes the review of the Division of Water Quality under Section 401 of the Clean Water Act. If you have any

STREAM CROSSING SUMMARY
SANITARY SEWER IMPROVEMENTS
HALIFAX COUNTY

Proiect Description
The purpose of this project is to extend sanitary sewer to serve the Halifax
Industrial Center southwest of Weldon, North Carolina. This project will include
construction of approximately 3,682 LF of 8" ? Ductile Iron Force Main, approximately
1,910 LF 12" ? Sanitary Sewer, and approximately 813 LF 8" ? Sanitary Sewer to serve
the proposed Reser's Fine Foods industrial site. The project is located approximately 5
miles southwest of the Town of Weldon, North Carolina. The total disturbed area of the
project will be approximately 2.0 acres. The included stream crossing will consist of
placing pipe beneath streambed with a minimum of 3.0' of cover.
Stream Crossing Description
The stream crossing is found on Sheet 6 of 10 Construction Plans and consists of
8" ? force main crossing by a 30" ? RCP at Station 31+46.64 within the NC Hwy 903
Right of Way. A minimum of 3.0' of cover will be maintained beneath the existing
streambed and no altering of streambed characteristics will occur during construction.
Also, a maximum of 20' width construction corridor will be maintained throughout the
crossing. The stream to be crossed is an unnamed tributary of Quankey Creek and drains
to the Roanoke River Basin.

Please disregard the information previously submitted for this project. I have also asked
the Army Corps of Engineers to do the same. In replacement, you will find enclosed seven (7)
copies of each of the following items pertaining to the above referenced project:
• Joint Form for Nationwide Permit No. 12 - Revised to contain additional information
• Revised Project Narrative describing method of construction and stream and wetlands
crossing locations
• 8 t/2" x 11" copy of Sheet 4,5 and 6 of 10 Construction Plans showing the proposed
wetlands crossings. Also, please find 7 copies of existing crossings made by 6"
diameter force main off of I-95. As shown, the existing 6" line will be paralleled by
8" diameter force main in proposed construction.
• 8 t/2" x 11" copies of Sheet 4 of 10 and Sheet 5 of 10 Construction Plans in their
entirety for reference
• 81/2" x 11" copies of Quadrangle Maps showing all stream crossing locations
